Bordetella trematum bacteraemia secondary to an empyema in an immunocompromised host: A case report and review of the literature.

INTRODUCTION: Bordetella trematum infection remains uncommon. More cases of bacteraemia are reported in recent years with the primary infection largely originating from skin and soft tissue sites. Yet, our understanding of its virulence, antibiotic susceptibility profile and treatment is still limited. CASE PRESENTATION: Case presentation. We report the first case of B. trematum bacteraemia from a left-sided empyema. An 87-year-old female patient with a past medical history of ischaemic heart disease, diabetes mellitus complicated by nephropathy and locally advanced left breast adenocarcinoma

Comparison of Different Selection Strategies for Tolvaptan Eligibility among Autosomal Dominant Polycystic Kidney Disease Patients.

BACKGROUND: Tolvaptan can slow down renal function decline in autosomal dominant polycystic kidney disease (-ADPKD). While there is consensus across international recommendations that the drug should only be used in patients with high risk of rapid progression, identification criteria for rapid progression vary. Here, we investigated different assessment strategies using a real-life ADPKD cohort. METHODS: Observational retrospective cohort analysis. The study included 131 ADPKD patients aged 19-78 years who were referred to the Hannover Medical School outpatient clinic for evaluation of tolva

Dual left anterior descending artery - a clinical entity in search of an identity: A novel classification.

A dual left anterior descending artery (LAD) is not an uncommon anomaly of the coronary arterial system. The earlier classifications by various authors lacked standardization and uniformity and are too complex for clinical usage. Therefore, a new classification in the form of coding system is proposed to facilitate the identification and reporting of the anomaly by the cardiologists to maintain uniformity in reporting, which aids in adequate planning of the revascularization strategies in patients with symptomatic coronary artery disease involving the dual LAD system.

Intraoperative facial nerve monitoring: a description of a unique system.

Intraoperative facial nerve monitoring has become an integral adjunct in facial nerve identification and preservation for patients undergoing cerebellopontine angle surgery. Since the first description of EMG monitoring of facial nerve activity intraoperatively, many systems have been developed. These systems often rely on unilateral monitoring of the facial nerve with auditory feedback to the surgeon, and it is difficult to distinguish between artifact and significant stimulation of the facial nerve. In this paper, we present the use of a bilateral, multialarm, facial nerve monitoring system

Chronic otitis media due to Auritidibacter ignavus: a two-case series and brief literature review.

INTRODUCTION: Auritidibacter ignavus is an emerging Gram‑positive bacillus with otic tropism linked to recurrent and refractory ear infections. METHODS: We describe two cases of chronic otitis media seen in 2024 at a Spanish tertiary hospital. Ear discharge was cultured on standard media; identification was achieved by MALDI‑TOF and susceptibility interpreted using EUCAST criteria for Corynebacterium spp. given the lack of organism‑specific breakpoints. RESULTS: Two octogenarian patients with persistent otorrhea after failed topical therapy grew A. ignavus. Isolates were susceptible to selec

Autoimmune Encephalitis: A Hidden Enigma Demystified.

Autoimmune encephalitis is a condition marked by inflammation in the brain due to an immune system response targeting self-antigens within the central nervous system (CNS). This class of disorders is at least as prevalent as infectious causes of encephalitis and encompasses a wide range of conditions. The field has rapidly expanded thanks to the identification of various pathogenic autoantibodies responsible for varied neurological and neuropsychiatric diseases. These disorders often present with distinct cognitive, seizure, and movement disorder phenotypes, making them clinically identifiable

Fracture-Related Infection of the Proximal Femur - Diagnostics and Treatment.

PURPOSE: With the aging population and rising life expectancy the incidence of trauma-related injuries, particularly proximal femur fractures, is expected to increase. Complications such as fracture-related infections (FRI) significantly impede the healing process and pose substantial risks to patients. Despite advancements in understanding, diagnosing, and treating FRI, challenges persist in achieving optimal outcomes. This review addresses the significance of FRI following proximal femur fractures, emphasizing diagnostic methodologies and therapeutic modalities to enhance clinical care. FIN

A case of hypoplasia of internal carotid artery and intracranial vasculopathy with Moyamoya syndrome in association with Alagille syndrome.

Alagille syndrome (ALGS), also known as arteriohepatic dysplasia, is a rare multisystem vascular disorder affecting brain, liver, heart, ophthalmic and skeletal systems. Moyamoya syndrome is a rare arteriopathy due to an underlying cause that can lead to ischemic and hemorrhagic strokes. Here, we report a rare case of ALGS in a patient with congenital narrowing of bilateral internal carotid arteries (ICA) with superimposed intracranial arteriopathy and associated moyamoya syndrome. A 34-year-old female presented with progressively worsening intermittent right eye visual blurriness, associated

Operationalizing patient flow in high-complexity service lines: a health system perspective using oncology care.

BACKGROUND: Health systems face increasing oncology demand amid workforce constraints and rising care complexity. Outpatient cancer volumes in the United States are projected to grow nearly 18% over the next decade, driven by earlier diagnoses, evolving screening guidelines, and continued site-of-care shifts - while inpatient volumes remain essentially flat but grow in case mix complexity. Many organizations attempt to improve access through isolated interventions - navigation programs, clinic expansion, or scheduling redesign - yet delays in treatment initiation and operational inefficiencies

Boosting predictive models and augmenting patient data with relevant genomic and pathway information.

The recurrence of low-stage lung cancer poses a challenge due to its unpredictable nature and diverse patient responses to treatments. Personalized care and patient outcomes heavily rely on early relapse identification, yet current predictive models, despite their potential, lack comprehensive genetic data. This inadequacy fuels our research focus-integrating specific genetic information, such as pathway scores, into clinical data. Our aim is to refine machine learning models for more precise relapse prediction in early-stage non-small cell lung cancer. To address the scarcity of genetic data,

An automated diagnostic system of polycystic ovary syndrome based on object growing.

OBJECTIVE: Polycystic ovary syndrome (PCOS) is a complex endocrine disorder that seriously affects women's health. The disorder is characterized by the formation of many follicles in the ovary. Currently the predominant diagnosis is to manually count the number of follicles, which may lead to inter-observer and intra-observer variability and low efficiency. A computer-aided PCOS diagnostic system may overcome these problems. However the methods reported in recently published literature are not very effective and often need human interaction. To overcome these problems, we propose an automated

Neutrophilic panniculitis in infancy: a cutaneous manifestation of juvenile rheumatoid arthritis.

Neutrophilic panniculitis is rare and is classified as a panniculitic member of the neutrophilic dermatoses spectrum. In affected patients, an underlying systemic disease, such as myelodysplasia, is often present. We describe an infant with juvenile rheumatoid arthritis who developed neutrophilic panniculitis. Neutrophilic panniculitis clinically mimics other panniculitides and biopsy specimen can be diagnostic. Identification of this entity can aid the diagnosis of the underlying systemic process.

[Reading numbers in pure alexia: effects of the task and hemispheric specialization].

Selective conservation of the ability to read Arabic numbers in patients unable to read words or even letters is a classical characteristic of pure alexia described by Dejerine (1982). We report our work on the capacity of two patients with pure typical alexia to process numbers. Our main finding was that these patients could count pairs of Arabic numbers correctly when the reading task was simple (example 2 4-->"two four") or when the task involved comparing sizes (example 2 4-->"four is bigger than two"). Inversely, these patients often made mistakes when asked to perform arithmetic operatio

Disseminated melioidosis-challenge to routine antibiotic therapy: a case report.

INTRODUCTION: Melioidosis caused by Burkholderia pseudomallei, often referred to as a great mimicker or escapist, evades not only the immune system, but also all manual identification methods in an under-equipped clinical microbiology laboratory due to its tedious identification process. This is a case report of disseminated melioidosis with septic arthritis, misdiagnosed both clinicoradiologically and microbiologically as disseminated tuberculosis or other bacterial infection. CASE HISTORY: A middle-aged Asian diabetic male presented with high-grade fever and breathlessness for 4 days along

[Surviving Sepsis Campaign guideline update:problems and progress].

Sepsis is a clinical syndrome, the core of which is the disorder of the host's response to infection. Sepsis progresses to septic shock, which can lead to increased mortality, so early identification and standardized management are particularly important. Over the past 30 years, the definition of septic shock has gradually changed, with efforts to improve the identification and screening of early septic shock, and to emphasize the importance of organ dysfunction in patients with septic shock. The birth of the rescue systemic infection movement has played an important role in the management of
