Primary carcinoma in situ of the ureter or renal pelvis is rare. We describe 3 patients, each of whom had a different mode of presentation. When malignant cells in the urine from the upper urinary tract are associated with urographic evidence of an appropriate lesion, aggressive surgical therapy is indicated. In the absence of such a urographic abnormality patients with positive cytologic examinations should be followed closely because the exfoliated cells usually are from poorly differentiated neoplasms.

The authors describe a case of tumour of transitional cells in patient with a horseshoe-shaped kidney, the suspected diagnosis of which was reached with the aid of a selective cinearteriography. They stress the low incidence of the same in the literature consulted which reveals the importance of studying the patients with this malformation in an attempt to prevent complications and in particular, to make an early diagnosis of a possible malignant transformation.

2 cases of leucoplakia of the renal pelvis are reported. In the first one the diagnosis was established after nephrectomy and histological examination of the specimen. In the second one it was suspected on urine examination and was confirmed on histology. The current aspects about its cause and the difficulties concerning its diagnosis and treatment are discussed.

A case of carcinoma in situ of the renal pelvis, diagnosed by exfoliative cytology from ureteral specimens and treated by nephroureterectomy, is reported. Urothelial carcinomas in situ are very rare in the renal pelvis, but an increasing number in the bladder have been diagnosed. Urinary cytology plays the most important role in the diagnosis of carcinoma in situ of the renal pelvis and may be used as a screening test of patients exposed to known carcinogens.

Thw radiological appearances found in the pelvis and hip joints in 79 patients with cerebral palsy, aged 5--16 years, have been studied. These findings have been recorded and correlated with the clinical severity and distribution of the disease. Subluxation of the hip joints was found in 14% and dislocation in a further 6%. Selective renal arteriography was done in 5 patients with transitional cell carcinoma of the renal pelvis and upper ureter. In 2 cases, the presence of tumor vessels was unequivocally diagnostic. In the remaining 3 cases, the arteriographic changes were not as prominent but were suggestive of neoplasm and were helpful in differentiating other causes of filling defects in the collecting system.

The angiographic criteria of 19 renal pelvic carcinomas were analysed. Two groups of signs, direct and indirect, could be detected. Of 19 histologically proved cases of renal pelvic carcinomas, the diagnosis had not been made angiographically in the case of a small tumor, which was 11 mm in diameter. The usefulness of preoperative angiography for diagnosis and operative planning is emphasized.

A case of transitional cell carcinoma of the renal pelvis in a 4-month-old japanese boy is reported. This patient is the youngest one described in the literature with a tumor of this type. The differential points of Wilms tumor and other types of neonatal renal tumors are discussed.

Twenty-two cases of renal pelvic carcinoma are reviewed, of which nine were squamous cell and 13 were transitional cell carcinomas. No difference in symptoms between the two types was seen except for an increased incidence of a palpable mass and weight loss among patients having the squamous cell variety. Thirteen of the 22 patients have died of renal pelvic carcinoma. Reasons for the low survival rate are discussed. Treatment of transitional cell carcinoma with nephroureterectomy and local lymph node dissection is recommended.
