This review aims at describing and comparing materials commonly used in root canal therapy, the cytotoxicity, histopathological, microbiological and clinical aspects ofa iodoform-based paste (Guedes-Pinto Paste-GPP) used in endodontic treatment of primary teeth. GPP has shown excellent biocompatibility to pulp fibroblasts and mild inflammatory reactions, having been well-tolerated by the periapical and connective tissues. Moreover, GPP bactericidal and bacteriostatic effects against many oral microorganisms were also demonstrated. Regarding clinical trials, the GPP technique has achieved succe

Granuloma formation includes an immune response in oral tissues to various microorganisms and their products. The immunocompetent cells of both series (T and B) are present in the periapical lesions. In order to further analyze the relative contribution and pathophysiological significance of the T cell subsets in granuloma formation, we undertook the quantitative analysis of the CD3-positive, CD4-positive, CD8-positive and Ig-positive cells in these lesions by using indirect immunofluorescence. Evidence is provided showing predominance of T cells in diffuse and B cells in focal mononuclear inf

Apical periodontitis is initiated primarily by the mixed microflora of infected root canals. Continuous flow of bacteria and their products through the apical foramen induces influx, activation and coordinated interaction of immune-inflammatory cells within the periapical area. Successful mobilization of host defense mechanisms prevents abundant extraradicular bacterial invasion. However, anti-infective effector mechanisms are not restricted to killing the invading microorganisms but also destroy normal tissue components and induce bone absorption, resulting ultimately in the loss of the affec

BACKGROUND: Non-syndromic tooth agenesis (NSTA) is a type of ectodermal dysplasia (ED) in which patients with non-syndromic oligodontia may only affect teeth. No pathological findings were found in other tissues of the ectodermal. Herein, we report a case of a NSTA patient with severe dental anxiety and poor oral health.
CASE PRESENTATION: A 5-year-old boy without systemic diseases presented as a patient with oligodontia, extensive caries, and periapical periodontitis. Molecular genetic analysis found a mutation in the Ectodysplasin A (EDA) gene, confirming the diagnosis of NSTA.
CONCLUSION:

Iatrogenic complications in endodontics are not irrelevant. One of the most seen is overfilling of the root canal, which has a negative effect on prognosis for endodontically treated teeth. Although more than a half of the overfilled teeth heal satisfactorily after proper endodontic therapy, in some cases, such as injury of the inferior alveolar nerve or presence of filling material in soft tissues or sinus spaces, surgical procedure is more suitable and effective. The purpose of this article is to present 2 clinical cases of iatrogenic complications of endodontically treated teeth and the app

AIM: To evaluate the expression of genes involved in chronic pain conditions in apical periodontitis (AP) tissues.
METHODOLOGY: An electronic search was performed in Scopus and MEDLINE (via PubMed) databases to identify articles (n = 173) related to genes involved in chronic pain conditions. Full-text reviews of the selected articles allowed the prioritization of 16 genes to be investigated with regards to their expression in AP tissues. Periapical lesions (n = 42) were collected during surgical endodontic procedures and processed for mRNA extraction and investigation of target genes via RT-q

Radiographic observations show that periapical replacement resorption (PARR) is a frequent and unpredictable sequella of orthodontic movement of permanent maxillary incisors. Only the apical root portion undergoes resorption, and it is subsequently replaced with normal bone. PARR also occurs without orthodontic tooth movement, but these resorptions differ as to type and location. PARR clearly occurs in teeth with vital pulps after orthodontic movement; however, our two cases and a review of 43 other cases indicate that endodontically treated incisors show a statistically significant lesser fre

The use of the calcium hydroxide-containing root canal sealer, Sealapex, has been questioned regarding its predisposition to solubility and subsequent effect on the root canal seal and periradicular healing. This case report demonstrates, radiographically, dissolution of excess sealer in the periradicular tissues with subsequent osseous repair which was similar to healing in periradicular tissues where there was no extruded sealer.

Several studies have reported devitalization of teeth following surgical procedures in the area of the root apices. The purpose of this investigation was to observe the 1-4-week wound healing pattern in monkey dental tissues following intentional vital apicoectomy (IVA). Two adult Macaca mulatta were used in this study. All of the teeth were surgically transected within the apical one-third with a bur. The soft tissues were then closed. At death, a maxillary and a mandibular quadrant for 1, 2, 3 and 4 weeks following surgery were obtained after vascular perfusion. Each quadrant was demineraliz

Periapical periodontitis (AP) is an inflammatory disease caused by persistent endodontic biofilms, and its treatment remains challenging because conventional root canal therapy sometimes fails to achieve effective disinfection and tissue regeneration. Herein, a multifunctional bimetallic ion-modified metal-organic framework (MOF) nanozyme composite, MOF-FeAg-AMP@HA, was engineered for synergistic AP therapy. Incorporation of Fe3+ and Ag+ ions into a UiO-67 MOF scaffold endowed the nanozyme with pH-responsive dual enzymatic activities. Under acidic inflammatory conditions, strong peroxidase (PO

AIM: To compare 1-year clinical and radiographic outcomes of narrow- versus wide-diameter tissue-level implants for single molar replacement.
MATERIALS AND METHODS: This two-arm randomised controlled trial enrolled 20 patients (23 implants) in one sextant. Participants received either 3.5-mm narrow-diameter implants (NDI, n = 12) or 5.0-mm wide-diameter implants (WDI, n = 11) using a tissue-level design with computer-guided surgery. The primary outcome was marginal bone loss (MBL) at 1 year assessed by periapical radiographs. Secondary outcomes included operation time, pre-allocation analysis

PURPOSE: Atrophic ridges usually have both hard and soft tissue shortages, and bone augmentation can make soft tissue closure difficult. The aim of this case series is to present a new method utilizing transfer of free fat tissue grafts (FFG) from the buccal fat bad (BFP) during bone augmentation to enhance immediate primary soft tissue closure and improve long-term soft tissue thickness and quality at the recipient site.
MATERIALS AND METHODS: Patients with moderate to severe ridge atrophy in different regions of the jaws were treated over a 4-year period with bone augmentation and FFG. The

Tooth autotransplantation is a procedure in which a donor tooth is transplanted within the same patient's jaw to replace a missing tooth. Donor tooth root morphology and periodontal ligament integrity are key factors influencing success. We report a semi-immediate autotransplantation of a maxillary third molar with a 90° divergent root into a site previously affected by a periapical abscess. After the extraction of the compromised tooth (#15) and removal of the interradicular septum, the site was left to heal to allow soft tissue closure and infection resolution. During this period, CBCT imagi

INTRODUCTION: Revitalization procedures have been widely used for the treatment of immature permanent teeth with apical periodontitis. The treatment procedures appear to be capable of encouraging continued root development and thickening of the canal walls. The nature of tissues formed in the canal space and at the root apex after revitalization has been shown histologically in several animal studies; similar studies in humans were recently reported.
METHODS: A 9-year-old boy had a traumatic injury to his upper anterior teeth. Tooth #9 suffered a complicated crown fracture with a pulp exposur

AIM: To present two successful cases involving management of endodontic-periodontal combined lesions with root resection and guided tissue regeneration (GTR) therapy.
SUMMARY: Mesiobuccal resection surgery was performed concomitant with GTR therapy to manage endodontic-periodontal combined lesions on teeth 17 and 26. The increased mobility after the periodontal surgery gradually decreased during the healing period. At 15 and 20 month follow-ups, respectively, the teeth showed no mobility, 2-3 mm pocket depths and no further bone loss.
KEY LEARNING POINTS: Treatment of periodontally involved
