Case report: successful retrieval of an entrapped balloon during balloon pericardiotomy after rupturing.

BACKGROUND: Balloon rupture and entrapment during percutaneous balloon pericardiotomy (PBP) is rare and usually requires surgical intervention. However, various bailout techniques can be tried. We describe a case of balloon entrapment in the pericardium and the strategies employed to successfully remove the balloon percutaneously. CASE SUMMARY: A 58-year-old woman presented with recurrent malignant pericardial effusion. She refused surgery and opted for PBP. During the procedure, the balloon ruptured and was trapped within the pericardium. The entrapped balloon was secured by snaring onto the…

Partial Pericardial Agenesis.

Congenital absence of the pericardium is a rare anomaly, affecting the left pericardium (86%) more than the right, with male predilection distribution (3:1). In the majority of cases, the condition is asymptomatic. We describe a case of a 55-year-old female with a history of chronic hypercapnic respiratory failure secondary to restrictive lung disease who was referred to cardiovascular magnetic resonance (CMR) lab for shunt evaluation based on right ventricular pressure overload and paradoxical septal motion.

Chest complications of amoebic liver abscess: a report of six cases from Nigeria.

During a seven year period, six patients with intra-thoracic rupture of amoebic liver abscesses required surgical intervention. Of the six cases, three ruptured into the right pleural space, two into the right lower lobe and one into the pericardium. Definitive surgical procedures performed were decortication and repair of the diaphragm in three, segmental lung resection and repair of the diaphragm in two cases and continuous subxyphoid catheter drainage of the pericardium in the remaining case. There were no death and the morbidity was insignificant.

The Importance of Complete Pericardiectomy and the Role of the Apical Suction Device in Chronic Constrictive Pericarditis.

BACKGROUND: The aim of this study was to analyze the preoperative attributes and clinical impacts of complete pericardiectomy in chronic constrictive pericarditis. METHODS: A total of 26 patients were treated from January 2001 to December 2013. The pericardium was resected as widely as possible. When excessive bleeding or hemodynamic instability occurred intraoperatively, a cardiopulmonary bypass (CPB; n=3, 11.5%) or an apical suction device (n=8, 30.8%) was used. Patients were divided into 2 groups: those who underwent ≥ 80% resection of the pericardium (group A, n=18) and those who underwen…

Effects on drainage of the mediastinum and pleura during negative pressure wound therapy when using a rigid barrier to prevent heart rupture.

Right ventricular heart rupture is a devastating complication associated with negative pressure wound therapy (NPWT) following cardiac surgery. The use of a rigid disc has been suggested to offer protection against this lethal complication by preventing the heart from being drawn up towards, and damaged by, the sharp sternum edges. The aim of the present study was to compare the wound fluid evacuation from the pericardium and the left pleura when using NPWT with such a disc between the sternal edges and the heart, and when using conventional NPWT. Six pigs underwent median sternotomy followed …

Truly stentless autologous pericardial aortic valve replacement: an alternative to standard aortic valve replacement.

OBJECTIVE: The aim of this study was to determine the feasibility and durability of truly stentless aortic valve replacement using autologous pericardium sutured directly onto the aortic wall. METHODS: Eleven patients (mean age, 55.9 years) requiring aortic valve replacement were recruited. A circular piece of pericardium about 8 cm in diameter was harvested and treated in 0.6% glutaraldehyde for 10 minutes. The aortic valve was excised and, with the use of specially designed instruments (CardioMend LLC, Santa Barbara, Calif), the sinotubular junction was sized and the pericardium was tailore…

[Forensic expertise of thoracic aorta, heart and pericardial injuries in car-occupant fatalities].

INTRODUCTION: Forensic expertise has not specified with certainty any specific injury among fatally injured frontal car-occupants in frontal car collisions. OBJECTIVE: To establish if blunt injuries of thoracic aorta, heart, and pericardium could be used as predictors where the fatally injured car-occupant was at the moment of car-collision. METHODS: A retrospective autopsy study was performed. The subjects were fatally injured car-drivers, front-seat and rear-seat passengers in head-on car collisions. In each of them we analyzed the injuries of thoracic aorta, heart and pericardium. RESULT…

The impact of pericardial disruption on heart function and remodeling after myocardial infarction in mice.

Myocardial infarction (MI) remains a leading cause of deaths globally. Thus, reliable and relevant animal MI models are crucial to understand underlying disease mechanisms and develop new therapies. Generally, MI in rodents is induced by ligation of the left anterior descending artery (LAD) leaving a ruptured pericardium. This contrasts MI in man, where the pericardium remains intact. Herein, we therefore determined in adult mice whether pericardial disruption along MI influences left ventricular (LV) heart function, remodeling, and scar formation. Baseline heart function was assessed in 30 C5…

[Constrictive Pericarditis Associated with Pericardial Hematoma:Report of a Case].

A 59-year-old man was admitted to our hospital due to keto-acidosis. Electric cardiogram showed history of myocardial infarction. Cardiac echogram showed severe left ventricular hypokinesis, thickened pericardium and pericardial effusion. Right ventricular pressure curve showed dip and plateau pattern, and coronary angiography showed severe three vessel disease. So, we diagnosed with constrictive pericarditis accompanying pericardial effusion and ischemic heart disease. After cardiac catheterization, as the hemodynamics was getting worse, we inserted intraaoric balloon pumping (IABP) immediate…

[Superior vena cava replacement with autologous pericardial conduit in redo cardiac surgery].

BACKGROUND: Iatrogenic lesions needing repair can occur during redo cardiac surgery. Lacerations of the superior vena cava in redo cardiac surgery have been underreported. There is still debate around what prosthesis is best to replace the superior vena cava. A case of superior vena cava replacement for iatrogenic injury in a case of redo mitral valve surgery is reported. Use of fresh autologous pericardium to create a total conduit restoring the continuity of the superior vena cava is described. CLINICAL CASE: 54-year-old female with previous cardiac surgery undergoing prosthetic mitral valv…

A cardiac hydatid cyst mimicking a pericardial tumour in a paediatric case.

Cardiac hydatid cysts are a rare presentation of hydatid cyst disease in the body, with a reported cardiac involvement rate of <2%. The left ventricle is the most common site of cardiac involvement. Here, we report a patient with a hydatid cyst that ruptured into the pericardium after producing an aneurysm on the right ventricular free wall, appearing as fibrinated fluid and a solid mass lesion in the pericardium. Our aim in this case report was to emphasise that the possibility of a hydatid cyst should not be overlooked in the differential diagnosis of pericardial tumours.

Congenital partial pericardial defect in a neonate: a case report.

This report describes a neonate with congenital partial absence of the pericardium associated with hypoplastic left heart syndrome. The patient presented with severe heart failure caused by a hypoplastic left ventricle. The infant died at 15 hours of age because of intractable congestive heart failure. Necropsy revealed, in addition to a hypoplastic left ventricle, a transverse biventricular constriction suggestive of partial absence of the pericardium. In addition to cardiovascular malformations, a left hydronephrosis and a bicornuate uterus was found. We report a rare clinical entity and rev…

Magnetic resonance imaging of cardiac and paracardiac masses.

Magnetic resonance imaging (MRI) with ECG-gated acquisition displays the blood pool as a signal void and thereby provides high contrast differentiation between cardiovascular structures and soft-tissue masses. The role of MRI for the detection and definition of the extent of paracardiac and intracardiac masses is reviewed. The extension of mediastinal, lung, and upper abdominal tumors to the heart and pericardium is depicted favorably by MRI and this attribute is also demonstrated. It is anticipated that MRI will have an increasing role in the evaluation of primary and secondary masses of the …

In search of the best xenogeneic material for a paediatric conduit: an experimental study.

OBJECTIVES: The development of calcification-resistant bioprosthetic materials is a very important challenge for paediatric surgery. The subcutaneous implantation in rats is the well-known first-stage model for this kind of research. Using this model, we aimed to compare calcification of the porcine aortic wall and bovine pericardium and jugular vein wall cross-linked with glutaraldehyde (GA) and ethylene glycol diglycidyl ether (DE). We also determined the efficacy of DE-preserved tissue modification with 2-(2-carboxyethylamino)ethylidene-1,1-bisphosphonic acid (CEABA). METHODS: Three groups…

Three-month outcomes of aortic valve reconstruction using collagenous membranes (biosheets) produced by in-body tissue architecture in a goat model: a preliminary study.

BACKGROUND: Autologous pericardium is widely used as a plastic material in intracardiac structures, in the pulmonary artery, and in aortic valve leaflets. For aortic valve reconstruction (AVRec) using the Ozaki procedure, it has produced excellent clinical results over a 10-year period. In-body tissue architecture (iBTA), which is based on the phenomenon of tissue encapsulation of foreign materials, can be used to prepare autologous prosthetic tissues. In this preliminary study, we examined whether biosheets can be used as valve leaflet material for glutaraldehyde-free AVRec by subchronic impl…

Squamous cell carcinoma developed in a chronic radiation-induced chest wall ulcer that is difficult to undergo thorough preoperative histological examination.

INTRODUCTION: We report a case in which squamous cell carcinoma (SCC) developed in a large chronic radiation-induced thoracic ulcer after flap surgery in areas where preoperative histological examinations are difficult. PRESENTATION OF CASE: The patient was a 75-year-old female. She had undergone resection and radiotherapy for left breast cancer 15 years earlier. Six years ago, the ulcer expanded from the subclavian to xiphoid levels, exposing the lung and pericardium. A histopathological examination, which avoided the lung and pericardium, was performed. Inflammation was diagnosed. We recons…

Concomitant occurrence of IgG4-related pleuritis and periaortitis: a case report with review of the literature.

IgG4-related sclerosing disease is an established disease entity with characteristic clinicopathological features. Some recent reports have demonstrated that this disease can occur in the respiratory system including the pleura. Herein, we describe the first documented case of concomitant occurrence of IgG4-related pleuritis and periaortitis. A 71-year-old Japanese female with a history of essential thrombocythemia presented with persistent cough and difficulty in breathing. Computed tomography demonstrated thickening of the right parietal pleura, pericardium, and periaortic tissue and pleural…
