Influencing factors of outcome after lower-limb amputation: a five-year review in a plastic surgical department.

The crude major lower limb amputation procedure rate is 8.8 per 100,000 of the population per year in Taiwan. From January 2002 to October 2006, patients that received major lower limb amputation in our department were enrolled in this study. Retrospective chart reviews concerning different factors that can affect the eventual postoperative functional status were investigated. Factors that affected the length of hospital stay included duration before amputation (P < 0.001) and renal function (P = 0.045). Phantom limb pain was affected by wound healing time (P = 0.006). Factors that affected th…

Effect of Adding Virtual Reality Training to Traditional Exercise Program on Pain, Mental Status and Psychological Status in Unilateral Traumatic Lower Limb Amputees: A Randomized Controlled Trial.

Background: Lower limb amputation is an emotionally devastating condition that causes a complete change in the quality of life, may lead to phantom limb pain in most of the cases, and puts the individual in a high risk of developing psychological disorders. The objective of this study is to evaluate the consequence of adding virtual reality (VR) to a traditional exercise program on pain, mental status, and psychological status in traumatic unilateral lower limb amputees (LLAs). Methods: Thirty-two traumatic LLAs were randomly assigned into two equal groups in this randomized control trial. Par…

Possible inflammatory pain biomarkers in postamputation pain.

Although chronic pain after amputation is frequent, the underlying mechanisms are still not well understood. It is widely accepted that the pathogenesis of postamputation pain is multifactorial, with both peripheral and central mechanisms playing an essential role. However, recent studies suggest that the immune system plays an important role in different neuropathic pain conditions, including postamputation pain. Eleven amputees were included in this clinical study. Information on the type and intensity of spontaneous postamputation pain was obtained and evoked pain responses for brush, cold,

Amputation for vascular disease.

Amputation for peripheral ischaemia still has a depressingly high early and late mortality, and morbidity and the end result are usually less than satisfactory. Individual surgeons probably see too few amputees to treat them with maximal efficiency, and these patients create a large burden on beds and resources. There is room for improvement in all aspects of our management of amputees. Primary healing rates might be better with less heroic attempts to obtain a distal amputation. Sepsis is lessened by the use of prophylactic antibiotics. Tight bandaging and the intra-operative fitting of prost

Ultra-low-dose CT reconstructed with the artificial intelligence iterative reconstruction algorithm (AIIR) in 18F-FDG total-body PET/CT examination: a preliminary study.

PURPOSE: To investigate the feasibility of ultra-low-dose CT (ULDCT) reconstructed with the artificial intelligence iterative reconstruction (AIIR) algorithm in total-body PET/CT imaging. METHODS: The study included both the phantom and clinical parts. An anthropomorphic phantom underwent CT imaging with ULDCT (10mAs) and standard-dose CT (SDCT) (120mAs), respectively. ULDCT was reconstructed with AIIR and hybrid iterative reconstruction (HIR) (expressed as ULDCT-AIIRphantom and ULDCT-HIRphantom), respectively, and SDCT was reconstructed with HIR (SDCT-HIRphantom) as control. In the clinical

Phantom Perception of Sound and the Abnormal Cortical Inhibition System: An Electroencephalography (EEG) Study.

OBJECTIVES: Despite no observable external sound present, a perceived feeling of a recurrent unpleasant sound is a main complaint in the patients with chronic tinnitus. This phantom perception of sound is considered as the auditory equivalent of phantom limb pain, and altered excitability may be involved in its underlying pathology. Tinnitus-related hyper-excitation is suppressed by inhibitory repetitive transcranial magnetic stimulation (rTMS). However, the neural mechanism underlying the treatment is not fully understood, and quantifying the suppression induced by rTMS has yet to be consider…

Centrocentral anastomosis of the proximal nerve stump in the treatment of painful amputation neuromas of major nerves.

The term "centrocentral anastomosis" is used to describe the end-to-end connection across interposed nerve grafts between paired fascicular groups of the proximal stump of a severed nerve. In 22 patients harboring a painful terminal neuroma following amputation of a lower limb (20 neuromas on the sciatic nerve and two on the peroneal nerve), a centrocentral anastomosis was performed on the end of the sectioned nerve to treat pain that had not improved with conventional conservative treatment. Follow-up review at 1 year revealed that the typical neuroma pain had disappeared in all cases, althou

Assessment of Lower Limb Muscle Volume Using 3D Ultrasonography: Validity and Reliability Compared to MRI.

INTRODUCTION: Muscle volume is a key indicator of strength and neuromuscular health, commonly assessed via Magnetic Resonance Imaging (MRI). While accurate, MRI is expensive and time-intensive. Three-dimensional ultrasonography (3DUS) offers a more accessible alternative but requires validation due to its setup-dependent accuracy. This study investigated the validity and reliability of a custom 3DUS setup for measuring lower limb muscle volumes. METHODS: Fifteen participants (8 female; 18-40 years) underwent two 3DUS and one MRI sessions. The tibialis anterior, vastus lateralis, gastrocnemii,

Perioperative intravenous ketamine infusion for the prevention of persistent post-amputation pain: a randomized, controlled trial.

We hypothesized that perioperative ketamine administration would modify acute central sensitization following amputation and hence reduce the incidence and severity of persistent post-amputation pain (both phantom limb and stump pain). In a randomized, controlled trial, 45 patients undergoing above- or below-knee amputation received ketamine 0.5 mg x kg(-1) or placebo as a pre-induction bolus followed by an intravenous infusion of ketamine 0.5 mg x kg(-1) x h(-1) or normal saline for 72 hours postoperatively. Both groups received standardized general anaesthesia followed by patient-controlled …

Application of Patient-Specific Instrumentation in a Dog Model with Antebrachial Growth Deformity Using a 3-D Phantom Bone Model.

One of the most frequent bone deformities in dogs is antebrachial growth deformity (AGD), which results from malunion of the distal growth plates. The objective of the present study was to re-align the limbs, which can correct the length mismatch and reset the coherence of the joint with the aid of a 3-D phantom model for surgical preplanning. A 14-month-old, intact female Golden Retriever with an angular deformity of the left radius and ulna was selected for the study. The diagnosis was confirmed by orthogonal radiographs. Moreover, computed tomography (CT) scans revealed a multiplane deformi

Continuous blockade of both brachial plexus with ropivacaine in phantom pain: a case report.

A 39-year-old patient developed phantom pain after amputation of both upper arms following a burn injury. The pain did not respond to naproxen, morphine, carbamazepine, amitriptyline, calcitonin or transcutaneous electrical nerve stimulation (TENS). At the 39th post-operative day an axillary catheter was placed on the right side, as well as an interscalene catheter on the left. Ropivacaine 0.2% was infused, starting with a rate of 4 ml/h, that was increased to 6 ml/h during the subsequent 6 days. Within 20 min of catheter placement complete pain relief was achieved. The patient did not need an

Chronic neuropathic pain and its control by drugs.

The medical treatment and some currently known aspects of the aetiology of five neurogenic pain states are discussed. Neurogenic pain can be described as pain resulting from noninflammatory dysfunction of the peripheral or central nervous system without nociceptor stimulation or trauma. The enormity of the field has limited this review to post-herpetic neuralgia, complex regional pain syndromes, phantom pain, trigeminal neuralgia and diabetic neuralgia. Evidence suggests that many neurogenic pain states are not effectively controlled. This may be due in part to a lack of understanding of the a

[Comparative analysis of the results of the treatment of pain after amputation of the arm].

Postamputation pain syndrome significantly embarrasses prosthetics. In the article is described the transposition of the nerve trunks of the brachial plexus with terminal neuromas into the formed tunnel under the musculus pectoralis major, applied by the authors at 47 invalids (52 operations) with pain in the arm stump. The results of an operation have been studied in comparison with the results obtained after resection of the terminal neuromas. The data allow to recommend the translocations of the nerve trunks with the terminal neuromas in case of the pain syndrome after an arm amputation as
