Using Human 'Experiments of Nature' to Predict Drug Safety Issues: An Example with PCSK9 Inhibitors.

INTRODUCTION: When a new drug enters the market, its full array of side effects remains to be defined. Current surveillance approaches targeting these effects remain largely reactive. There is a need for development of methods to predict specific safety events that should be sought for a given new drug during development and postmarketing activities. OBJECTIVE: We present here a safety signal identification approach applied to a new set of drug entities, inhibitors of the serine protease proprotein convertase subtilisin/kexin type 9 (PCSK9). METHODS: Using phenome-wide association study (Phe

Phenotype Analysis of Early Risk Factors from Electronic Medical Records Improves Image-Derived Diagnostic Classifiers for Optic Nerve Pathology.

We examine imaging and electronic medical records (EMR) of 588 subjects over five major disease groups that affect optic nerve function. An objective evaluation of the role of imaging and EMR data in diagnosis of these conditions would improve understanding of these diseases and help in early intervention. We developed an automated image-processing pipeline that identifies the orbital structures within the human eyes from computed tomography (CT) scans, calculates structural size, and performs volume measurements. We customized the EMR-based phenome-wide association study (PheWAS) to derive di

Quantitative and qualitative differences in subcutaneous adipose tissue stores across lipodystrophy types shown by magnetic resonance imaging.

BACKGROUND: Lipodystrophies are characterized by redistributed subcutaneous fat stores. We previously quantified subcutaneous fat by magnetic resonance imaging (MRI) in the legs of two patients with familial partial lipodystrophy subtypes 2 and 3 (FPLD2 and FPLD3, respectively). We now extend the MRI analysis across the whole body of patients with different forms of lipodystrophy. METHODS: We studied five subcutaneous fat stores (supraclavicular, abdominal, gluteal, thigh and calf) and the abdominal visceral fat stores in 10, 2, 1, 1 and 2 female subjects with, respectively, FPLD2, FPLD3, HIV

A wearable accelerometry landscape of human health and disease.

Digital wearable devices provide objective characterization of human behaviors in real-world settings, yet their implications for health and diseases remain incompletely understood. Here, we analyzed a large-scale accelerometry-based cohort, including 70,473 participants with up to 10 years of follow-up, yielding a total of 11.8 million person-hours of data. From this cohort, we constructed a comprehensive atlas of 210 accelerometer-derived phenotypes (ADPs), encompassing physical activity, sedentary behavior, sleep, circadian rhythm, step counts, and step intensity. The study identified 8149

Mendelian randomization study of GLP-1R effects on ovarian cancer subtypes mediated by metabolic factors.

BACKGROUND: Ovarian cancer is a major female reproductive health issue with heterogeneous biological features on its subtypes, which may require different therapeutic strategies. Glucagon-like peptide-1 receptor (GLP-1R) agonists were reported to be beneficial for ovarian cancer, but the causal effects and mechanisms on its heterogeneous subtypes remain unclear. METHODS: We used genetic variants robustly associated with gene expression, protein level, splicing event, and DNA methylation of GLP-1R in six endocrine-related tissues (N ≤ 35,431) as genetic instruments to proxy the effect of GLP-1

Treprostinil reduces blood pressure and aortic inflammation in hypertension.

Prostaglandin I2 (PGI2) signaling is vasoprotective. Further, PGI2 signaling exhibits immunomodulatory properties that largely promote an anti-inflammatory state. Inflammation and immune activity are associated with the development of hypertension. However, it remains unknown whether exogeneous PGI2 can restrain the immune and inflammatory responses in the context of hypertension. A phenome-wide association study evaluated single-nucleotide polymorphisms (SNPs) in the receptor for PGI2, IP (gene name PTGIR), and the associated odds of developing cardiovascular pathologies. Unveiling the Immune Mechanisms of Hypertension With Single-Cell Transcriptome-Wide Mendelian Randomization.

Immune dysregulation is closely implicated in hypertension pathogenesis, yet specific therapeutic targets are lacking. We integrated expression quantitative trait loci (eQTLs) for genes expressed in 14 immune cell subsets (OneK1K) with two-sample Mendelian randomization (MR) and colocalization to provide evidence for cell type-specific causal effects of eGenes (genes with eQTLs) on hypertension and related complications (FinnGen and UK Biobank). Single cell-level expression of causal eGenes was compared between individuals with and without hypertension. A tiered framework was built to prioriti

Functional analysis of NPR2 variants supports the therapeutic rationale for CNP in short stature.

Human height is a complex trait regulated by genetic and environmental factors. Several components of chondrocyte proliferation and differentiation regulatory pathways are implicated in short-stature disorders; one such component is natriuretic peptide receptor 2 (NPR2), the receptor for C-type natriuretic peptide. We developed a high-throughput GFP reporter assay to evaluate the functional impact of NPR2 missense variants on receptor activity, which was utilized alongside a commercially available CatchPoint assay, and previously published data. Measurements from these sources were integrated

Integrative Mendelian randomization and experimental validation unveil novel druggable targets in primary biliary cholangitis.

BACKGROUND: Primary Biliary Cholangitis (PBC) is a progressive autoimmune liver disorder characterized by immune-mediated bile duct destruction, leading to cirrhosis and liver failure. Current first-line therapy with ursodeoxycholic acid (UDCA) exhibits suboptimal efficacy in 30-40% of patients, underscoring the urgent need for novel therapeutic targets. METHODS: We employed a druggable genome-wide Mendelian randomization (MR) framework integrating blood/liver cis-eQTLs, pQTLs, and PBC GWAS meta-analysis. Bayesian colocalization and phenome-wide MR (Phe-MR) validated causal relationships and

A Genome-Wide Association Study of Hidradenitis Suppurativa from the VA's Million Veteran Program.

UNLABELLED: <p>Introduction: Data from family and twin studies as well as prior genome-wide association meta-analyses suggest that hidradenitis suppurativa (HS) has a hereditary component. This GWAS identifies genetic variants associated with HS. METHODS: Individuals with a diagnosis of HS (defined as at least one instance of ICD9 705.83 or ICD10 L73.2) were identified within the Million Veteran Program. Multi-population and population-specific (African, European, and Hispanic ancestries) case-control genome-wide association studies (GWAS) were performed. Lead single-nucleotide polymorphisms

Characterisation of the SMN1/2 locus using a highly specific variant caller on whole-genome sequence data from 500,000 individuals.

Spinal muscular atrophy (SMA) is a rare autosomal recessive condition caused by biallelic loss of the SMN1 gene that results in severe lower motor neuron degeneration in childhood. Targeted pre-symptomatic treatments have been shown to dramatically improve outcomes, supporting the inclusion of SMA in newborn screening programmes. However, ~95% of infantile-onset cases are caused by homozygous deletions of exon 7-8 of the SMN1 gene, which is challenging to detect using short-read next-generation DNA sequencing technology due to the high sequence similarity between SMN1 and its nearby paralog SM

Identification of Treatment Targets in Allergic Conjunctivitis Through Proteome-Scale Mendelian Randomization Analysis.

PURPOSE: Allergic conjunctivitis (AC) is a condition with a rising prevalence that occasionally leads to irreversible visual impairment. However, few novel therapeutic targets have been identified for AC. The investigation aims to utilize the Mendelian randomization (MR) technique to explore the causal impacts exerted by the plasma proteome on AC. METHODS: An evaluation was conducted on a two-sample MR study utilizing 2,940 plasma proteins from the UK Biobank Pharma Proteomics Project (UKB-PPP) to investigate their causal links with AC. Confirmation of these MR results was achieved using Summ

Exploring the association between DNA methylation and pancreatic cancer susceptibility through epigenome-wide Mendelian randomization and multi-omics data integration.

Investigating the role of DNA methylation in the development of pancreatic cancer (PC) may facilitate identification of potential targets for both diagnosis and treatment. We carried out a comprehensive epigenome-wide Mendelian randomization (EWMR) analysis to investigate the correlation of genetically predicted blood CpG sites with PC. Following this, we conducted various sensitivity analyses and repeated analyses using different selection criteria for instrumental variables and conditional Bayesian colocalization to guarantee the reliability of the results.
