Vibratory analysis of the neoglottis after surgical intervention of cricopharyngeal myotomy and implantation of tracheal cartilage.

OBJECTIVE: To examine the effect of a new surgical intervention, consisting of cricopharyngeal myotomy and tracheal cartilaginous implantation on the anterior wall of the esophagus, for tracheoesophageal shunt and esophageal phonation. DESIGN: We examined the vibration of the neoglottis of tracheoesophageal shunt and esophageal speakers after total laryngectomy using a high-speed video camera (frame rate, 1000 per second). PATIENTS: Twenty-one alaryngeal patients were involved: 13 who had undergone the present procedure and 8 who had not. RESULTS: The regularity of neoglottal vibration and …

[S/z ratio in glottic closure defects].

The quotient between the maximum phonation time (MPT) of the sound /s/ and a vowel sound, measures the relation of resistances to the expiratory air flow. When the sound /s/ is emitted, resistance is located at the teeth, and when a vowel is phonated, the site of resistance is the glottis. Estimating the quotient instead of measuring only the MPT, we avoid the influence of respiratory alterations in the results. We evaluated the utility of the s/z ratio as a clinical indicator of laryngeal pathology in 72 patients with organic vocal cord pathology. We compared the results of the s/z ratio to t…

Evidence of the voice-related cortical potential: an electroencephalographic study.

The Bereitschaftspotential (BP) is a slow negative-going cortical potential associated with preparation for volitional movement. Studies since the 1960s have provided evidence for a BP preceding speech-related volitional motor acts. However, the BP associated specifically with phonation [corrected] has not to date been systematically investigated. The current investigation utilizes a novel experimental design to address methodological confounds typically found in studies of movement-related cortical potentials, to demonstrate the existence and localization of generators for the voice-related c…

[Glottal and supraglottal configuration during whispering].

BACKGROUND: The purpose of the study was to assess glottal and supraglottal configurations during whispering in comparison to habitual phonation and to visually estimate to what extent force is exerted onto the vocal fold epithelium during whispering. Is whispering a good compromise between preservation of epithelium and permission to communicate orally, e. g. postoperatively? PATIENTS AND METHODS: We investigated on 100 patients with organic or non-organic voice disorders. Patients with vocal fold immobility and patients with organic lesions large enough to impede glottal closure were exclud…

Effects of implant and vocal fold stiffness on voice production after medialization laryngoplasty in an MRI-based vocal fold model.

Medialization laryngoplasty is one of the primary surgical interventions in the treatment of glottal insufficiency due to vocal fold paralysis, paresis, or atrophy. During the surgery, an implant is laterally inserted into the larynx to medialize the affected vocal fold toward glottal midline, with the goal of improving glottal closure during phonation and voice production efficiency. While implants of different materials and geometry designs have been used, the effect of implant design on the voice outcome remains unclear. In this simulation study, the effect of implant stiffness was investig…

The morphometry of the laryngeal phonatory system - base of the anatomical study of the voice aptitudes.

Speaking is one of the characteristics of the human race and the main factor that has marked our progress over time. The singing voice is the crowning of the speech act and the main component of the lyrical manifestation of personality. Doctors in various fields, but especially anatomists have been concerned about discovering how the voice and the substrate of its variability are formed, but these aspects have not yet been fully deciphered. This study is the starting point in our research on the phonation system, organized on three levels: laryngeal, oral, palatinal, pharyngeal, epiglottal and…

Evaluating the effect of risperidone on speech: A cross-sectional study.

Speech subsystems are susceptible to the effects of several factors including medications. The atypical antipsychotics can also adversely affect the speech because of its action on serotonin and dopamine neurotransmitters. The present study aims to analyze the speech characteristics associated with atypical antipsychotic risperidone. Speech of 92 patients on risperidone with or without trihexyphenidyl and/or clonazepam were compared with that of 31 persons who were not on any psychotropic medicines. Compared to control group, maximum phonation duration, sequential motion rate of diadochokinesi…

Modeling coupled aerodynamics and vocal fold dynamics using immersed boundary methods.

The penalty immersed boundary (PIB) method, originally introduced by Peskin (1972) to model the function of the mammalian heart, is tested as a fluid-structure interaction model of the closely coupled dynamics of the vocal folds and aerodynamics in phonation. Two-dimensional vocal folds are simulated with material properties chosen to result in self-oscillation and volume flows in physiological frequency ranges. Properties of the glottal flow field, including vorticity, are studied in conjunction with the dynamic vocal fold motion. The results of using the PIB method to model self-oscillating …

Anatomical and functional correlates of voice quality in tracheoesophageal speech.

The purpose of the current study was to assess the anatomic and functional correlates of voice quality in tracheoesophageal speech, with dynamic imaging studies of the neoglottis. Videofluoroscopy (providing a lateral view), digital high-speed endoscopy (providing a "birds-eye" view), and their relationships with perceptual evaluations of voice quality were investigated. Several significant relationships were found. Imaging with videofluoroscopy revealed that the following anatomic and functional parameters (established during phonation) are related to voice quality: presence of a neoglottic b…

Chaos in voice, from modeling to measurement.

Chaos has been observed in turbulence, chemical reactions, nonlinear circuits, the solar system, biological populations, and seems to be an … ce signals. Nonlinear dynamic methods, however, have differentiated between normal and pathological phonations and can describe the aperiodic or chaotic voice. Chaos theory and nonlinear dynamics can enchance our understanding and therefore our assessment of pathological phonation.

Muscle misuse voice disorders: description and classification.

It is apparent that voice disorders frequently labelled "functional" are associated with laryngeal muscle misuse. This use of the word "functional" is, however, intrinsically ambiguous, and so we propose an alternative term based on descriptive features of dysfunction: "muscle misuse voice disorders". Persistent phonation with an abnormal laryngeal posture can lead to organic changes such as nodules or polyps, particularly in females with posterior glottic chink. We hypothesized that the chink was related to an overall increase in laryngeal muscle tension, and more directly due to inadequate r…

Reflex activation of laryngeal muscles by sudden induced subglottal pressure changes.

In measuring the effect of subglottal pressure changes on fundamental frequency (Fo) of phonation, the effects of changing laryngeal muscle activity must be eliminated. Several investigators have used a strategy in which pulsatile increases of subglottal pressure are induced by pushing on the chest or abdomen of a phonating subject. Fundamental frequency is then correlated with subglottal pressure changes during an interval before laryngeal response is assumed to occur. The present study was undertaken to repeat such an experiment while monitoring electromyographic (EMG) activity of some laryn…

Temporal specificity of abnormal neural oscillations during phonatory events in laryngeal dystonia.

Laryngeal dystonia is a debilitating disorder of voicing in which the laryngeal muscles are intermittently in spasm resulting in involuntary interruptions during speech. The central pathophysiology of laryngeal dystonia, underlying computational impairments in vocal motor control, remains poorly understood. Although prior imaging studies have found aberrant activity in the CNS during phonation in patients with laryngeal dystonia, it is not known at what timepoints during phonation these abnormalities emerge and what function may be impaired. To investigate this question, we recruited 22 adduct…

Quantification of False Vocal Fold Hyperfunction During Quiet Breathing in Muscle Tension Dysphonia.

BACKGROUND/OBJECTIVES: False vocal fold (FVF) hyperfunction during phonation is thought to be a diagnostic sign of primary muscle tension dysphonia (pMTD). However, hyperfunctional patterns with phonation are also observed in typical speakers. This study tested the hypothesis that FVF posturing during quiet breathing, as measured by the curvature of FVF, could differentiate patients with pMTD from typical speakers. METHODS: Laryngoscopic images were collected prospectively in 30 subjects with pMTD and 33 typical speakers. Images were acquired at the end of expiration and maximal inspiration d…
