Propeller and Perforator Flaps.

Lower extremity reconstruction remains a challenge to the plastic surgeon. With the advent of the perforasome theory and refinement of operative techniques, perforator and perforator propeller flaps serve as an attractive option that expands the armamentarium of reconstruction options available for lower extremity defects.

Open surgery in lambdoid-related plagiocephaly: detailed techniques, risks, and long-term results.

Lambdoid craniosynostosis is the rarest form among monosutural ones, with a prevalence ranging between 1 and 4% of all craniosynostosis. Open-strip craniectomy used to be the common treatment method; however, a better understanding of the pathology as well as a more careful analysis of outcomes and complications led to a shift towards other techniques such as open cranial vault reconstruction, device-mediated distraction, and endoscopic procedures. The present paper will deal with open surgical options, namely open cranial vault reconstruction (OCVR) and device-assisted procedures (DAP). In pa

Vascularized Flaps for Anterior Skull Base Defects.

A variety of techniques exist for the reconstruction of skull base defects. However, the gold standard for skull base reconstruction is vascularized flaps, especially when there is a cerebrospinal fluid leak. The most frequently used flap is the nasoseptal flap, which has numerous applications, has relatively low morbidity, and is straightforward to harvest. When this flap is unavailable, other options include the inferior turbinate flap, middle turbinate flap, lateral wall flap, pericranial flap, and temporoparietal fascia flap.

Facial loss injuries.

Facial loss injuries present a significant surgical challenge, often requiring not only the expertise of a plastic surgery specialist but also interdisciplinary collaboration. Special attention is given to acral structures, such as the nose, lips, and auricles. This article details each step in the reconstruction of these areas, including the use of flap plastics and microsurgical techniques. Emphasis is placed on the specifics of treating animal-induced injuries, with an overview of infection prevention and treatment strategies. The article also includes a list of replantation centers in the

Omentum transplantation for malignant tumors: a narrative review of emerging techniques and clinical applications.

Omentum transplantation has emerged as a versatile and effective technique across various surgical disciplines due to its unique properties of immunological surveillance, anti-inflammatory effects, and wound healing promotion. In breast cancer surgeries, it has been utilized to manage locoregional issues and immediate reconstruction, providing satisfactory cosmetic outcomes and minimal complications, particularly in patients who had previously undergone irradiation. For esophageal cancer, omental reinforcement has significantly reduced anastomotic leak rates and postoperative complications, su

An Overview of the Currently Used Congenital Auricular Anomalies (CAA) Classifications for Surgical Reconstruction: A Scoping Review.

BackgroundCAAs are congenital malformations of the auricle ranging from ear underdevelopment to anotia, lacks standardized classification, impacting our outcome of different reconstruction approaches. This scoping review aimed to explore which CAA classifications are most used in current ear reconstruction practices.MethodsWe conducted a scoping review following the PRISMA guidelines, searching MEDLINE and Embase databases on November 1st, 2023. Studies on CAA reconstruction that included clear descriptions of the used classification published in the past 5 years were included. Studies were ap

Microtia: A Review.

Microtia is a congenital condition characterized by a spectrum of ear malformations, ranging from minor structural anomalies to anotia. The etiology is multifactorial, with interplay of genetic and environmental factors being implicated. Early multidisciplinary involvement should include pediatrics, plastic surgery, otolaryngology, audiology, and speech pathology, to ensure that the child is appropriately supported as they grow. This article provides an overview of the condition, a brief historic review, and illustrated descriptions of 2 common surgical approaches: porous polyethylene reconstr

Reconstruction of the Charcot Spine with a Vascularized Pedicle: A Report of 3 Cases.

CASE: We present 3 patients with Charcot spinal arthropathy (CSA) as a result of previous spine trauma treated initially with spinal instrumentation. All developed CSA in a delayed presentation years after the original injury and surgery. All were reconstructed with anterior and posterior surgery. In each case, the anterior reconstruction was supplemented with a vascularized pedicle graft. CONCLUSION: All 3 patients went on to fusion after previously demonstrated bone loss after a vascularized pedicle was used.

[Secondary reconstruction of the forehead defects].

The forehead is the protective helmet of the brain, if we refer to the etymology of the word calvaria. Destroyed, it must be rebuilt; priority is given to its bone infrastructure, even though we can live without it, as long as the other tissues (skin, muscle, aponeurosis, meninge) are intact. Complex surgical project which will be mentioned when the multiplicity of techniques described demonstrates their imperfection and until now the best restoration of a missing tissue requires its replacement by the same tissue. Invitation to invent a true regenerative surgery.

[Laryngotracheal Resection and Reconstruction].

Cervical tracheoplasty requires knowledge and skills not only of the trachea but also of the larynx. Even if the lesion is thought preoperatively to be confined to the cervical trachea, intraoperative manipulation of the larynx is often necessary. This area that involves the larynx and trachea is a borderline between otorhinolaryngology and thoracic surgery, and there are very few surgeons in Japan who are well trained in both areas. The author studied at the Department of Thoracic Surgery at the Medical University of Vienna for clinical lung transplantation and was fortunate to have the oppor

Orbital Hypertelorism.

Orbital hypertelorism may be seen in isolation or more commonly within the context of other congenital craniofacial dyscrasias such as craniosynostosis, midface hypoplasia, exorbitism, atypical orofacial clefting, and malocclusion. Careful consideration of the preoperative cranial, orbital, and midface phenotypes, as well as the time of intervention, is imperative to choosing the optimal surgical approach to orbital repositioning. Virtual surgical planning serves as a powerful tool to increase surgical accuracy, decrease skeletal relapse, and mitigate historical complication profiles.

The Application of a Cross-Shaped Advancement Flap in Polysyndactyly of the Fifth Toe.

Polysyndactyly of the foot is a common congenital malformation of the lower extremity. We introduce our surgical technique for reconstruction of polysyndactyly of the fifth toe fused with the fourth toe. The technique includes the removal of the medial hypoplastic ray, web-space reconstruction using a dorsal cross-shaped advancement flap and closure of the lateral incisions of the separated toes with a tongue-shaped flap and zigzag triangular flaps. This technique is advantageous to prevent postoperative web creep, reduce the tension of the dorsal incision closure and it is relatively easy to

Extended autologous tracheal replacement by a novel pedicled thoraco-chondro-costal flap: a cadaveric proof of concept.

OBJECTIVES: Our aim was to report an anatomic model of an autologous flap based on the internal thoracic blood supply: the pedicled thoraco-chondro-costal flap; and establish the feasibility of various types of extended tracheal replacement with this novel flap, according to a newly proposed topographic classification. METHODS: In a cadaveric model, a cervicotomy combined with median sternotomy was performed. The incision was extended laterally to expose the chest wall. The internal thoracic pedicle was freed from its origin down to the upper limit of the delineated flap to be elevated. The p
