[Reoperation for recurrent or second primary lung cancer].

From 1975 to July 1994, twenty patients underwent second or third pulmonary resections for 7 recurrent lung cancers and 14 second primary lung cancers. The initial surgical procedures were lobectomy in 18, pneumonectomy in 1 and bilateral segmentectomy in 1. The procedures at the second operation were completion pneumonectomy in 4, ipsilateral wedge resection in 3, contralateral lobectomy in 1, contralateral segmentectomy in 4, contralateral wedge resection in 7 and resection of left main bronchus in 1. At the third operation, wedge resection was done in one 28 months after completion pneumone…

Extent of lung resection in non-small lung cancer with interlobar lymph node involvement.

BACKGROUND: Optimal resection type for non-small cell lung cancer (NSCLC) with interlobar lymph node involvement (ILNI) has seldom been reported. To completely resect a NSCLC with ILNI, some surgeons believe that a pneumonectomy is needed. METHODS: We retrospectively studied 151 patients (147 men, 4 women; mean age 58 ± 8 years, range 34-79) with non-small lung cancer without mediastinal or hilar lymph node metastasis who underwent an anatomic lung resection with systematic lymph node dissection between January 1995 and November 2006. All patients had involvement of the surgical-pathologic in…

The effect of extent of local resection on patterns of disease progression in malignant pleural mesothelioma.

BACKGROUND: We sought to determine whether or not there are differences in disease progression after radical or nonradical (debulking) surgical procedures for malignant pleural mesothelioma. METHODS: Over a 49-month period, 132 patients with malignant pleural mesothelioma underwent surgery. Fifty-three underwent extrapleural pneumonectomy and 79 underwent nonradical procedures. Time to evidence of clinical disease progression was recorded, as was the site(s) of that disease. RESULTS: One-hundred nineteen patients were evaluable, of which 59% (22 radical; 48 nonradical) had disease progressio…

Favorable results after sleeve lobectomy or bronchoplasty for bronchial malignancies.

BACKGROUND: Sleeve lobectomy and bronchoplasty are established alternatives to pneumonectomy for bronchial malignancies involving a main bronchus. However, potential bronchial anastomotic complications have deterred the general application of these types of resection. Some reports have contained a mixture of non-small cell lung cancer (NSCLC) and tumors of low-grade malignancy, making it difficult to assess the long-term results of these procedures as an alternative to pneumonectomy for lung cancer. METHODS: We retrospectively reviewed our experience with sleeve lobectomy and bronchoplasty fo…

Lung resection for lung cancer after pleural empyema.

BACKGROUND: The association of pleural empyema and lung cancer has traditionally been considered as a contraindication for lung resection. H … y two patients, the diagnosis of empyema preceded the diagnosis of lung cancer. Among patients with pneumonectomy, a good intrapleural cleavage plane existed in only one patient with no signs of infection. In each patient with a lobectomy, preoperative chest tube aspiration took 20 to 30 days and in none of them intraoperative signs of infection existed. In pat …

Increased expression of hypoxia-inducible factor-1α in proliferating neointimal lesions in a rat model of pulmonary arterial hypertension.

INTRODUCTION: The role of hypoxia-inducible factor-1α (HIF-1α) in pulmonary vascular remodeling is still undetermined. The objective of this study is to investigate the expression of HIF-1α and its role in proliferating neointimal lesions in a rat model of pulmonary arterial hypertension induced by monocrotaline (MCT) administration after left pneumonectomy. METHODS: The rats were subjected to MCT (60 mg/kg, subcutaneously) 7 days after left pneumonectomy or sham surgery; controls with vehicle treatment after left pneumonectomy or sham surgery were also studied. On day 35, hemodynamic paramet…

[Postoperative chest: normal imaging features].

Chest radiographs and CT are especially useful for the follow-up of patients after lung resection. The purpose of this paper is to illustrate normal postsurgical imaging findings that differ according to the type of surgery (pneumonectomy, lobectomy, segmentectomy and wedge resection). Anatomic changes induced by surgery affect the remaining lung, pleura and chest wall, mediastinum and diaphragm. After pneumonectomy, there is accumulation of fluid in the post pneumonectomy space, progressive chest retraction with mediastinal shift toward the operated side and elevation of the ipsilateral hemid…

Postpneumonectomy pulmonary oedema.

The occurrence of pulmonary oedema was studied retrospectively in 243 patients who underwent pneumonectomy in one hospital from 1975 to 1984. Pulmonary oedema developed in eight of 113 patients who had a right sided pneumonectomy and in three of 130 patients undergoing a left sided procedure. It occurred more commonly in patients requiring a second thoracotomy because of blood loss (in three out of seven patients). There were no significant differences preoperatively in pulmonary function, lung perfusion scans, or cardiovascular condition between patients who subsequently developed pulmonary o…

Resection of proximal left main bronchus carcinoma.

A carcionoma at the orifice of the left main bronchus is generally considered inoperable. Since 1963, in six patients, we have mobilised the transverse aortic arch and, working above the aortic arch, cut the left main bronchus off the trachea and closed the stump. The operation is completed as a pneumonectomy; it is referred to as a supra-aortic pneumonectomy. The indications, technique, and results are described.

Postpneumonectomy syndrome: surgical management and long-term results.

OBJECTIVE: Postpneumonectomy syndrome is a rare syndrome of dynamic airway obstruction caused by extreme rotation and shift of the mediastinum after pneumonectomy, resulting in symptomatic central airway compression. We have treated this syndrome by mediastinal repositioning and placement of saline-filled prostheses into the pneumonectomy space. There is a paucity of outcome data for patients treated surgically, with only a single series of 11 patients previously reported. We analyzed our recent experience with treatment of this syndrome and report on the short and long-term outcomes and quali…

[Thoracic surgery: the major surgical procedures].

The most frequent thoracic surgeries are performed for the treatment of primary lung cancer and pleural mesothelioma. For lung cancer, the standard procedures are pneumonectomy and lobectomy with associated mediastinal lymphadenectomy. In order to avoid pneumonectomy, extended lobectomy with sleeve bronchoplasty and/or angioplasty can be done. When adjacent organs are involved, extended resections are accepted (chest wall, vena cava...). For small lesions (<2 cm) without lymph nodes involvement and for patients with limited respiratory function, segmentectomy is an option (results under evalua…

Intracerebral abscess: a complication of severe cystic fibrosis lung disease.

Intracerebral abscess is an uncommon complication of severe cystic fibrosis lung disease. The present report describes a case of fatal multiple intracerebral abscesses in a patient with a severely bronchiectatic, nonfunctioning right lung and chronic low-grade infection. The patient was previously turned down for pneumonectomy. Intracerebral abscess in cystic fibrosis and the potential role of pneumonectomy in the present patient are discussed.

Sarcopenia as independent risk factor of postpneumonectomy respiratory failure, ARDS and mortality.

OBJECTIVES: Sarcopenia is associated with poor outcome in cancer-patients. However, the methods to define sarcopenia are not entirely standardized. We compared several morphometric measurements of sarcopenia and their prognostic value in short-term-outcome prediction after pneumonectomy. MATERIAL AND METHODS: Consecutive lung-cancer patients undergoing pneumonectomy from January 2007 to December 2015 and having a pre-operative computed tomography (CT) scan were retrospectively included. Sarcopenia was assessed by the following CT-based parameters measured at the level of the third lumbar vert…

Facial flushing due to recurrent bronchial carcinoid.

Carcinoid syndrome is quite a rare presentation in bronchial carcinoid. A review of the literature suggests a figure of 2% to 7% in various series. This is usually associated with recurrent carcinoid tumor in the presence of hepatic metastasis. We discuss a patient who presented with flushing attacks 13 years after a left pneumonectomy. Further investigation found that the patient had recurrence at the pneumonectomy stump and in subcarinal lymph nodes.

Analysis of risk factors in the development of bronchopleural fistula after major anatomic lung resection: experience of a single centre.

BACKGROUND: The bronchopleural fistula (BPF) is a rare but potentially fatal complication of major thoracic surgery. The purpose of this work is to investigate the risk factors associated with the development of fistulas after lobectomy and pneumonectomy. METHODS: We retrospectively reviewed the records of 835 patients who underwent major anatomic lung resection at our centre from January 2003 to December 2013. Of these, 49 underwent pneumonectomy (P group) and 786 lobectomy (L group). RESULTS: A total of 18 patients (2.6%) developed a BPF in the postoperative period, of which there were 11 …

Postpneumonectomy syndrome related to the thickness of the fat tissue in the anterior mediastinum: a retrospective observational study.

BACKGROUND: Postpneumonectomy syndrome (PPS) is a life-threatening condition characterized by the extrinsic compression of the mainstem bronchus after pneumonectomy, causing rapidly progressive dyspnea. Information regarding the predictive factors of this condition is limited. Thus, the current study aimed to identify the predictive findings that can help prepare the treatment of PPS in advance. METHODS: The present study is a retrospective observational study. We reviewed the medical records of 12 consecutive patients who underwent pneumonectomy for lung cancer or tuberculosis between 2009 a…

[Pleuro-pneumo-phreno-pericardiectomy with pericardial and diaphragmatic plasty].

Aim of the paper is to evaluate 43 extrapleural pneumonectomy performed from 1988 to May 2000. Criteria for extrapleural pneumonectomy were pleural biopsy by thoracoscopy, potentially completely resectable unilateral disease by computed tomography and predicted postresection forced expiratory volume >1,3 L/sec. The resections regarded 33 pleural mesothelioma, 9 pleural lung-carcinosis and 1 pleural melanoma effusion. The perioperative mortality rate was 2,2% (1 death) and morbidity 21,4%.

Management of a large bronchopleural fistula using a tracheobronchial stent.

Bronchopleural fistula, one of the most serious complications following pneumonectomy, has a complicated treatment protocol and carries a high mortality rate. We present a case report of a 75-year-old female with squamous cell carcinoma of the lower lobe with positive peribronchial hilar nodes who underwent a right pneumonectomy. She represented with a large bronchopleural fistula one month postoperatively, eventually treated by a novel tracheobronchial stenting procedure.

Early complications. Recurrent nerve palsy.

Unilateral vocal cord paralysis following pneumonectomy has profound effects on deglutition, phonation, protection of the airway, and generation of an adequate cough. If untreated in patients with limited pulmonary reserve, these detrimental effects can have devastating consequences. Several techniques are currently available which allow adequate medialization and rehabilitation of the paralyzed vocal cord. Adequate diagnosis and treatment can minimize the negative consequences associated with vocal cord paralysis following pneumonectomy.
