A turn for the worse: The impact of deteriorating healthcare spending in South Africa.

The November 2023/2024 national health budget cuts represent the largest reduction in public health expenditure in South Africa's post-apartheid history. This article critically examines the implications of these budget cuts within the context of the country's post-apartheid health system reforms and macroeconomic strategies. Specifically, it documents the historical evolution of fiscal policies and health expenditure across three distinct periods: the 2008 - 2013 period, which witnessed a counter-cyclical fiscal strategy that improved healthcare access and began addressing apartheid-era dispa

Influences on decision-making in road safety.

This paper extends work previously undertaken to map the actors involved in the road transport industry at a municipal level in the UK and explores the decision-making of, and relationships between, key actors with regard to road safety intervention. Interviews were undertaken with 21 key actors which found that the predominant approach to road safety problems remains person-based as opposed to system-based and that the devolution of responsibility to a municipal level has the potential to encourage this further. Interrelationships and formal partnerships between actors are key in influencing

Ensuring progress on sustainable access to effective antibiotics at the 2024 UN General Assembly: a target-based approach.

Rising antimicrobial resistance (AMR) is a global health crisis for countries of all economic levels, alongside the broader challenge of access to antibiotics. As a result, development goals for child survival, healthy ageing, poverty reduction, and food security are at risk. Preserving antimicrobial effectiveness, a global public good, requires political will, targets, accountability frameworks, and funding. The upcoming second high-level meeting on AMR at the UN General Assembly (UNGA) in September, 2024, is evidence of political interest in addressing the problem of AMR, but action on targe

Subversive mythical figures and feminist resistance: On the rise of posthuman 'professionals'.

Within the context of neoliberal healthcare, nurses and other health professionals face working conditions that leave them perpetually feeling inadequate, as though they are not enough. They are consistently expected to achieve more with less resources. In such an environment, mere professionalism proves wholly insufficient, enforcing norms of altruism and kindness. Professionals must transcend this disciplinary tool and embody a 'more-than-professional' approach. This study, informed by critical posthumanism, employs three mythical archetypes-the Medusa, the Witch and the Siren-to illuminate

The cultural turn in understanding 'addiction'.

In this essay we want to foreground a question: what happens to 'addiction' when we take seriously cultural scripts informing its trajectories? Can this bring us to unthink addiction as problematic notion and move it onto new paradigms that fit better the now acknowledged fluidity and pluralistic episteme of 'addiction' and more broadly of chronic life conditions? Indeed, 'addiction' has become a pivotal concept in the contemporary world. A powerful diagnostic framework in interpreting human behaviour, for some 'addiction' has become the 'new normal' with chronic relations with different thing

Between the witness and the observer: what ethnography can learn from James Baldwin.

What is the role of the ethnographer during a time of increased racial hostility, political mobilization to keep racial minorities "in their place," and commitments to revisionist interpretations of the country's past and projected future? While the traditional, classic ethnographic approach would recommend that the researcher should avoid taking a stance on so-called political matters and merely observe them, I argue that that position is insufficient to address the issues that people are currently facing. Ethnography can, and should, do more. Therefore, this essay argues that the role of the

Setting targets for universal access to HIV prevention, treatment and care for injecting drug users (IDUs): towards consensus and improved guidance.

Scaling-up access to HIV/AIDS prevention, treatment and care for injecting drug users (IDUs) has been frustrated by the lack of a framework, indicators and agreed targets for interventions specifically targeting IDUs. Major progress in this regard has been achieved with the recent development of a joint Technical Guide for Countries to Set Targets for Universal Access to HIV Prevention, Treatment and Care for Injecting Drug Users and related technical consultations. This guide provides technical guidance to countries on setting ambitious, but achievable national targets for scaling-up towards

The complexities of integrating evidence-based preventative health into England's NHS: lessons learnt from the case of PrEP.

BACKGROUND: The integration of preventative health services into England's National Health Service is one of the cornerstones of current health policy. This integration is primarily envisaged through the removal of legislation that blocks collaborations between NHS organisations, local government, and community groups. AIMS AND OBJECTIVES: This paper aims to illustrate why these actions are insufficient through the case study of the PrEP judicial review. METHODS: Through an interview study with 15 HIV experts (commissioners, activists, clinicians, and national health body representatives), w

Outbreak dynamics of COVID-19 in China and the United States.

On March 11, 2020, the World Health Organization declared the coronavirus disease 2019, COVID-19, a global pandemic. In an unprecedented collective effort, massive amounts of data are now being collected worldwide to estimate the immediate and long-term impact of this pandemic on the health system and the global economy. However, the precise timeline of the disease, its transmissibility, and the effect of mitigation strategies remain incompletely understood. Here we integrate a global network model with a local epidemic SEIR model to quantify the outbreak dynamics of COVID-19 in China and the

Factors associated with fears due to COVID-19: A Scleroderma Patient-centered Intervention Network (SPIN) COVID-19 cohort study.

INTRODUCTION: No studies have examined factors associated with fear in any group of people vulnerable during COVID-19 due to pre-existing medical conditions. OBJECTIVE: To investigate factors associated with fear of consequences of COVID-19 among people living with a pre-existing medical condition, the autoimmune disease systemic sclerosis (SSc; scleroderma), including country. METHODS: Pre-COVID-19 data from the Scleroderma Patient-centered Intervention Network (SPIN) Cohort were linked to COVID-19 data collected in April 2020. Multivariable linear regression was used to assess factors asso

Assessing healthcare status and challenges in border regions: insights from Tak and Mae Hong Son provinces, Thailand - a mixed-method approach.

BACKGROUND: Thailand's Tak and Mae Hong Son (MHS) provinces, bordering Myanmar, are critical entry points for individuals seeking employment or healthcare. The COVID-19 pandemic and Myanmar's political instability have significantly impacted the health and performance of border-area health systems. This analysis examines the current state of the health sector, border health challenges resulting from cross-border movement, and identifies key gaps through data from health organizations and interviews in Tak and MHS provinces. METHODS: A mixed-method situational analysis was conducted, incorpora

Conceptualising hepatitis C stigma: A thematic synthesis of qualitative research.

BACKGROUND: Stigma is an important element in the experience of living with chronic viral hepatitis B (HBV) and C (HCV), impacting healthcare access and uptake as well as health outcomes. Conceptualisations of stigma in research are, however, often assumed and implicit. This study aimed to synthesise and critically engage with the qualitative literature to provide an overarching conceptualisation of stigma as it pertains to viral hepatitis. METHODS: We critically reviewed qualitative literature that mobilised concepts or theories of stigma in relation to viral hepatitis. We searched seven ele

[E. M. Jellinek's silenced and silencing transgenerational story].

Jellinek is a kind of archetypal character for future generations in the field of addiction studies. His implosion in the arena of alcoholism around the age of 50 was an unexpected challenge to medical science. We know very little about his own role models giving an intellectual and moral compass to his pragmatic creativity. More than 30 years has passed since Jellinek's death when an American sociologist Ron Roizen started unearthing his silent story. Roizen discerned that there are a lot of unsaid and muted issues in his personal Hungarian past. Our paper, based on the authors' research in H

The Efficiency of the German Healthcare System: Some Puzzling Findings.

This article examines the cost-effectiveness of the German healthcare system from the perspective of statutory health insurance (SHI). Using data for 2011-2014-the most recent period for which consistent national inputs are available-we compare a marginal, system-wide cost-effectiveness ratio with the cost-effectiveness of newly approved cancer drugs. The analysis indicates that the German healthcare system incurred costs per life-year gained that were broadly comparable to the incremental cost-effectiveness ratios reported for many new cancer drugs. Sensitivity analyses varying the attributio

Beyond cost-effectiveness: Reframing the value of essential nursing care in healthcare priority-setting.

Cost-effectiveness analysis (CEA) has become a dominant framework for healthcare priority-setting, offering policymakers a standardized way to compare costs with expected health gains under conditions of scarcity. Yet its dominance raises ethical and conceptual problems when applied to forms of care that are relational, continuous, preventive, and foundational to human wellbeing. Essential nursing care is often difficult to represent through the outcome metrics that CEA privileges, particularly in critical care, chronic illness management, palliative care, gerontological care, and end-of-life

Effects, Challenges, and Preconditions for Implementing Direct Health Facility Financing and the Prime Vendor Model in Low-Resource Settings: The Experience from Tanzania.

Primary health care (PHC), while critical to achieving universal health coverage, remains underfunded and often delivers poor-quality care. In response, Tanzania introduced two innovative and complementary reforms-Direct Health Facility Financing (DHFF) and the Prime Vendor Model-to improve PHC financing and commodity availability by decentralizing financial management and granting greater autonomy to align financing with local needs. DHFF involves direct transfer of funds to PHC facility bank accounts, enabling timely access to resources and strengthening local decision-making. The Prime Vend

Ethical Governance of Open Data Across Biomedical Research, Healthcare, and Public Health: Privacy, Equity, Trust, and Controlled Access.

Open data has become central to biomedical research and public health, but health information is uniquely sensitive and difficult to share responsibly. In this narrative review, open data is considered as a spectrum of health-data sharing arrangements, ranging from public aggregate datasets to controlled-access repositories, federated analysis, and synthetic data. This narrative review synthesizes the scientific and societal rationale for greater openness with the ethical, legal, and governance constraints that shape what "open" can realistically mean in healthcare. We examine how data sharing

Medicaid Waivers to Address Homelessness: Political Development and Policy Trajectories.

This article reviews the role of Medicaid waivers in homeless policy and their emerging role as a mechanism to address homelessness. The authors evaluate the political development of waivers in housing and homeless policy over the past thirty years, and they investigate the status of current and approved waivers targeting homelessness. They then consider how waivers may shape homeless policy governance going forward (including the success of existing systems), and they address implementation and efficacy questions related to the role of health care payers in solutions to homelessness. The auth

Sociodemographic, political, and policy contexts of cancer care: A comparative analysis of countries with the highest survival rates.

INTRODUCTION: Cancer remains a leading cause of mortality, with 20 million new cases and 10 million deaths in 2022 (WHO). Despite advances in detection and treatment, structural inequalities affect exposure to risk factors and healthcare access. This study compares the cancer care policy contexts of five countries with the highest five-year survival rates. METHODS: This qualitative review examines cancer care policies in Australia, Canada, Costa Rica, Belgium, and Japan countries through a critical comparative approach.
