OBJECTIVE: To study ICAM-1, IL-1alpha expression in capillaries and to explore whether capillaries play a role in the pathogenesis of polymyositis.
METHODS: Ten cases of patients with polymyositis and 6 patients with other myopathies were collected. CD34, ICAM-1 and IL-1alpha were selected for a quantitative measurement using an image analysis system, and results were presented as the percent of positive staining in the tissue area.

OBJECTIVE: To present a rare case of breast cancer associated with both inflammatory muscle disease and liver disease as a paraneoplastic syndrome.
CLINICAL PRESENTATION AND INTERVENTION: A woman with breast cancer presented with elevated liver enzymes and progressive proximal muscle weakness. Liver biopsy was consistent with hepatitis and muscle biopsy revealed myositis. The start of corticosteroid therapy was followed by relief of the myopathic symptoms and regression of hepatitis histopathologically.

Abstract We present a rare case of simultaneous muscle weakness and cutaneous depigmentation. Muscle and skin biopsies confirmed the diagnosis of polymyositis and generalized vitiligo vulgaris. All symptoms improved after steroid therapy. Immunohistochemical analyses revealed predominant CD8-positive T cell infiltration in both muscular and cutaneous lesions. This case suggests that a common autoimmune mechanism mediated by cytotoxic T lymphocytes underlies the pathogeneses of these two diseases.

It is well known that a certain percentage of patients with polymyositis and dermatomyositis (PM/DM) is corticosteroid resistant. Established and novel approaches to steroid-resistant PM/DM are discussed in this review. Methotrexate (MTX) is a first-line treatment in the case that steroid therapy fails. Azathioprine and cyclophosphamide also fall into this category. Cyclosporine, a specific inhibitor of calcineurin, has been reported almost as effective as MTX. We report anesthetic experience of two patients suffering from polymyositis. The first case is a 56 year old woman who underwent tympanoplasty for cholesteatoma of the middle ear. The frequency of clinical and biochemical relapses was determined in a group of 50 patients with polymyositis (PM), dermatomyositis (DM), or overlap syndromes who were followed for periods of up to 13 years. Relapses occurred in 30 of the 50 patients (60%) during the period of follow-up. The annual relapse rate was not significantly different in the three groups of patients. Through register research 21 new cases (10 male, 11 female) of polymyositis/dermatomyositis during the period 1984-1993 in the county of Gävleborg, Sweden, were identified.
