Maternal age-specific drivers of severe maternal morbidity.

BACKGROUND: The maternal age influences the risk of adverse pregnancy outcomes, including severe maternal morbidity. However, the leading drivers of severe maternal morbidity may differ between the maternal age groups. OBJECTIVE: To compare the contribution of different risk factors to the risk of severe maternal morbidity between various maternal age groups and estimate their population-attributable risks. STUDY DESIGN: This was a retrospective, population-based cohort study of all US live births from 2012 to 2016 using birth certificate records. The demographic, medical, and pregnancy fact

A cohort study of cervical screening using partial HPV typing and cytology triage.

HPV testing is more sensitive than cytology for cervical screening. However, to incorporate HPV tests into screening, risk-stratification ("triage") of HPV-positive women is needed to avoid excessive colposcopy and overtreatment. We prospectively evaluated combinations of partial HPV typing (Onclarity, BD) and cytology triage, and explored whether management could be simplified, based on grouping combinations yielding similar 3-year or 18-month CIN3+ risks. We typed ∼9,000 archived specimens, taken at enrollment (2007-2011) into the NCI-Kaiser Permanente Northern California (KPNC) HPV Persiste

Cancer attributable to cigarette smoking by ethnic group in the US in 2022-2023.

BACKGROUND: Smoking patterns vary in the US population. We aimed at estimating the attributable fraction (AF) of smoking-related cancer across racial/ethnic groups. METHODS: To account for ~ 25-year latency period, sex-specific smoking prevalence data from 1994 to 1996 were linked with recent cancer incidence (2022) and mortality (2023) records from the United States Cancer Statistics (USCS) database. Estimates were calculated for Whites, Blacks, Hispanics, Asians/Pacific Islanders and Alaskans/Native Americans. Relative risks for smoking-cancer associations were derived from meta-analyses. T

Cutaneous vitamin D synthesis versus skin cancer development: The Janus faces of solar UV-radiation.

In scientific and public communities, there is an ongoing discussion how to balance between positive and negative effects of solar UV-exposure. On the one hand, solar UV-radiation represents the most important environmental risk factor for the development of non-melanoma skin cancer. Consequently, UV protection is an important measure to prevent these malignancies, especially in risk groups. Otherwise, approximately 90% of all vitamin D needed by the human body has to be formed in the skin through the action of UV-radiation. This dilemma represents a serious problem, for an association of vita

Intersectional disparities in climate vulnerability and cancer risk.

Despite significant progress in the early detection, treatment, and survivorship of cancer in recent decades, cancer disparities continue to plague segments of the US population. Many of these cancer disparities, especially those among historically marginalized racial and ethnic groups and those with lower socioeconomic resources, are caused and perpetuated by social and structural barriers to health. These social and structural barriers, which operate beyond the framework of cancer control, also systematically increase vulnerability to and decrease adaptive capacity for the deleterious effect

Developing an online health intervention for young gay and bisexual men.

Young gay and bisexual men continue to experience increases in HIV incidence in the US highlighting a need for competent health services, while the prominence of the internet in their social and sexual lives calls for novel preventive modalities. Toward this goal, we adapted an efficacious in-office HIV risk reduction intervention for online delivery. This paper describes the development of the online intervention and highlights the results of interviews and focus groups with the original intervention participants regarding effective adaptation and online delivery recommendations. The final in

Are women who have abortions different from those who do not? A secondary analysis of the 1990 national survey of sexual attitudes and lifestyles.

Studies of women undergoing termination of a pregnancy are often used to make recommendations about family planning and health education policy. However, it is not clear how similar the women in these studies are to the general population of women of childbearing age. Our secondary analysis of the National Sexual Attitudes and Lifestyles Survey allowed us to test the hypothesis that women who have undergone an abortion are the same as those who have not (in terms of socio-demographic characteristics, lifestyles, and attitudes). Our findings show significant differences between these two groups

Metagenomic profiling of plaque microbiota in Indian subjects: identified hidden ecological tapestry.

Dental plaque biofilms are the primary etiologic factor for various chronic oral infectious diseases. In recent years, dental plaque shows enormous potential to know about an individual microbiota. Various microbiome studies of oral cavity from different geographical locations reveals abundance of microbial species. Although, the representation of Indian population in this respect is limited, which make us curious to undergo this study. This study investigates the dental plaque microbiota of North Indian individuals based on their age, gender, and dietary patterns; specifically, food preferenc

Estimation of Health and Economic Benefits of a Small Automatic External Defibrillator for Rapid Treatment of Sudden Cardiac Arrest (SMART): A Cost-Effectiveness Analysis.

BACKGROUND: Sudden cardiac arrest (SCA) occurs in 0.4% of the general population and up to 6% or more of at-risk groups each year. Early CPR and defibrillation improves SCA outcomes but access to automatic external defibrillators (AEDs) remains limited. METHODS: Markov models were used to evaluate the cost-effectiveness of a portable SMART (SMall AED for Rapid Treatment of SCA) approach to early SCA management over a life-time horizon in at-risk and not at-risk populations. Simulated patients (n = 600,000) who had not received an implantable cardioverter defibrillator (ICD) were randomized to

Malignant neoplasm of bone and articular cartilage mortality trends in the United States: A 25-year population-based serial cross-sectional study.

Malignant neoplasms of bone and articular cartilage (MNBAC) carry a disproportionately high mortality burden. Long-term mortality patterns may inform targeted public health interventions. This study examines MNBAC-related mortality among adults aged 25 years and older from 1999 to 2023. This study analyzed MNBAC-related mortality from 1999 to 2023 using the CDC WONDER Multiple Cause of Death database and International Classification of Diseases, Tenth Revision codes. Age-adjusted mortality rates (AAMRs) per 100,000 population were standardized to the 2000 US population. Trends were assessed us

Medicaid Patients With ED Visits For Overdose: Disparities In Initiation Of Medications For Opioid Use Disorder.

Medications for opioid use disorder (MOUD) after emergency department (ED) visits for overdose can reduce subsequent overdose deaths, but disparities in receiving MOUD persist in the US. Using national Medicaid claims data from the period 2016-20, we examined racial and ethnic disparities in MOUD initiation after ED visits for opioid overdose. Overall, 6.4 percent of Medicaid ED visits were associated with a claim for MOUD within thirty days. This rate was highest among non-Hispanic White (7.3 percent) patients and lowest among non-Hispanic Black (4.3 percent) and Hispanic (4.9 percent) patien

Understanding consumer and clinician perceptions of a potential Lyme disease vaccine.

Each year, over 450 000 Lyme disease diagnoses are estimated to occur in the United States, and current preventive measures have been insufficient to stem the rising incidence. An effective human Lyme disease vaccine could be a powerful intervention for population-level impact. In advance of new Lyme disease vaccines coming to market, this study explored barriers to acceptability and motivations for the uptake of a new Lyme disease vaccine. Researchers conducted 9 online focus groups among consumers who may potentially benefit from the vaccine and 30 in-depth interviews among clinician groups

"Frog, where are you?" Narratives in children with specific language impairment, early focal brain injury, and Williams syndrome.

In this cross-population study, we use narratives as a context to investigate language development in children from 4 to 12 years of age from three experimental groups: children with early unilateral focal brain damage (FL; N=52); children with specific language impairment (SLI; N=44); children with Williams syndrome (WMS; N=36), and typically developing controls. We compare the developmental trajectories of these groups in the following domains: morphological errors, use of complex syntax, complexity of narrative structure, and types and frequency of evaluative devices. For the children with

The burden of severe pneumococcal infection in Ireland: potential effectiveness and indirect benefits of the 7-valent pneumococcal conjugate vaccine.

This study attempts to overcome the problem of under-reporting of serious pneumococcal infection in the Republic of Ireland by adding a proportion of 'unspecified' disease to pneumococcal disease reports. ICD-9 data for all age groups was collected on meningitis (from the National Disease Surveillance Centre), and septicaemia and pneumonia (from the Hospital In-Patient Enquiry system) for the year 1999. A 7-valent pneumococcal conjugate vaccine (PCV) has been shown to have significant effectiveness in the target paediatric age group and also indirect herd effects on the US adult population. Th

Trends in renal disease morbidity and mortality in the United States, 1979 to 1990.

Four indices of morbidity and mortality due to seven groups of renal diseases are evaluated in the United States for the period 1979 through 1990. These indices include mortality, hospitalization, doctor's office visits, and prevalence. Age-adjusted and age-specific rates are calculated. Estimates are provided for racial-, ethnic-, and gender-specific subpopulations. The burden of some diseases had decreased, especially renal infections. Most indices of the burden of diabetes with renal involvement and hypertensive renal disease have increased, especially among segments of the population that
