Unilateral pseudophakia in children under 4 years.

PURPOSE: We examined the efficacy of intraocular lens implantation (IOL) in children younger than 4 years of age for unilateral aphakic visual rehabilitation. METHODS: Twenty-one patients underwent unilateral cataract extraction, IOL placement, posterior capsulotomy, and anterior vitrectomy between 1990 and 1994. Postoperative vision, refractive change, and complications were monitored prospectively. Two cataract subgroups were analyzed: 12 patients with infantile (congenital and developmental) cataracts, and nine patients with posttraumatic cataracts. RESULTS: Overall age at surgery average…

Optical Quality Degradation Following Nd:YAG Laser-Induced Intraocular Lens Pitting: A Multimodal Experimental Study.

Nd laser posterior capsulotomy is the standard treatment for posterior capsule opacification following cataract surgery. Although generally considered safe, inadvertent laser impacts on the intraocular lens (IOL) optic may induce permanent surface defects that contribute to postoperative dysphotopsias and reduced visual quality. This experimental study investigated the optical consequences of Nd laser-induced damage on two commercially available hydrophobic acrylic IOLs, focusing on retinal light distribution and optical image quality. Two hydrophobic acrylic monofocal IOL models, the CT LUCIA…

Sequential Surgical Management of Refractory Primary Angle-Closure Glaucoma in a Functionally Monocular Patient.

BACKGROUND: Primary angle-closure glaucoma (PACG) carries a high risk of irreversible blindness. This case highlights the challenges of mana … nd hyphema. This necessitated a secondary intervention involving pars plana anterior vitrectomy and posterior capsulotomy. Over an 18-month follow-up, the patient suffered recurrent complications, including a spontaneous vitreous hemorrhage and eventually uncontrolled IOP. Management required a vitrectomy and the subsequent implantation of an Ahmed glaucoma valve. Alth …

Improving resident education through unstable chicken hips: a novel way to teach an infant hip examination.

Current clinical screening for infantile hip dysplasia relies on combined Ortolani and Barlow maneuvers, which have a quoted sensitivity and specificity of 74-98 and 84-99%, respectively. Teaching this delicate physical examination maneuver is difficult as it requires a distinct tactile feel. The purpose of this study is to validate a benchtop learning grocery-bought chicken simulator model of newborn hip instability through a pre- and post-test surveys. This model of hip dysplasia uses whole chickens. A posterior capsulotomy of the chicken's hip joint was performed to create instability. Prov…

Surgical interventions for bilateral congenital cataract.

BACKGROUND: Congenital cataracts are opacities of the lens in one or both eyes of children, causing a reduction in vision bad enough to require surgery. Cataract is the largest preventable cause of visual loss in childhood. Paediatric cataracts provide different challenges to those in adults. Intense inflammation, amblyopia and posterior capsule opacification can affect results of treatment. Two treatments commonly considered for congenital cataract are lensectomy and lens aspiration. OBJECTIVES: The objective of this review is to assess the effects of surgical treatments for bilateral symmet
