Urinary acidification was studied in six unrelated infants with fetal alcohol syndrome and eight healthy age-matched infants. Creatinine clearance, fractional sodium excretion, plasma renin activity, and plasma aldosterone were normal in all patients but fractional potassium excretion was lower in the patients than in the controls (p = 0.0001). After ammonium chloride loading, minimum urine pH was significantly higher in FAS patients than in control subjects (5.5 +/- 0.1 and 4.7 +/- 0.1, respectively, p = 0.00005). Two cases of congenital chloride diarrhoea in newborn infants are presented. The diagnosis was made by a combination of clinical findings--hydramnios, low birth weight, large abdomen, severe dehydration, dyselectrolytemia with hypochloremia and hypokalemia, and metabolic alkalosis. A crucial finding for the confirmation of the diagnosis was the fourfold rise of faecal CI exceeding the sum of sodium and potassium concentrations.

Based on the parameters of the acid-base status, the effect of different anions in potassium and magnesium substitution was studied. A mixture of chloride with the metabolisable anions acetate and malate does not cause any changes in the acid-base balance. The exclusive supply of metabolisable anions in the form of aspartate always results in a change of blood pH values towards an alkalosis.

A case of combined interstitial and intrauterine pregnancy is described. The sonographic finding of a thick myometrial bridge separating a twin pregnancy may be unique to heterotopic cornual pregnancies and is illustrated. The method of selective embryo reduction with potassium chloride to terminate the interstitial pregnancy without damage to the intrauterine pregnancy is also described. The intrauterine pregnancy continued successfully to term.
