When ambulatory monitoring results differ from office values it is very likely that the ambulatory readings more accurately reflect patient status. Thus, a determination of 24-hour antihypertensive control and comparison of the efficacy of drug regimens can help to prevent undertreatment as well as overtreatment.

This paper reviews the relevant pediatric literature about blood pressure (BP) hyperreactivity to stress as a possible precursor of hypertension and coronary heart disease. Two prospective studies of children indicate that BP hyperreactivity predicts later development of hypertension. Several studies have identified correlates of BP reactivity. The authors report on the behavior of the intraocular pressure of 20 patients who had sustained severe head injury. The 8 patients who retained normal intraocular pressure all survived. The 12 patients who developed intraocular hypotony all suffered brain death. Although optic disc edema was not noted in any brain-dead patients, we did find signs of fundus ischemia in most.

Two studies, both retrospective, were done at our institution. The first study evaluated 100 consecutive patients with exfoliation (EX) syndrome found as an incidental finding in a private, general ophthalmology practice, to determine the incidence of glaucoma. Peak pressure and temporal parameters of foot function were determined in 21 men and 11 women; few differences between men and women in any of the parameters were observed, either when walking barefoot or when wearing shoes. However, significant differences were observed when barefoot and shod walking were compared. A review is presented of 508 feet in 310 patients after telescoping osteotomy of the lesser metatarsals for metatarsalgia. The patients were predominantly female (80%), with a mean age of 55 years; the range of follow-up was 1 to 12 years. In 22% of the patients the metatarsalgia was associated with rheumatoid arthritis. Improvements in assessment and modifications in technique are reported and the management of complications is discussed. Timolol maleate produced no significant difference in the initial mean intraocular pressure after keratoplasty using disparate-sized grafts (7.5 mm recipient, 8.0 mm donor). However, timolol did significantly reduce the number of eyes with an intraocular pressure elevation greater than 24 mm Hg. A series of patients with equal-sized donor and recipient cuts (8.0 mm) had initial intraocular pressure of 19.6 mm Hg. The normal intraocular pressure in these patients is thought to be due to superficial suturing, thereby preventing angle distortion.
