The authors report on their experience of 300 open-heart operations performed in Abidjan for cardiac valve disease (149 cases), congenital cardiopathy (100 cases), endomyocardial fibrosis (40 cases) and other lesions. The operational method involved haemodilution, hypothermia, aortic clamping and cardioplegia. The death rate was 13.3%, mostly due to the severity of the condition prior to surgery. A series of 100 patients suffering from trigeminal neuralgia treated by percutaneous radiofrequency thermocoagulation of the trigeminal ganglion is reported. The authors make some considerations about the basic principles of the thermocoagulation and comment a few important methods of treatment used prior to this new technique. Total anomalous pulmonary venous connection to the left vertical vein often may be recognized on the plain postero-anterior chest radiograph by the characteristic supracardiac shadow. This "snowman sign" is useful in older patients but rarely is apparent in infancy. In such young patients the authors have discovered a density anterior to the trachea on the lateral chest radiograph. This finding is present prior to the appearance of the snowman sign and should prove useful in the early diagnosis of this anomaly, facilitating prompt, appropriate treatment. Pediatric catatonia is a complex neuropsychiatric syndrome. Benzodiazepines are standard first-line pharmacotherapy. When benzodiazepines do not provide relief of symptoms, electroconvulsive therapy (ECT) is the most proven effective therapy. Most mediastinal goiters are extensions of cervical goiters. Patients are generally female and only occasionally have symptoms. Patients most commonly complain of a mass-like sensation, dyspnoea, dysphagia and cough. The authors describe the case of a 67 year-old female who presented at the emergency room with dyspnoea, anterior cervical discomfort and tightness which had onset a few months prior but which had worsened in the last week. The patient cited a history of bronchial asthma, for which she was under medication, but the true diagnosis was mediastinal goiter.

A case of a bilateral necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy is described. A 71-year-old man was operated on for esotropia due to prior and equilibrated Grave's disease. Two weeks after surgery, he had a scleral necrosis that was located on the surgery sites (insertion of the two medial rectus muscles). There was no infectious or autoimmune etiology. The authors show a 36-year old female patient with a gigantocellular tumor of the distal metaphysis of the left tibia (stage III by Campanacci). A 7.6 cm long distal articular defect of the tibia has been formed by radical segmentary resection. The defect was compensated by the extension of the rest of the tibia; whereas the support function of the limb was provided through tibio-talar arthrodesis. 5.5 years after the surgery, there are no signs of local relapses; the patient walks without any orthopedic aids and works at the same job as prior to the operation.
