Traumatic Intraperitoneal Bladder Rupture Presenting With Massive Ascites and Acute Kidney Injury Following a Ground-Level Fall.

A previously healthy 39-year-old man presented with progressive abdominal distension and discomfort one week after a high-velocity ground-level fall. He initially sought care elsewhere but was discharged without a definitive diagnosis. On presentation to our institution, he exhibited signs of acute kidney injury, abdominal compartment syndrome, and elevated intra-abdominal pressure. Point-of-care paracentesis revealed 7.9 L of fluid with markedly elevated creatinine, consistent with urinary ascites. A CT urogram confirmed bladder dome rupture. The patient underwent exploratory laparotomy with

Making the diagnosis of degenerative cervical myelopathy in clinical practice: essential evidence-based examination tools for healthcare practitioners.

Degenerative cervical myelopathy (DCM) is a progressive neurological disorder characterized by cervical spinal cord compression, leading to significant disability worldwide. The variability in clinical presentations and the subtle onset of symptoms make early diagnosis challenging, often resulting in delayed intervention. The objective of this study is to provide an evidence-based approach for the clinical examination and diagnosis of DCM, aimed at enhancing the role of general practitioners and allied health providers in early detection. Early identification and appropriate intervention in DC

A case report of secondary haemophagocytic lymphohistiocytosis following chikungunya virus infection: Insights into post-viral immune dysregulation.

Chikungunya virus infection is typically a self limiting febrile illness; however, rare cases may trigger severe immune dysregulation. We report an elderly man who developed secondary hemophagocytic lymphohistiocytosis following confirmed chikungunya infection. He presented with persistent fever, progressive pancytopenia, splenomegaly, and marked hyperferritinemia. Bone marrow examination demonstrated hemophagocytosis, and an HScore of 191 indicated a high probability of reactive Haemophagocytic lymphohistiocytosis. Despite fulfilling diagnostic criteria, the patient remained hemodynamically s

Primary Central Nervous System (CNS) Post-Transplant Lymphoproliferative Disorder: A Case Report.

Post-transplant lymphoproliferative disorder (PTLD) is the most common non-dermatologic malignancy in patients with a history of solid organ transplantation (SOT). The onset of PTLD is not limited to a specific timeframe post-SOT, but most cases are diagnosed within five years of transplantation. While both metastatic spread and multifocal involvement are common, very few of these cases arise from the central nervous system. Here, we present a patient with progressive memory loss 15 years after renal transplantation who was found to have primary central nervous system PTLD. This case underscor

Colonic Adenocarcinoma Presenting as Abdominal Wall Necrosis: A Case Report.

While typical presentations of colorectal cancer include gastrointestinal symptoms such as changes in bowel habits, anemia, and hematochezia, it can also present atypically, including direct invasion of adjacent structures such as the abdominal wall. This report details a rare case of cecal adenocarcinoma manifesting as rapidly progressive abdominal wall necrosis in a 43-year-old woman, highlighting the diagnostic challenge in such unusual presentations. The case underscores the importance of maintaining a high index of suspicion for underlying malignancy in atypical clinical scenarios and ill

The Postthrombotic Syndrome: A Practical Overview.

Postthrombotic syndrome (PTS) is a common, progressive form of chronic venous disease (CVD) with an incidence of up to 70% at 10 years following deep vein thrombosis (DVT). PTS is a clinical diagnosis with patients exhibiting signs and symptoms common to other CVD. Severe symptoms, including venous leg ulcers, are identified in up to 5% of patients. Understanding the diagnosis, pathophysiology, and diagnostic testing affords the provider the opportunity to educate the patient and provide optimal longitudinal care for this disease. Prevention of PTS begins with preventing DVT in patients. Compr

Progressive Global Ataxia With Sensory Changes as a Paraneoplastic Syndrome in a Patient With Chromophobe Renal Cell Carcinoma.

Paraneoplastic syndromes (PNS) are rare and can be challenging to diagnose and treat. The uniqueness of PNS lies in the complexity of presentation, the importance of early diagnosis, and the role of multidisciplinary care in managing those patients to mitigate long-term neurologic complications. We describe a patient with metastatic renal cell carcinoma who presented with a complex constellation of neurological symptoms (progressive global ataxia and sensory changes) that did not resolve following nephrectomy. While complete resolution of symptoms was not achieved, he did have stabilization of

Idiopathic Basal Ganglia Calcification: Fahr's Syndrome, a Rare Disorder.

Fahr's syndrome is a rare neurological disorder with varied clinical manifestations. It is characterized by the progressive deposition of calcium in the walls of the blood vessels of basal ganglia and dentate nuclei of the cerebellum in young and middle-aged people. It is important for neurologists, geneticists, psychiatrists, dentists, and other appropriate care specialists to have a thorough knowledge of this syndrome as any of them could be the first person to diagnose the disease. This case report of Fahr's syndrome presents the signs and symptoms of the patient and the treatment for oral

Coordinated approach to spinal and tracheal reconstruction in a patient with morquio syndrome.

Morquio syndrome (Mucopolysaccharidosis IVA) is an autosomal recessive lysosomal storage disease with manifestations ranging from mild to severe phenotype. Mechanical spinal cord injury and airway insufficiency are major causes of mortality. A 17-year-old male patient with severe Morquio syndrome presented with cervical and upper thoracic spinal stenosis with spinal cord myelopathy, and progressive severe tracheal stenosis. Coordinated care among otolaryngology, orthopedic surgery, neurosurgery, anesthesiology, cardiovascular surgery, radiology, and pulmonology teams facilitated the successful

A Qualitative Approach to Health Related Quality-of-Life in Congenital Muscular Dystrophy.

Congenital muscular dystrophies (CMD) cause progressive muscle weakness resulting in severe motor disabilities. Previous studies focused on the effects of motor disability. Here, we explore other factors affecting health related quality-of-life (HRQOL) in CMD. Qualitative interviews were conducted with participant-parent dyads to identify symptoms having the greatest impact on HRQOL. Symptoms were classified into themes and domains representing physical, mental, social health, and disease specific issues. Social role limitations and specific activity impairment were frequently mentioned. A gre

Risk Factors, Disease Associations, and Dupuytren Diathesis.

Dupuytren disease (DD) is a benign, fibroproliferative disease of unknown cause. The disease predominantly affects the palms of the hands, causing permanent digital contracture of affected digits. DD is a late-onset disease and is often progressive, irreversible, and bilateral. The disease has a significant impact on the health care economy. The mainstay of treatment of DD is surgical excision of diseased palmar fascia. There is evidence of genetic susceptibility. This article introduces the epidemiology of DD and examines the Dupuytren diathesis to highlight the importance of identifying clin

Symptomatic therapy of multiple system atrophy.

Multiple system atrophy is a progressive neurodegenerative disease characterized by the association of autonomic failure and a movement disorder that consist of either a hypokinetic movement disorder or a cerebellar syndrome or both. In addition to these core characteristics other movement disorders (e.g. dystonia, myoclonus, spasticity), and neuropsychiatric symptoms (e.g. depression, cognitive dysfunction) may occur in the course of the disease and can severely impair patients' quality of live. To date no causal therapy is available and therefore symptomatic treatment plays a pivotal role in

Neurological disease may precede lymphadenopathies in Actinomyces europaeus infection.

Actinomyces species are part of the commensal flora of the mucous membranes of the oropharynx, gastrointestinal tract and female genital tract. Actinomyces europaeus is a short, nonmotile, facultative anaerobic rod first described in 1997, susceptible in vitro to a wide range of b-lactam antibiotics which are regarded as first choice. In this report we described the case of A. europaeus infection in a young female patient admitted to Intensive Care Unit and the possible damage of vascular endothelium due to a chronic progressive actinomycosis that at first involved neck soft tissue, then cervi

Multiple Crush Concept Applied to Multiple Nerves in Leprous Neuropathy.

There is a large reservoir of leprosy patients, no longer contagious, due to multidrug therapy, who are considered cured and are becoming increasingly disabled due to progressive chronic nerve entrapment in the upper and lower extremities. After a review of the history of understanding leprous neuropathy, an approach is outlined based on the approach taken to relieve pain and restore sensation that prevents ulcers and amputations in diabetics with neuropathy and superimposed nerve compressions. The results of the first application of this approach in an indigenous area for leprosy, Guayaquil,

Lipoid proteinosis: a rare encounter in dental office.

Lipoid proteinosis is a sporadic congenital metabolic disorder which is characterized by deposition of hyaline material in dermis, submucosal connective tissue, and various internal organs. It has an extremely low prevalence rate with less than 300 cases reported so far. This progressive disease has a vast spectrum of manifestations ranging from asymptomatic lesions to fatal seizures and respiratory obstruction making timely diagnosis of this rare disorder an imperative task for oral health care practitioners. We report a case of characteristic oral manifestations of lipoid proteinosis in a 28

Necrotizing fasciitis: strategies for diagnosis and management.

Necrotizing fasciitis (NF) is uncommon and difficult to diagnose, and it cause progressive morbidity until the infectious process is diagnosed and treated medically and surgically. The literature addressed NF contains confusing information, inaccurate bacteriologic data, and antiquated antibiotic therapy. A delay in diagnosis is associated with a grave prognosis and increased mortality. The main goal of the clinician must be to establish the diagnosis and initially treat the patient within the standard of care. This review is planned as a guide for the clinician in making an early diagnosis of

Doxapram reversal of respiratory failure in a patient refusing assisted ventilation.

A 75-year-old man with severe chronic obstructive pulmonary disease and epistaxis experienced progressive respiratory failure after receiving an injection of meperidine and hydroxyzine in preparation for nasal packing. The patient and his family refused endotracheal intubation; despite aggressive medical care, he continued to deteriorate toward an expected fatal outcome. An i.v. infusion of doxapram hydrochloride at 2 mg/min rapidly reversed his downhill course and hypercarbic coma. This case illustrates the potential usefulness of doxapram in the emergency setting. The mechanism of action, in

Elevating the Standard of Care for Patients with Psoriatic Arthritis: 'Calls to Action' from a Multistakeholder Pan-European Initiative.

INTRODUCTION: Psoriatic arthritis (PsA) is a complex, progressive, and often debilitating disease. Despite recent advances in treatment, numerous unmet needs in patient care persist. Rheumacensus is a multistakeholder, pan-European initiative designed to identify ways to elevate the standard of care (SoC) and treatment ambition for patients with PsA, using the perspectives of three key stakeholder groups: patients, healthcare professionals (HCPs) and payors. METHODS: Rheumacensus followed three phases: an insights-gathering workshop to identify current unmet needs in PsA and an area of focus
