Vestibular function in auditory neuropathy.

Auditory neuropathy is characterized by mild-to-moderate pure-tone hearing loss, poor speech discrimination out of proportion with this loss, absent or abnormal auditory brainstem responses and normal outer hair cell function as measured by otoacoustic emissions and cochlear microphonics. We followed three patients in our clinic whom we classified as auditory neuropathy patients. These patients also complained of balance disorders and we report our auditory and vestibular system analyses of these patients. The data presented herein include results of audiometric tests (serial pure-tone audiome

Multisensory assessment and machine learning for athlete classification in talent identification.

BACKGROUND: Talent identification in elite sport is challenged by maturation confounding and limited objective assessment tools. This preliminary study examined whether visual-vestibular-somatosensory and autonomic (VVS-A) measures distinguished podium-level from entry-level divers using machine learning. OBJECTIVES: (1) Identify VVS-A features distinguishing podium-level divers from a Come and Try group using traditional statistical comparisons; (2) evaluate machine-learning models' ability to classify podium-level athletes; and (3) examine the distribution of classification probabilities us

Chorea-Acanthocytosis: From Motor to Behavioral and Olfactory Profiles.

BACKGROUND: Chorea-acanthocytosis (ChA) clinically presents with motor and non-motor symptoms. Some features of the disease and their intercorrelations have remained insufficiently characterized. Thus, we aimed to perform a multidimensional assessment of ChA patients, focusing on motor, behavioral, cognitive and olfactory domains. METHODS: A cross-sectional clinical assessment of 23 genetically confirmed ChA patients was conducted from 2022 to 2024. Patients were evaluated using the Unified Huntington's Disease Rating Scale (UHDRS) and Mini-Mental State Examination. A case-control comparison

Eye movements and ocular biomarkers in mild traumatic brain injury: from oculomotor pathophysiology to diagnosis and prognosis.

Mild traumatic brain injury (mTBI) is among the most common neurological presentations worldwide, yet its diagnosis remains largely clinical and its prognosis difficult to predict. Structural imaging is typically normal and self-reported symptom scales are insensitive and confounded by effort and comorbidity. Because the generation and control of eye movements recruit an unusually large and anatomically distributed proportion of the brain-from brainstem and cerebellar circuits to parietal, frontal and prefrontal cortices linked by long white-matter tracts-ocular-motor behaviour is sensitive to

Abnormalities in cognitive-related functional connectivity can be used to identify patients with schizophrenia and individuals in clinical high-risk.

BACKGROUND: Clinical high-risk (CHR) refers to prodromal phase before schizophrenia onset, characterized by attenuated psychotic symptoms and functional decline. They exhibit similar but milder cognitive impairments, brain abnormalities and eye movement change compared with first-episode schizophrenia (FSZ). These alterations may increase vulnerability to transitioning to the disease. This study explores cognitive-related functional connectivity (FC) and eye movement abnormalities to examine differences in the progression of schizophrenia. METHODS: Thirty drug-naive FSZ, 28 CHR, and 30 health

Retinal and optic nerve degeneration in α-mannosidosis.

BACKGROUND: α-mannosidosis is a rare, autosomal-recessive, lysosomal storage disease caused by a deficient activity of α-mannosidase. Typical symptoms include intellectual, motor and hearing impairment, facial coarsening, and musculoskeletal abnormalities. Ocular pathologies reported previously were mainly opacities of the cornea and lens, strabismus, and ocular motility disorders. However, retinal and optic nerve degeneration have been rarely described. METHODS: We report ocular findings of 32 patients with α-mannosidosis. We particularly concentrated on retinal abnormalities which we suppor

Antiretroviral treatment is associated with increased attentional load-dependent brain activation in HIV patients.

OBJECTIVE: The purpose of this paper was to determine whether antiretroviral medications, especially the nucleoside analogue reverse transcriptase inhibitors, lead to altered brain activation due to their potential neurotoxic effects in patients with human immunodeficiency virus (HIV) infection. METHODS: Forty-two right-handed men were enrolled in three groups: seronegative controls (SN, n = 18), HIV subjects treated with antiretroviral medications (HIV+ARV, n = 12), or not treated with antiretroviral medications (HIV+NARV, n = 12). Each subject performed a set of visual attention tasks with

Modelling of the concentration--effect relationship of THC on central nervous system parameters and heart rate -- insight into its mechanisms of action and a tool for clinical research and development of cannabinoids.

Pharmacokinetics after pulmonary administration of delta-9-tetrahydrocannabinol (THC) and its major metabolites 11-OH-THC and 11-nor-9-COOH-THC was quantified. Additionally, the relationship between THC and its effects on heart rate, body sway and several visual analogue scales was investigated using pharmacokinetic-pharmacodynamic (PK-PD) modelling. This provided insights useful for the research and development of novel cannabinoids and the physiology and pharmacology of cannabinoid systems. First, the PK-PD model gave information reflecting various aspects of cannabinoid systems. The delay b

Facilitated extinction but impaired extinction recall by eye movement manipulation in humans - Indications for action mechanisms and the applicability of eye movement desensitization.

Eye movement desensitization and reprocessing (EMDR) therapy utilizes the manipulation of eye movements to reduce affective distress during fear-exposure. Animal research recently suggested a potential neural mechanism underlying these effects, by which increased activity of the superior colliculus (SC), mediating visual attention, increases the inhibition of the basolateral amygdala (BLA), mediating defensive plasticity. We tested such mechanism in forty healthy humans using a multiple-day single-cue fear conditioning and extinction paradigm. The activity of the SC during extinction was exper

Acute binocular diplopia: peripheral or central?

OBJECTIVES: Acute diplopia is a diagnostic challenge for clinicians, in particular in the emergency department. The most common cause of acute diplopia are ocular motor nerve palsies (OMP). In this prospective study, we focused on identifying the most crucial signs and symptoms for differentiating between peripheral and central OMP. METHODS: We prospectively evaluated 56 non-consecutive patients who presented at our emergency department with acute binocular diplopia (≤ 10 days). The patient history was taken using a standardized questionnaire and patients underwent a neurological, neuro-ophth

What Is Behind Cerebellar Vertigo and Dizziness?

The differential diagnosis of vertigo or dizziness as a result of cerebellar disorders can be difficult as many patients with a cerebellar pathology do not present with the full spectrum of cerebellar signs. The main goal of this study was to describe the typical clinical features of these patients with vertigo or dizziness of a cerebellar origin. We reviewed the medical records of 5400 patients with vertigo and dizziness from our tertiary outpatient clinic for vertigo and balance disorders. In 459 the diagnosis of "cerebellar vertigo or dizziness" was made; 90 patients were excluded from furt

[Functional analysis of the thalamocortical pathways in eye movements].

Although the roles of the thalamocortical pathways in somatic movements are well documented, their roles in eye movements have only recently been examined. The oculomotor-related areas in the frontal cortex receive inputs from the basal ganglia and the cerebellum via the thalamus. Consistent with this, neurons in the paralaminar part of the ventrolateral (VL), ventroanterior (VA), and mediodorsal (MD) nuclei and those in the intralaminar nuclei exhibit a variety of eye movement-related responses. To date, the thalamocortical pathways are known to play at least 2 roles in eye movements. First,

Impaired oculomotor function in a community-based patient population with newly diagnosed idiopathic parkinsonism.

The differential diagnosis of idiopathic parkinsonism can be very challenging, especially early in the course of the disease. Oculomotor function has been reported to differ between the diseases constituting idiopathic parkinsonism. A detailed examination of the oculomotor functions could thus possibly be useful in the early differential diagnostic procedure. Oculomotor function could also differ between subgroups of patients with Parkinson's disease (PD). We examined the oculomotor function in a population-based incidence cohort with newly diagnosed idiopathic parkinsonism and 38 controls. We
