Questions and answers on proper peri-operative management of antiplatelet therapy after coronary stent implantation to prevent stent thrombosis.

Stent thrombosis (ST) is a rare but life-threatening complication of coronary artery stenting. Although dual-antiplatelet therapy is an effective management strategy in reducing the risk for ST, some patients may need to interrupt their regimens because of unforeseen circumstances, such as the requirement for surgery. In conclusion, this case presentation highlights some pertinent issues related to ST, including its risk factors, the perioperative management of antiplatelet agents, and treatment for ST.

Control and prevention of peritoneal adhesions in gynecologic surgery.

Postoperative adhesion formation is a natural consequence of surgical tissue trauma and healing and may result in infertility, pain, and bowel obstruction. Microsurgical principles and minimally invasive surgery may help decrease adhesion formation, but anti-inflammatory agents and peritoneal instillates have no demonstrable benefit. Although some surgical barriers are effective for reducing postoperative adhesions, none has been shown to improve fertility or to decrease pain or the incidence of postoperative bowel obstruction.

Omega-3 Fatty Acids for the Management of Hypertriglyceridemia: A Science Advisory From the American Heart Association.

Hypertriglyceridemia (triglycerides 200-499 mg/dL) is relatively common in the United States, whereas more severe triglyceride elevations (very high triglycerides, ≥500 mg/dL) are far less frequently observed. Both are becoming increasingly prevalent in the United States and elsewhere, likely driven in large part by growing rates of obesity and diabetes mellitus. In a 2002 American Heart Association scientific statement, the omega-3 fatty acids (n-3 FAs) eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) were recommended (at a dose of 2-4 g/d) for reducing triglycerides in patients wit

SGLT2 inhibitors to control glycemia in type 2 diabetes mellitus: a new approach to an old problem.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors are a new class of antidiabetic agents with a novel insulin-independent mechanism of action. The SGLT2 is a transporter found in the proximal tubule of the kidney and is responsible for approximately 90% of renal glucose reabsorption. The SGLT2 inhibitors reduce reabsorption of glucose in the kidney, resulting in glucose excretion in the urine (50-90 g of ~180 g filtered by the kidneys daily), which in turn lowers plasma glucose levels in people with diabetes. The insulin-independent mechanism of action of SGLT2 inhibitors dictates that they ar

Chemoprophylaxis in gastrointestinal tumors.

Chemoprevention of gastrointestinal tumors uses natural or synthetic agents to arrest, retard or reverse the carcinogenesis process. The prospect of prevention is clearly appealing, especially for colorectal cancer (CRC), that represents the second most common cause of cancer-related death in the Western world. Aspirin is the best studied chemopreventive agent for CRC, with randomized trials demonstrating its efficacy in reducing recurrence of colorectal adenomas in higher risk patients. Optimal chemoprevention requires long-term use and high dose of aspirin that may increase the risk of gastr

Management of cardiovascular risk with RAS inhibitor/CCB combination therapy.

Hypertension is a significant risk factor for cardiovascular disease (CVD), including stroke, myocardial infarction, kidney disease and heart failure. Considerable research has been undertaken to delineate the differential effects of various classes of antihypertensive agents in delaying or preventing cardiovascular morbidity and mortality. Although possible benefits may result from specific agents or classes of agents in certain high-risk subgroups, prompt and intensive blood pressure (BP) reduction to target levels remains the most crucial factor in this benefit. Despite this, the BP remains

Advancements in Colon Cancer Therapy: A Review on Docetaxel and the Role of Nanotechnology in Enhancing Efficacy.

Docetaxel exhibits high protein binding and P450-mediated metabolism, which influences its pharmacokinetics. As a taxane-family chemotherapeutic agent, it stabilizes microtubules and disrupts cell division. Combining docetaxel with agents like 5-fluorouracil and oxaliplatin enhances treatment outcomes for colon cancer. Emerging drug delivery systems, including nanoparticles and micelles, aim to improve efficacy while reducing toxicity. However, its safety during pregnancy remains uncertain, and ongoing clinical trials continue to evaluate its therapeutic potential.

Triple inhaler therapy in adolescents and adults with moderate or severe persistent asthma.

Expert guidelines, meta-analyses, and multiple randomized controlled trials have demonstrated the effectiveness of long-acting inhaled antimuscarinic agents (LAMAs) as an additive medication for patients with poorly controlled moderate or severe persistent asthma. LAMAs play an essential role in blocking acetylcholine binding to muscarinic receptors and reducing bronchoconstriction and mucus production. By adding this medication to other combination inhalers, patients can use a triple inhaler to improve FEV1 values and reduce exacerbations.

The pharmacotherapy of panic disorder.

Panic disorder is common and associated with significant morbidity and dysfunction. The pharmacologic treatment of panic disorder is aimed at reducing or eliminating panic attacks, avoidance behavior, anticipatory anxiety, and comorbid conditions--and substantially improving and normalizing overall function and quality of life. Antidepressants and benzodiazepines remain the current mainstays of pharmacotherapy for panic disorder, although other novel agents and strategies are becoming available and may add effective alternatives to the therapeutic armamentarium.

Mechanisms and Potential Benefits of Neuroprotective Agents in Neurological Health.

The brain contains many interconnected and complex cellular and molecular mechanisms. Injury to the brain causes permanent dysfunctions in these mechanisms. So, it continues to be an area where surgical intervention cannot be performed except for the removal of tumors and the repair of some aneurysms. Some agents that can cross the blood-brain barrier and reach neurons show neuroprotective effects in the brain due to their anti-apoptotic, anti-inflammatory and antioxidant properties. In particular, some agents act by reducing or modulating the accumulation of protein aggregates in neurodegener

Mucoactive agents for acute respiratory failure in the critically ill: a systematic review and meta-analysis.

PURPOSE: Acute respiratory failure (ARF) is a common cause of admission to intensive care units (ICUs). Mucoactive agents are medications that promote mucus clearance and are frequently administered in patients with ARF, despite a lack of evidence to underpin clinical decision making. The aim of this systematic review was to determine if the use of mucoactive agents in patients with ARF improves clinical outcomes. METHODS: We searched electronic and grey literature (January 2020). Two reviewers independently screened, selected, extracted data and quality assessed studies. We included trials o

The potential role of controlled-release delivery systems for chemotherapeutic agents in periodontics.

Following advances in periodontal disease etiology and pathogenesis, treatment strategies have evolved to eliminate specific pathogens or suppress the destructive host response. Research indicates that chemotherapeutic agents such as antimicrobials and antimetabolites can alter disease progression; consequently, three delivery systems have been investigated: systemic, topical, and controlled release. Using polymers to control drug administration, controlled-release delivery systems theoretically produce concentration profiles that are more constant and longer lasting than those of other system

Injectable antiplatelet therapies in acute coronary syndromes: Current status and Emerging Therapies. A position paper of the Task Force of Antithrombotic therapy of the ACVC.

Injectable antiplatelet agents can provide immediate and effective platelet inhibition in patients with acute coronary syndromes (ACS). These agents are able to overcome many of the limitations of oral antiplatelet agents, and maybe of particular benefit in patients with ST-elevation myocardial infarction (STEMI), especially those presenting with cardiogenic shock or rescuscitated cardiac arrest. In these patients, factors such as vomiting, altered physiology, sedation, mechanical ventilation or therapeutic hypothermia can impair drug absorption, reducing the intended antiplatelet effect and i
