Based on experimental studies, sustained total central retinal artery obstruction (CRAO) lasting more than one and one-half hours has been considered to cause irreversible retinal damage. We have seen five cases of patients with pure CRAO that lasted longer than one and one-half hours who regained significant visual acuity. Three of the patients had a completely spontaneous improvement in visual acuity to 20/50 or better. In addition, the other two patients regained significant vision to 20/80 or better with a modified treatment regimen of acetazolamide, inhalation of 5% CO2 and 95% O2, and as…

A 33-year-old woman with retinal artery occlusion also had Sneddon's disease (livedo reticularis, neurologic abnormalities, and labile hypertension). While retinal artery occlusive disease obviously must be added to the symptomatic spectrum of this rare disorder, the detection of antiphospholipid antibodies in this patient has pathogenetic implications.

Central retinal artery occlusion usually results in blindness. The association between central retinal artery occlusion and extracranial carotid disease has not been clearly delineated. We reviewed the case reports of 62 patients with central retinal artery occlusion, 25 of whom underwent carotid angiography as part of the diagnostic evaluation. Fourteen of the 25 (56 percent) were found to have ipsilateral extracranial carotid disease. These patients did not generally have carotid bruits and had normal noninvasive carotid tests. Ten patients underwent ipsilateral carotid endarterectomy; these…

OBJECTIVE: Retinal artery occlusion (RAO) is rarely seen as a complication in patients undergoing carotid artery stenting (CAS); hence, its characteristics have not been documented in detail. This study aimed to investigate the incidence of this complication and the related risk factors, focusing on differences in ophthalmic artery (OA) supply (whether by the external or internal carotid artery [ECA or ICA]) prior to CAS procedures.
MATERIALS AND METHODS: We retrospectively examined 342 patients who underwent CAS for severe and/or symptomatic carotid artery stenosis between January 2009 and D…

A 65-year-old woman with left hemiparesis and sudden loss of visual acuity in her right eye presented a few hours after cosmetic injection o … multiple lesions compatible with recent infarction were detected. The authors diagnosed multibranch retinal artery and vein occlusion in the right ophthalmic and middle cerebral arteries due to fat emboli. This case emphasizes the need to reevaluate the safety of such aesthetic procedures, particularly in the facial zone to prevent devastating complications.

Reported is a 68-year-old male with left central retinal artery occlusion (CRAO) followed by cerebral infarction due to left internal carotid artery disease. He was referred to our hospital diagnosed as having left CRAO. In view of severe stenosis of ipsilateral internal carotid artery by ultrasound duplex scan, carotid endarterectomy (CEA) was indicated. While waiting for the operation, he suffered from monoparesis in his right lower limb despite the administration of antiplatelet agents. Brain MRI showed cerebral infarction in the left frontal cortex. It was suggested that artery-to-artery e…

The purpose of this study is to describe a surgical technique and report the functional outcome of treating nonarteritic central retinal artery occlusion (CRAO) in 2 eyes of two consecutive male patients. Two males (A) and (B) presented 6 days and few hours, respectively, after symptom onset with the clinical features typical of CRAO, including sudden severe deterioration of vision to counting fingers and a cherry-red spot in the left and right fovea, respectively. While A had been on recent treatment for hypertension and hyperlipidemia, B had no significant medical history. Both patients unde…

PURPOSE: To evaluate blood flow parameters in the ophthalmic artery and central retinal artery in children with retinopathy of prematurity (ROP).
MATERIAL AND METHODS: The study comprised 57 premature children born between 24 and 33 weeks of gestation, with birth weight from 600 g to 1660 g, including 42 preterm children with retinopathy in stage 1, 2, 3 and 3 "plus" and 15 preterm children without retinopathy. Color Doppler ultrasonography (USG-CD) was used to measure in the studied vessels maximal systolic velocity (Vmax), end-diastolic velocity (Vmin) and resistance index (RI).
RESULTS: S…

INTRODUCTION: Simultaneous branch retinal artery occlusion (BRAO) and central retinal vein occlusion (CRVO) is an exceptionally rare cause of acute vision loss, particularly in young individuals without systemic vascular risk factors. Blood transfusion has been implicated in rare thromboembolic and hyperviscosity-related complications but is infrequently associated with retinal vascular occlusions.
CASE PRESENTATION: A 20-year-old man presented with sudden painless unilateral vision loss shortly after receiving a packed red blood cell transfusion for severe anemia. Ophthalmic examination reve…

BACKGROUND: Central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO) may be a result of platelet-fibrin emboli, cholesterol plaques, or calcific emboli that typically lodge at the bifurcation of retinal arterioles or at a location as the vessel narrows. Hollenhorst plaques (cholesterol emboli) can often be seen at the site of vessel obstruction. Obstruction of blood flow results in partial or complete retinal ischemia and sudden loss of vision. There is no effective therapy for CRAO and BRAO. The authors employed the Nd:YAG laser to photodisrupt emboli within the cent…

BACKGROUND: Central retinal artery occlusion (CRAO) rapidly produces inner retinal ischemia and irreversible vision loss. Although many therapeutic interventions have been proposed, no interventions have proven effective in restoring vision in large randomized controlled trials and final visual outcome in most patients is very poor.
METHODS: Retrospective case series.
RESULTS: We describe 2 cases of CRAO occurring after uncomplicated cataract surgery under topical anesthesia and rapidly diagnosed. Both had very severe vision loss at presentation with dramatic improvement after intra-ophthalm…

Central retinal artery occlusion is a rare association of traumatic optic neuropathy. Superselective fibrinolytic therapy is a management option for central artery occlusion that reduces systemic side effects of thrombolysis. A rare case of posttraumatic central retinal artery occlusion is described where optic nerve decompression was performed followed by restoration of central retinal flow by direct puncture of ophthalmic artery and institution of fibrinolytic therapy.

INTRODUCTION: We report a case of a 66-year-old woman presenting a central retinal artery occlusion with no cardiovascular risk factor, with assessment using supra-aortic artery ultrasonography showing total internal carotid artery thrombosis.
OBSERVATION: When vascular thrombosis risk factors are absent, more in-depth assessment such as a supra-aortic artery angioscan can provide the diagnosis of arterial dissection on arterial fibromuscular dysplasia.
CONCLUSION: Central retinal artery occlusion is a rare but severe pathology. Therefore it is very important not to neglect the etiological a…

PURPOSE: To investigate long-term changes in retinal nerve fiber layer (RNFL) and ganglion cell-inner plexiform layer (GC-IPL) thickness in patients with posterior cerebral artery (PCA) infarction and homonymous visual field defects.
METHODS: This retrospective study included patients with PCA infarction and homonymous visual field defects with a minimum follow-up of 6 months. Retinal thinning was defined as a ≥10% decrease from baseline to the final follow-up examination. Affected and non-affected sectors of the RNFL and GC-IPL were defined according to the retinotopic pattern of the homonym
