OBJECTIVE: Minimal Access Surgery (MAS) for Congenital Diaphragmatic Hernia (CDH) repair is well described, yet only a minority of surgeons report this as their preferred operative approach. Some surgeons find it particularly difficult to repair the defect using MAS and convert to laparotomy when a patch is required. We present in this study our institutional experience in using an easy and relatively cheap methodology to anchor the patch around the ribs using Endo Close™. Blunt chest trauma frequently leads to various complications such as pneumothorax, hemothorax, and lung contusion. Since neighboring organ injury caused by a rib fracture with chest trauma could be fatal when a great vessel is involved, immediate diagnosis and treatment including surgery are important. Here, we present a case of chest trauma, in which we performed video-assisted thoracoscopic rib resection to prevent injury to the descending aorta by the fractured rib tip.

The structures of the chest wall and thoracic outlet are complex. A working knowledge of their anatomy and of its variations is essential to any thoracic surgeon working in the area. Correlating imaging with anatomy is just as important if one wants to recognize surgical indications, and potential operating difficulties. In the past, conventional radiographic examination was the norm but interpretation was often difficult and incomplete. Currently, CT and MRI are the best available imaging tools, and most times they have complementary roles in the evaluation of chest wall anatomy.

BACKGROUND: The "rib-sparing" method of autologous breast reconstruction has gained popularity alongside the historically used "rib-sacrificing" technique. Literature has endorsed benefits of the "rib-sparing" technique, including reduced contour irregularities, postoperative morphine use, and operative time.
