Genetic basis of hereditary hypophosphataemic rickets and phenotype presentation in children and adults.

Hypophosphataemic rickets (HR) is a genetic disorder causing defects in the renal handling of phosphorus, resulting in rickets. HR can be classified into two groups. First- those with excess fibroblast growth factor 23(FGF23) levels, which are due to gene mutations in extrarenal factors and include X-linked dominant hypophosphataemic rickets (XLHR), autosomal dominant hypophosphataemic rickets (ADHR), autosomal recessive hypophosphataemic rickets (ARHR), and hypophosphataemic rickets with hyperparathyroidism. Second- those with normal or low FGF23, which are caused by gene mutations in renal t…

Monteggia Fracture in an Extremely Preterm Infant with Biochemical Rickets: A Case Report.

CASE: A preterm neonate with biochemical rickets is found to have a Monteggia fracture. The infant underwent percutaneous pinning. There was loss of fixation; however, the infant has been followed since discharge from the hospital and has completely healed with full range of motion. CONCLUSIONS: The medical management of this entity involves enteral feedings and optimization of nutrients. The optimal surgical treatment of this injury in the neonatal period is not yet known, although percutaneous pinning resulted in normal healing and function. These aspects require clinician awareness of this…

[Rickets and anaemia caused by coeliac disease in a three-year-old girl].

We present a case report of a three-year-old girl of Pakistani origin with coeliac disease. The first symptom was spontaneous fracture of the fibula. Vitamin D deficiency rickets and iron deficiency anaemia were demonstrated. An intestinal biopsy and elevated antibody levels confirmed the coeliac disease. Rickets is a known but rare presentation of coeliac disease, but this case report illustrates, how coeliac disease in children often presents with non-gastrointestinal symptoms. The importance of investigations for malabsorp-tion, if iron deficiency is not easily corrected with sufficient iro…

Iron Supplementation Associated With Loss of Phenotype in Autosomal Dominant Hypophosphatemic Rickets.

CONTEXT: Autosomal dominant hypophosphatemic rickets (ADHR) is the only hereditary disorder of renal phosphate wasting in which patients may regain the ability to conserve phosphate. Low iron status plays a role in the pathophysiology of ADHR. OBJECTIVE: This study reports of a girl with ADHR, iron deficiency, and a paternal history of hypophosphatemic rickets that resolved without treatment. The girl's biochemical phenotype resolved with iron supplementation. SUBJECTS: A 26-month-old girl presented with typical features of hypophosphatemic rickets, short stature (79 cm; -2.82 SDS), and iron…

RICKETS AT THE MEDICI COURT OF FLORENCE: THE CASE OF DON FILIPPINO (1577-1582).

Among the children found in the crypt of the Grand Duke Giangastone in S. Lorenzo Basilica (Florence), the skeletal remains of a 5-year-old … g of femora, tibiae and fibulae. Differential diagnosis suggests that Don Filippino was affected by rickets. The occurrence of this metabolic disease related to vitamin D deficiency in a Renaissance high social class individual can be explained by the practice of very prolonged breast-feeding, up until two years of age. Maternal milk contains insufficient …

[Updates on rickets and osteomalacia: vitamin D deficiency : its pathophysiology and treatment].

Vitamin D deficiency is a common cause of rickets/osteomalacia. Milder decrease in vitamin D store, vitamin D insufficiency, does not lead to clinical rickets/osoteomalacia but could result in mineralization defect and an increased fracture risk. Vitamin D store is easily assessed by serum 25 (OH) D concentration : less than 20 ng/mL is generally considered vitamin D deficiency whereas between 20 and 30 ng/mL vitamin D insufficiency. In the management of bone and mineral disorders including rickets/osteomalacia, it is a common practice in many countries to measure serum 25 (OH) D, and if defic…

Seizure as a presenting manifestation of vitamin D dependent rickets type 1.

There are two types of vitamin D dependent rickets (VDDR) that cause rickets in children. VDDR type 1 (VDDR-I) is caused by an inborn error of vitamin D metabolism, which interferes with renal conversion of calcidiol (25OHD) to calcitriol (1,25(OH)2D) by the enzyme 1-α-hydroxylase. Patients with VDDR-I have mutations of chromosome 12 that affect the gene for the enzyme 1-α-hydroxylase, resulting in decreased levels of 1,25(OH) vitamin D. Clinical features include growth failure, hypotonia, weakness, rachitic rosary, convulsions, tetany, open fontanels and pathologic fractures. We report a case…

Prevalence and risk factors of vitamin D deficiency rickets in Hokkaido, Japan.

BACKGROUND: Resurgence of vitamin D deficiency rickets has been recognized worldwide. While many cases of this disease have been reported in Hokkaido, the northern island of Japan, no prevalence data is available. Here, we investigated the prevalence and risk factors of vitamin D deficiency rickets in Hokkaido. METHODS: A specially designed questionnaire was sent to 84 major pediatric departments of hospitals in Hokkaido to collect information of the confirmed cases between July 1999 and June 2004. RESULTS: Sixty-seven hospitals responded to the questionnaire. Of these, 20 hospitals reported…

A case of X-linked hypophosphatemic rickets: complications and the therapeutic use of cinacalcet.

In hypophosphatemic rickets, there are both inherited and acquired forms, where X-linked dominant hypophosphatemic rickets (XLH) is the most prevalent genetic form and caused by mutations in the phosphate-regulating endopeptidase (PHEX) gene. XLH is associated with growth retardation and bone deformities. The renal tubular cells have an important role in calcium and phosphate metabolism, where the 1alpha-hydroxylase enzyme metabolizes the conversion of 25 (OH)-vitamin D to potent 1,25 (OH)2-vitamin D, whereas the sodium-phosphate transporter controls tubular phosphate reabsorption. The pathoph…

Apparent efficacy of food-based calcium supplementation in preventing rickets in Bangladesh.

To determine whether increased Ca intakes can prevent rickets in a susceptible group of children living in a rickets-endemic area of Bangladesh, we conducted a 13-month long, double-blind, clinical trial with 1-to 5-year-old children who did not present with rickets but ranked in the upper decile of plasma alkaline phosphatase (AP) activity of a screening cohort of 1,749 children. A total of 158 children were randomized to a milk-powder-based dietary supplement given daily, 6 days/week, and providing either 50, 250, or 500 mg Ca, or 500 mg Ca plus multivitamins, iron, and zinc. Upon initial sc…

10-year case review of nutritional rickets in Children's Hospital of Michigan.

Nutritional rickets has been on the rise in the United States. A chart review of patients with nutritional rickets from April 1995 to May 2005 was performed. Fifty-eight subjects were studied (62% males, 38% females, with an age range between 2 and 132 months). Of the subjects, 81% were African Americans and 14% were Arabic; 33% were Christians and 19% were Muslims. An increasing number of cases of nutritional rickets have been noted since 2000. Seventy-nine percent of patients with nutritional rickets presented at the emergency department, and in 69% of the cases, rickets was an incidental fi…

Hypophosphatemia leads to rickets by impairing caspase-mediated apoptosis of hypertrophic chondrocytes.

Rickets is seen in association with vitamin D deficiency and in several genetic disorders associated with abnormal mineral ion homeostasis. Studies in vitamin D receptor (VDR)-null mice have demonstrated that expansion of the late hypertrophic chondrocyte layer, characteristic of rickets, is secondary to impaired apoptosis of these cells. The observation that normalization of mineral ion homeostasis in the VDR-null mice prevents rachitic changes suggests that rickets is secondary to hypocalcemia, hypophosphatemia, or hyperparathyroidism, rather than impaired VDR action. To determine which of t…
