Electrogastrographic and autonomic nervous system responses to solid and liquid meals in youth with functional dyspepsia.

BACKGROUND: Meal-related symptoms are common in paediatric functional dyspepsia (FD). There are only a small number of paediatric studies assessing mechanisms for meal-related symptoms, and these have not utilized Rome IV criteria. The aim of the current study was to assess gastric myoelectric and autonomic nervous system (ANS) responses to both liquid and solid meals in youth with Rome IV-defined FD. METHODS: In healthy controls (N = 14) and youth with FD (N = 12), we recorded electrocardiograph (to assess heart rate variability; HRV) and electrogastrograph (EGG) signals before and after two…

Examination of serum pepsinogen in functional dyspepsia.

BACKGROUND/AIMS: Although serum pepsinogen (PG) is considered as a marker of gastric atrophy, it also reflects gastric acid secretion, which closely influences dyspeptic symptoms. We investigated serum PG levels and PGI/PGII ratios in dyspeptic patients, in relation to various different subtypes of symptoms including Rome III classifications. METHODOLOGY: Serum PGs were measured in 75 subjects with dyspeptic symptoms and 42 asymptomatic healthy subjects. RESULTS: PG II level was significantly higher (p=0.0001) and PG I/II ratio was significantly lower (p<0.0001) in subjects with H. pylori in…

Functional esophageal disorders: pharmacological options.

Functional esophageal disorders are a group of disorders that cause esophageal symptoms, although with negative results on investigation with standard esophageal tests. Therefore, structural disorders, motility disorders with a histopathological basis and gastroesophageal reflux disease (GERD) are excluded. They are frequently encountered by clinicians, and so a systematic and evidence-based approach to investigation, diagnosis and treatment are crucial. There are four functional esophageal disorders defined by the ROME III consensus, namely functional heartburn, functional chest pain, functio…

National Prevalence of Functional Gastrointestinal Disorders in Jordanian Children.

INTRODUCTION: Functional gastrointestinal disorders are common in children. After the introduction of Rome criteria for diagnosis, assessment of prevalence of such disorders became an attainable goal. Since data from our part of the world are scarce, this study aimed at estimating the prevalence of functional gastrointestinal disorders in Jordanian children. PATIENTS AND METHODS: In a school-based cross-sectional study, recruited children between the age of 4 and 18 were asked to fill the Arabic version of the Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III Version (QPGS-RIII). …

The irritable bowel syndrome and psychiatric disorders in the community: is there a link?

OBJECTIVE: Psychiatric morbidity is high among patients who present to referral centers with irritable bowel syndrome (IBS). However, few studies have investigated the relationship between psychiatric disturbance and IBS in community samples. We hypothesized that psychiatric disorders are linked to IBS in the general community, but this is influenced by the criteria used to establish a diagnosis of IBS. METHODS: The data were collected from a birth cohort born in Dunedin (New Zealand) between April 1972 and March 1973. This cohort consisted of 1037 members (52% male), who were assessed at age…

The rules of drug taking: wine and poppy derivatives in the ancient world. VII. A ritual use of poppy derivatives?

Besides fertility, poppies have been used to symbolize sleep, night, and death. Consistent with the agrarian origin of their ritual use, poppies also became a symbol of reincarnation. Several literary and iconographic sources, in particular of the early Roman imperial age, are here interpreted as evidence that poppy derivatives were ingested during mystery rites. The reversible narcotic effects of poppy derivatives should have allowed a "realistic" representation of death and reincarnation, as intended by the Orphic belief of the transmigration of souls.

Impact of different exposure models and spatial resolution on the long-term effects of air pollution.

Long-term exposure to air pollution has been related to mortality in several epidemiological studies. The investigations have assessed exposure using various methods achieving different accuracy in predicting air pollutants concentrations. The comparison of the health effects estimates are therefore challenging. This paper aims to compare the effect estimates of the long-term effects of air pollutants (particulate matter with aerodynamic diameter less than 10 μm, PM10, and nitrogen dioxide, NO2) on cause-specific mortality in the Rome Longitudinal Study, using exposure estimates obtained with …

Bowel habits and functional bowel disorders in Sri Lankan adults: A cross-sectional study.

BACKGROUND: Definitions of normal bowel habits vary across populations and are influenced by diet, lifestyle and cultural factors. Sri Lanka lacks population-level data describing normative bowel habits and the prevalence of functional bowel disorders. METHODS: A descriptive cross-sectional study was conducted among 501 Sri Lankan adults (aged 18-80 years) utilizing a hospital attendant-based sample with community representation. Data was collected using a structured interviewer-administered questionnaire incorporating the Bristol Stool Form Scale and ROME-IV diagnostic criteria. RESULTS: Th…

Immune Development in Early Life (IDEaL) longitudinal cohort study protocol: Identifying biomarkers of vaccine responsiveness, respiratory infection, and asthma.

BACKGROUND: Early-life immune development is a critical factor in predicting the risk of childhood respiratory infections, asthma, and poor vaccine responses. Identifying immune endotypes that predispose children to these conditions could lead to the development of predictive biomarkers and early interventions, potentially improving long-term health outcomes. The IDEaL (Immune Development in Early Life)-Rome prospective pediatric cohort, based at Children's Hospital Bambino Gesù (Rome, Italy), is part of a National Institutes of Health/National Institute of Allergy and Infectious Diseases-supp…

Navigating the Maze of Functional Dyspepsia: Emergence of a New Entity, Postprandial Epigastric Pain Syndrome.

BACKGROUND & AIMS: The Rome IV criteria subdivide functional dyspepsia (FD) into postprandial distress syndrome (PDS) with meal-related symptoms (early satiation and/or postprandial fullness) and epigastric pain syndrome (EPS) with meal-unrelated symptoms (epigastric pain and/or burning). We aimed to systematically assess the relationship of epigastric pain to meal ingestion in 4 independent FD databases. METHODS: In total, 1337 patients with FD were included. Three cohorts (local, national, and international; total n = 971) completed a Rome IV symptom questionnaire, with additional questions…

Testing the Food Insecurity Experience Scale across urban and regional contexts in Italy.

This study addresses a central measurement challenge in high-income countries: how to detect and characterize food insecurity (FI) when national prevalence is low but territorially uneven. Using original survey data collected across Italian regions and in Rome, we assess the validity and local applicability of the Food Insecurity Experience Scale (FIES). The analysis yields estimated prevalence of moderate or severe FI of approximately 13.5% in the regional sample and 7.1% in Rome. Although these figures are application-specific and should not be read as population-level prevalence estimates, …

Clinical diagnostic criteria for gout: comparison with the gold standard of synovial fluid crystal analysis.

BACKGROUND: Several classification or diagnostic criteria sets for gout have been proposed but none validated. OBJECTIVE: This small pilot study considered urate crystal identification as the gold standard for diagnosis and compared the clinical aspects of 3 proposed criteria sets with that standard. METHODS: Eighty-two subjects who had synovial fluid analyses in a VA medical center were studied. ARA (ACR), Rome, and New York clinical criteria sets and individual criteria were recorded in the 30 patients who had urate crystals versus the remainder with no urate crystals. RESULTS: Presence o…

[A new surgical technique to reduce genital-urinary prolapse].

The authors describe a new surgical technique to reduce the prolapse of genitals and urinary organs. In retrospect, we studied 36 women--between the ages of 59 and 85--who had been hospitalized from 1988 through 1990 at the Ospedale di S. Anna of Rome and from 1991 through May 1994 at the Ospedale Sandro Pertini of Rome. Twenty-eight out of 36 patients presented a III prolapse of the uterus with cystorectocele; 4 patients had a prolapse of the cupola vaginae after hysterectomy; 4 patients presented a prolapse of the residual stump after sub-total hysterectomy. In addition, 8 out of 36 patients…
