BACKGROUND: Up until now there has existed no precise, agreed-upon terminology for referring to the areas of the balding scalp.
OBJECTIVE: A standardized nomenclature system for the areas of the balding scalp is proposed so that physicians and other paraprofessionals can have a common, precise language for communicating with each other.
METHODS: The following, in its initial form, was proposed to the surgical hair restoration community in the Hair Transplant Forum International in 1998. This final proposal includes feedback and input from those physicians.

Lactate has been measured to evaluate fetal metabolic acidosis for some decades. However, not until the past few years have reliable lactate meters become available for bedside obstetric practice. The new technology, which requires only 5 microl blood, has reduced the sampling failure rate of fetal scalp blood to almost nil. A growing body of evidence has also shown lactate to be good at predicting neonatal outcome.

These injuries are most commonly seen in unrestrained drivers or occupants involved in motor vehicle crashes in which the victim is partially or totally ejected. The characteristic MRI of scalp trichilemmal cysts is reported. The feature of an iso-intense area on T1-weighted images is unique to trichilemmal cysts. MRI is helpful in diagnosing the trichilemmal cyst and useful in distinguishing cystic tumors on the median scalp from a cranial malformation, meningo- or encephalocele.

Because of the increase of travels to tropical countries the physician in Germany is more often confronted with parasitic tropical diseases. In an apparently routine case a 28 year old patient suffered from infected stitches of the scalp, caused by an unknown insect, after returning from Sri Lanka. Four larvae of Dermatobia hominis, which causes the cutaneous myiasis, were found into the pyogenic furuncles and were removed by surgical approach.

A 63-year-old woman with a history of exaggerated use of topical skin care products of post-traumatic scarring alopecia presented with a monstrous cutaneous horn-like pomade crust of the scalp hidden beneath a wig. The crust consisted of corneocytes and bacterial debris. Pomade crusts have not been described previously in this localization. They are usually found in the inguinal or gluteal folds of infants, and occasionally in other body regions, e.g., on the face of adults.

A case of total scalp avulsion is described in surgical detail. The neurovascular bundles remained nearly untouched on the surface of the galea aponeurotica. Only one frontal branch of the left temporal artery was found to serve as a nutrient vessel for the whole scalp. Three veins around the scalp circumference supplied the venous drainage. The right-dorsal part of the scalp underwent necrosis, obviously because of internal disruption on the capillary level. This area had to be skin-grafted secondarily. Two severe cases of pyoderma (staphylococcus aureus) of the scalp, resistant to appropriate antibiotic treatment, proved to be due to exotic dermatophytes (Trichophyton soudanense and Trichophyton violaceum) in two North African children living in France. Both cases exemplify the growing number of Tinea capitis due to anthropophilic species imported from Africa by immigrated populations, their sometimes atypical clinical presentation and their familial epidemiology. Fungal cultures and identification procedures are essential for the diagnosis and epidemiological investigation of such cases.

A 30 years old maly complaining of loss of 1-2 cm long fragments of hair from his scalp, was discovered to have trichorrhexis nodosa involving approximately 40% of his scalp hairs. The defect was located 1-4 mm proximal to the distal ends of the hairs and was attributable to the topical application of calcium hydronide in an ayurvedic preparation.

The management of congenital scalp and skull defects, as generally advocated, is surgical. The authors report such a case that was treated conservatively. At her 3-year follow-up review, the patient's scalp and skull defects and other associated cutaneous defects were fully reconstituted. Such a nonoperative approach, while rarely reported, emphasizes the natural course that some of these lesions may follow. The literature on aplasia cutis congenita is briefly reviewed.
