Establishment of a stable three-dimensional fibrotic cell culture model for reproducible baseline and experimental applications.

Systemic sclerosis (SSc) is characterized by progressive fibrosis and complex alterations in extracellular matrix dynamics; however, experimental modeling of fibrotic processes remains limited by poor control of baseline signaling and spontaneous phenotypic drift in vitro. Such variability complicates the interpretation of perturbation-driven responses and reduces the reliability of antifibrotic and toxicological studies. In this study, we developed and characterized a three-dimensional (3D) fibrotic in vitro platform designed to maintain a stable and controlled baseline state across distinct

CAPI-Detect: machine learning in capillaroscopy reveals new variables influencing diagnosis.

OBJECTIVES: Nailfold videocapillaroscopy (NVC) is the gold standard for diagnosing SSc and differentiating primary from secondary RP. The CAPI-Score algorithm, designed for simplicity, classifies capillaroscopy scleroderma patterns (CSPs) using a limited number of capillary variables. This study aims to develop a more advanced machine learning (ML) model to improve CSP identification by integrating a broader range of statistical variables while minimizing examiner-related bias. METHODS: A total of 1780 capillaroscopies were randomly and blindly analysed by three to four trained observers. Con

Tenascin-C in fibrosis in multiple organs: Translational implications.

Systemic sclerosis (SSc, scleroderma) is a complex disease with a pathogenic triad of autoimmunity, vasculopathy, and fibrosis involving the skin and multiple internal organs [1]. Because fibrosis accounts for as much as 45% of all deaths worldwide and appears to be increasing in prevalence [2], understanding its pathogenesis and progression is an urgent scientific challenge. Fibroblasts and myofibroblasts are the key effector cells executing physiologic tissue repair on one hand, and pathological fibrogenesis leading to chronic fibrosing conditions on the other. Recent studies identify innate

Quantification of Ground Glass Opacities Can Be Useful to Describe Disease Activity in Systemic Sclerosis.

Interstitial lung disease (ILD) is the main cause of death in systemic sclerosis (SSc) patients. Usually, patients have lung involvement characterized by ground glass opacities (GGOs), but honeycombing (HC) is also possible. The Wells score is a semi-quantitative index, which is able to assess ILD by distinguishing its main components. The aim of this work is to evaluate the Wells score in relation to the disease activity (DA) index. We enrolled 40 consecutive SSc-ILD patients (26 diffuse cutaneous form, dcSSc, and 14 limited form, lcSSc). All patients were evaluated by the European Scleroderm

Rheumatology in Africa-challenges and opportunities.

Africa faces many health challenges despite sustained growth and development over the past decade. Contributory factors are the lack of financial resources, an inadequate health professional workforce, a high burden of communicable diseases, and an increasing burden of non-communicable diseases. Rheumatology services are limited or non-existent in many parts of sub-Saharan Africa. Over the past decade, partnerships with international academic institutions have resulted in some progress in the training of rheumatologists and health professionals and development of rheumatology services in count

Analysis of complement biomarkers in systemic sclerosis indicates a distinct pattern in scleroderma renal crisis.

BACKGROUND: The complement system has been implicated in pathogenesis of systemic sclerosis (SSc). The goal of the present study was to evaluate improved complement biomarkers in SSc. METHODS: The presence of C4d, reflecting activation of the classical/lectin pathways, C3bBbP corresponding to activation of the alternative pathway, and soluble terminal complement complexes (all complement pathways), was measured in plasma samples by enzyme-linked immunosorbent assay and correlated to clinical parameters. The study included 81 patients with limited cutaneous SSc and 41 with diffuse cutaneous SS

Low prevalence of coeliac disease in patients with systemic sclerosis: a cross-sectional study of a registry cohort.

OBJECTIVES: Two prior studies suggested that coeliac disease (CD) has a higher prevalence rate (8%) in SSc than in the general population (1%), but these studies were limited by small numbers and the use of traditional coeliac screening antibody tests, where newer ones with improved accuracy have since emerged. Our aim was to determine the prevalence of CD in a larger SSc population using a more modern serological approach to coeliac testing and to correlate coeliac antibody status with gastrointestinal symptoms. METHODS: Stored sera from 72 SSc patients in the Scleroderma Registry at the Hos

Clinical and serological hallmarks of systemic sclerosis overlap syndromes.

OBJECTIVE: To determine the prevalence of systemic sclerosis (SSc) overlap syndrome and autoantibody profile in a large single-center cohort. METHODS: SSc diagnoses, subsets, and autoantibody profiles were obtained from clinical records of patients attending the Centre for Rheumatology, Royal Free Hospital, between September 1999 and February 2007. RESULTS: In total, 332 (20%) of 1700 patients with SSc had overlap syndrome. This comprised myositis (42.8%), rheumatoid arthritis (RA; 32%), Sjögren's syndrome (SS; 16.8%), and systemic lupus erythematosus (SLE; 8.4%). Antinuclear antibody was po

[Autoantibodies in systemic sclerosis: clinical interest and diagnosis approach].

This review describes different autoantibodies that are associated to systemic sclerosis disease, in presenting their interest for the diagnosis and prognosis, and suggests an immunologic diagnosis approach. The systemic sclerosis (SSc) is characterized by variant specific autoantibodies (autoAbs). More than 90% of SSc cases have antinuclear antibodies (ANA). Anti-centromere and anti-Th/To antibodies are often associated to the limited SSc and to the CREST syndrome. The anti-topo-isomerase I, anti-RNA polymerases and anti-fibrillarin/U3-RNP Abs are diffuse SSc markers with several organ involv

GVHD-Associated Angiomatosis: An Overlooked Vascular Sequela of Chronic Graft-Versus-Host Disease.

Graft-versus-host disease-associated angiomatosis (GVHD-AA) is a rare clinicopathologic variant of chronic GVHD characterized by benign vascular proliferations arising within areas of sclerodermatous skin change. The entity was formally named in 2014 and presents clinically as red-to-violaceous papules or nodules, confined to fibrotic GVHD skin. We present a case of GVHD-AA in a 29-year-old man with Philadelphia chromosome-positive acute lymphoblastic leukemia, diagnosed in 2017 and treated with matched unrelated donor allogeneic peripheral blood stem cell transplantation. Periostin is induced by IL-4/IL-13 in dermal fibroblasts and promotes RhoA/ROCK pathway-mediated TGF-β1 secretion in abnormal scar formation.

Excess scar formation can occur after skin injurふy and lead to abnormal scar formation, such as keloids and hypertrophic scars, which are characterised by substantial deposition of extracellular matrix in the dermis. Periostin, an extracellular matrix protein that plays a crucial role in skin development and maintaining homeostasis, is also involved in skin disorders such as systemic/limited scleroderma, wound closure, and abnormal scar formation. However, the mechanism of periostin involvement in abnormal scar formation is not yet fully understood. In this study, we investigated the mechanism

IGFBP-5 Promotes Fibrosis via Increasing Its Own Expression and That of Other Pro-fibrotic Mediators.

Pulmonary fibrosis is a hallmark of diseases such as systemic sclerosis (SSc, scleroderma) and idiopathic pulmonary fibrosis (IPF). To date, the therapeutic options for patients with pulmonary fibrosis are limited, and organ transplantation remains the most effective option. Insulin-like growth factor-binding protein 5 (IGFBP-5) is a conserved member of the IGFBP family of proteins that is overexpressed in SSc and IPF. In this study, we demonstrate that both exogenous and adenovirally expressed IGFBP-5 promote fibrosis by increasing the production of extracellular matrix (ECM) genes and the ex

Pediatric morphea: MRI evaluation of superficial and deep tissue involvement and impact in clinical practice.

BACKGROUND: Pediatric morphea (also known as localized scleroderma) is an inflammatory disorder that can involve not only the skin but also deeper tissues such as fascia, muscle, and bone. Clinical examination often underestimates the depth and activity of disease, creating a need for objective imaging tools to guide management. Despite increasing clinical reliance on MRI, data describing MRI findings and clinical utility in pediatric morphea remains limited. OBJECTIVE: To characterize superficial and deep tissue abnormalities on MRI in patients with pediatric morphea, assess correlations wit
