Editorial Commentary: Unsolved Problems in Shoulder Instability-The Dilemma of Bone Loss!

Bone loss in the setting of shoulder instability is an unsolved problem. Procedures restoring bone stock on the glenoid side vary from minimally invasive arthroscopic techniques of arthroscopic bone block procedures to traditional transposition of the coracoid or bone transplant. These techniques are evolving, and several tips and tricks have been optimized to improve outcomes and to reduce the risk of complications, even though the most terrible complication of transposition of the coracoid remains osteoarthritis in the young adult. The major innovation in the past few years has been the use

Arthroscopic double-pulley remplissage technique for engaging Hill-Sachs lesions in anterior shoulder instability repairs.

We present a modified arthroscopic technique used to treat anterior shoulder instability associated with mild glenoid bone loss and a large Hill-Sachs lesion. The procedure aims to convert a bony intra-articular defect into an extra-articular defect by insetting the infraspinatus into the Hill-Sachs lesion. The arthroscopic procedure is performed with the patient in the lateral decubitus position, and the same portals used for anterior instability repair are used for this technique. The sequence of steps involves placing and passing the glenoid anchors and sutures and then waiting to tie the a

[Immobilization by external rotation after primary traumatic shoulder dislocation].

Immobilization of the shoulder following dislocation and reduction is traditionally carried out using internal rotation braces. However, high recurrence rates of up to 96% may result depending on several concomitant intrinsic and extrinsic prognostic factors, such as age and level of physical activity. Recently, different cadaveric and radiologic studies have suggested an improved tissue apposition of the affected labroligamentous structures in a better anatomic position during external rotation of the humeral head. Recent clinical outcome studies have supported the hypothetical benefit of thi

[Arthroscopic ventral shoulder stabilization].

Traumatic antero-inferior dislocation of the shoulder is a common injury among the young athletic population. The incidence of reluxation after conservative treatment in young patients is up to 80%. Arthroscopic antero-inferior shoulder stabilization has become the gold standard in recent years due to technical and material improvement. The recurrence rate after arthroscopic and open surgery is similar (approximately 6.5%), but morbidity after arthroscopic surgery is less. The authors of this paper advise the use of suture anchors for refixation of the capsulolabral complex. An antero-inferior

[Shoulder pain in inflammatory rheumatic diseases : Importance of musculoskeletal ultrasound].

Shoulder pain is a frequent reason for presentation in medical practices; however, it can sometimes be challenging to differentiate between an inflammatory versus mechanical or degenerative cause of shoulder pain. A swelling of the shoulder joint is only palpable in cases of pronounced clinical findings. In rheumatoid arthritis (RA) and psoriatic arthritis (PsA) the shoulder joints can be involved, which is early and frequent, particularly in the case of RA and is associated with a more severe course of the disease. Polymyalgia rheumatica (PMR) is typically manifested with shoulder and hip gir…

Editorial Commentary: Not Every Hill-Sachs Needs Filling: Most Do, But Selective, Cost-Conscious Remplissage Is Still Warranted.

Recent evidence continues to support the clinical advantages of adding remplissage to most Bankart repairs, even in many cases with on-track Hill-Sachs lesions. The study by Schriener et al. strengthens the argument for broader use, while also promoting reflection on the need for judicious, patient-specific, decision-making. The presence of other risk factors, such as lesion orientation, activity level, and capsular quality all influence outcomes, and the long-term implications of remplissage remain incompletely understood. As Voltaire reminded us, "the perfect is the enemy of the good"-and in

TRAUMATIC ANTERIOR SHOULDER INSTABILITY: A NARRATIVE REVIEW OF CURRENT LITERATURE.

BACKGROUND: Traumatic anterior shoulder instability (TUBS) is defined as one-directional shoulder instability that is often associated with Bankart lesions and glenoid bone loss. If not managed properly it can lead to recurrent shoulder dislocation. TUBS usually occurs as a result of traumatic dislocation. Early diagnosis and intervention are essential to achieve favorable outcomes and prevent long term complications. AIM: This narrative review offers a comprehensive overview of traumatic anterior shoulder instability, with an aim to cover all aspects including patient presentation, tools tha

Editorial Commentary: Anchor Position Affects Glenoid Resorption Rates After Arthroscopic Bankart Repair: Shoulder Stabilization Surgery Is a Game of Millimeters.

The technical nuances of arthroscopic Bankart repair cannot be overstated. Previous literature has identified a number of risk factors for failure of arthroscopic stabilization procedures, and the implications of glenoid bone loss is widely recognized as a critical driver of postoperative outcomes. However, other technical considerations (inadequate number of suture anchors, improper position of suture anchors) have been acknowledged as risk factors for the failure of arthroscopic stabilization procedures. More recently, concerns have been raised regarding the observed rates of glenoid bone re

Advances in the understanding of anterior instability of the shoulder.

Anterior instability involves a spectrum of disease ranging from the obvious acute first-time dislocation to the athlete presenting with shoulder pain and no history suggestive of instability. It is important to recognize the pathophysiology and how it relates to this spectrum of disease. The arthroscope has helped to identify the underlying pathology in both acute and chronic situations. Diagnostically, a history of a painful shoulder, especially in the athlete, should suggest anterior instability. Tests of translation, apprehension, and the use of local anesthetic can be useful. Arthroscopy

Hand, shoulder and back stiffness in long-term type 1 diabetes; cross-sectional association with skin collagen advanced glycation end-products. The Dialong study.

AIMS: We aimed to: (i) estimate the prevalence of Dupuytren's disease, trigger finger, carpal tunnel syndrome and frozen shoulder; (ii) assess stiffness of the hand, shoulder and back; and (iii) explore the association of joint stiffness with both long-term HbA1c and collagen advanced glycation end-products (AGEs) in long-term type 1 diabetes mellitus (T1DM). METHODS: Patients with T1DM from 1970 or earlier attending a specialized diabetes center were included in this cross-sectional controlled study. We collected HbA1/HbA1c measurements from 1980 to 2015 and data on hand and shoulder diagnos

A rare complication of tuberculous meningitis pediatric anterior glenohumeral instability.

Dislocation and instability of the shoulder joint are rare occurrences in childhood. Traumatic, infectious, congenital, and neuromuscular causes of pediatric recurrent shoulder dislocations are reported before. Central nervous system infection in infancy may be a reason for shoulder instability during childhood. This situation, which causes a disability for children, can be treated successfully with arthroscopic stabilization of the shoulder and postoperative effective rehabilitation protocols. Tuberculous meningitis may be a reason for neuromuscular shoulder instability. We describe a 12-year…

Arthroscopic posterior portal closure.

Posterior "working" portals in arthroscopic posterior stabilization could result in defects of the posterior capsule if not repaired. We describe a single portal technique used to close the posterior portal defect after arthroscopic stabilization. It is a safe and easy-to-perform technique, which could strengthen the structural integrity of the repair.

Anterior glenohumeral instability.

The understanding of and approach to anterior shoulder instability has changed and improved dramatically in recent years. It is now accepted that a shoulder can subluxate as well as dislocate and that chronic instability may or may not be caused by an initial traumatic event. An anteriorly unstable shoulder also can be unstable inferiorly and/or posteriorly (multi-directional instability). The author's diagnostic acumen has increased with the addition of glenohumeral axillary arthrotomography, glenohumeral CT arthrography, glenohumeral arthroscopy, and other studies. Surgical treatment has mov

Posterior shoulder instability.

Understanding the anatomic restraints to posterior shoulder instability and the resulting pathophysiology helps the treating physician make a correct diagnosis and formulate an appropriate treatment plan. A nonoperative program directed at reducing pain and increasing stability through comprehensive shoulder strengthening methods has generally been successful in treating recurrent posterior shoulder subluxation. Surgical options for treatment are reserved for those patients who fail to recognize improvement after six months of therapy and have no evidence of a psychological disturbance as the

Mechanism of injury and management in traumatic anterior shoulder dislocation with concomitant humeral shaft and ipsilateral scapula fracture: a case report and review of the literature.

INTRODUCTION: Traumatic anterior dislocation of the shoulder is an emergency and warrants urgent attention. However, it becomes difficult to manage in cases of associated fractures of humerus and other bones surrounding the shoulder joint. There have been reports of traumatic anterior dislocation of the shoulder associated with humeral fractures in the literature but the trilogy of anterior dislocation of the shoulder; humeral shaft fracture; and scapular fracture have never been described. CASE PRESENTATION: We present the case of a 27-year-old south Asian man presenting with the above-menti…

Neuromodulation of the suprascapular nerve.

The shoulder joint is an enarthrodial or ball-and-socket joint. A complex network of anatomic structures endows the human shoulder with tremendous mobility, greater than any other joint in the body. Many pathologies can been found in those patients with chronic shoulder pain. The painful limitation of shoulder motion affects hand and arm motion as well; therefore, it significantly influences work performance and everyday activities as well as the quality of life. Therefore, the treatment of patients with chronic shoulder pain has major social and health economic implications. In this article w…

Generation and characterization of a novel shoulder contracture mouse model.

Frozen shoulder is a relatively common disorder that leads to severe pain and stiffness in the shoulder joint. Although this disorder is self-limiting in nature, the symptoms often persist for years, resulting in severe disability. Recent studies using human specimens and animal models have shown distinct changes in the gene expression patterns in frozen shoulder tissue, indicating that novel therapeutic intervention could be achieved by controlling the genes that are potentially involved in the development of frozen shoulder. To achieve this goal, it is imperative to develop a reliable animal…
