This study examined the thesis that patients with coronary heart disease (CHD) comprise discrete personality subtypes and that identification of these subtypes may benefit biobehavioral research on CHD. Measures of Negative Affectivity, Social Inhibition, and Self-Deception were used to generate a personality taxonomy through cluster analysis in a sample of 405 men with CHD. This empirical taxonomy served as a basis for the development of a conceptual model which delineates hardy, distressed, inhibited, and repressive subtypes. We report here on a child who over a period of 8 years was admitted several times to hospitals in different states of Nigeria based on fictitious illnesses described by his mother. The child had various unnecessary, expensive and invasive investigations followed by treatment with harmful drugs. The evolution of this case of Munchausen syndrome by proxy is described in order to alert paediatricians in developing countries to a problem which is described frequently in more affluent societies. We believe this is the first such case to be recorded in West Africa.

Interviews with 48 Latino medical outpatients assessed depression via a brief self-report screening measure and a structured diagnostic interview. Twenty-eight percent of the sample received a DSM-III-R diagnosis of major depression. Validity information is provided for the screening measure, which can be used with Latino medical patients, a traditionally high-risk group for depressive disorders.

To determine the validity of the DSM-IIIR category organic mood disorder, we compared 50 psychiatric consultations with this diagnosis to 50 psychiatric consultations diagnosed with major depression in the medically ill. Organic mood disorder patients were more likely to be in the index affective disorder episode and have a negative family history of depression. For a small proportion of patients with respiratory disorders, the regular medical care provided by general practitioners and chest physicians does not result in control of respiratory symptoms. In the Dutch Asthma Center, Davos, Switzerland, two conditions not available in routine medical settings are instrumental in treating patients with severe respiratory illness. A review was conducted of the records of 147 patients above the age of 60 in a 350-bed general university hospital for whom a request for consultations was made over a two-year period by a geriatric psychiatry division in a department of psychiatry. In a prospective study of 283 myocardial infarction (MI) patients, state-dependent feelings of anxiety, depression, and irritability were assessed twice during hospital stay, and four times during a 3-5 yr follow-up. The authors analysed the psychiatric profile of 208 general hospital patients referred for psychiatric consultation, about half of these patients presented with parasuicidal behaviour. The diagnoses of depression and adjustment disorders predominated. Characteristics of the referred patients, source of referral and main forms of psychiatric intervention are documented. Means to promote good relationships between psychiatrists and physicians are suggested. The need for a better liaison psychiatric service is emphasised.

The performance of the General Health Questionnaire as a case detector was assessed in 75 patients receiving chemotherapy for early breast cancer. Seventy-five patients with an early RA (disease duration between 2 and 12 months; 79% female; average age 49 years; 72% with "definite" RA) were followed in a prospective study over a period of 2 years. The longitudinal study of a cohort of 591 men and women aged 20 and 21 years respectively at outset, and 23/24 years at a subsequent investigation, was analyzed for the manifestation of depressive syndromes. Patients who were to undergo surgical intervention for low back pain were investigated preoperatively. The effect of surgical intervention was determined after 6 months.
