We report here on a patient with recurrent sigmoid colon carcinoma. Postmortem examination revealed a fist-sized tumor in the retroperitoneum, invasive to the left ureter obstructing its lumen causing hydronephrosis of the ipsilateral kidney. Histological examination of the kidney showed multiple foci of adenocarcinoma cells on the pelvic surface. Invasion into the underlying tissue was not observed, and there was no tumor in the submucosal tissue of the pelvis or in the parenchyma of the left kidney. Cancer cells on the renal pelvic mucosa showed strong immunoreactivities for CEA and CA 19-9.

The authors analyzed the site distribution of 122 consecutive cases of colorectal carcinoma in which a barium enema examination was available for review. The carcinoma was detected initially with barium enema studies in 82 cases (67%), sigmoidoscopy in 18 (15%), colonoscopy in 12 (10%), digital rectal examination in seven (6%), and computed tomography in three (3%). We present a case in which a patient with suspected colorectal cancer, referred to the surgical outpatient clinic, was subsequently found to have a chicken wishbone apparently perforating the sigmoid colon. This demonstrates the complexities of diagnosis and management of an unusual presentation of ingested foreign body. This case is a useful learning point in consideration of differential diagnosis in the presentation of an apparently malignant lesion.

A 31-year-old man who died of metastatic adenocarcinoma of the sigmoid colon had undergone bilateral ureterosigmoidostomy for exstrophy of the bladder 20 years earlier. The patient never underwent a colonoscopy. Neoplastic transformation at the ureterosigmoid anastomosis must be considered as a possible severe complication, and therefore, periodic follow-up of these patients is mandatory.

Current choices of operation for left-sided large bowel emergencies have been established by a questionnaire sent to 218 consultant surgeons asking which operation they would perform under varying circumstances for obstructing sigmoid carcinoma and diverticular disease. Incidence of multiple primary malignancies is reportedly increasing globally. Limited cases of triple metachronous cancers are available in the literature. Here, we report a case of a female with an unusual combination of triple metachronous malignancy over a span of 15 years involving endometrium, nasal cavity and rectosigmoid that has not been reported before in the literature. A 67-year old woman underwent laparoscopy-assisted high anterior resection for sigmoid colon cancer. A histopathological examination revealed no lymph node metastasis, but extramural cancer deposits were present. Four years later, enhanced computed tomography revealed recurrence in the retroperitoneum and metastasis to the left kidney. A left nephrectomy and left hemicolectomy were performed. A year after the second operation, computed tomography revealed metastasis to the lung. Histopathological reexamination for extra-lymph nodal spread after the first operation revealed venous involvement.

Results of an occult-blood-test in 1 072 persons are reported. The test, recently registered as KRYPTO-HAEM, was developed by the Central Institute of Molecular Biology of the Academy of Sciences of the GDR. 27 patients (2.5%) had positive findings and by clinical examination 1 carcinoma of the sigmoid colon and 2 adenomas were detected. In 1 case (adenoma of the rectum) the test had a false-negative result. The findings were compared with own experiences with the HAEMOCCULT-Test. The KRYPTO-HAEM-Test is recommended for screening for colorectal tumors.
