Radial Forearm Free Flap with Parapharyngeal Pedicle Corridor for Reconstruction of Anterior Skull Base Defects.

BACKGROUND: The radial forearm free flap (RFFF) is an indispensable tool for the reconstruction of a range of defects, including those of the skull base. Several options for routing of the RFFF pedicle have been described, and the parapharyngeal corridor (PC) has been proposed as an option and employed for a nasopharyngeal defect. However, there are no reports of its use for the reconstruction of anterior skull base defects. The objective of this study is to describe the technique for free tissue reconstruction of anterior skull base defects using the RFFF and routing of the pedicle via the PC

Cavernous hemangiomas of the cranial vault in infants: a case-based update.

BACKGROUND: Mass lesions arising in the scalp or the cranial vault of children require a significant diagnostic work-up but their nature, in most instances, can only be clarified after histopathological study. Tumors of osseous consistency or merging from within cranial bones are seldom susceptible of fine needle aspiration biopsy and require excision. Ossified cephalhematoma is a rare entity that is usually related to perinatal or accidental trauma. Exceptionally, ossified cephalhematoma has a non-traumatic origin and arises from a different condition, as vascular tumor or malformation. DISC

Maffucci's syndrome with bilateral cartilaginous tumors of the cerebellopontine angle.

Maffucci's syndrome is characterized by the combination of multiple enchondromas (Ollier's disease) and hemangiomatosis. These hemangiomas develop in the subcutaneous tissue and form red-blue tumors dispersed over the whole body. Intracranial involvement is rare, making a preoperative radiological diagnosis and differentiation from other tumors rather difficult. The radiological characteristics and successful removal of the intracranial part of a chondrosarcoma of the cerebellopontine angle in a case of Maffucci's syndrome are reported in this paper.

The mutational landscape of skull base and spinal chordomas and the identification of potential prognostic and theranostic biomarkers.

OBJECTIVE: Chordomas are rare bone neoplasms characterized by a high recurrence rate and no benefit from any approved medical treatment to date. However, the investigation of molecular alterations in chordomas could be essential to prognosticate, guide clinical decision-making, and identify theranostic biomarkers. The aim of this study was to provide a detailed genomic landscape of a homogeneous series of 64 chordoma samples, revealing driver events, theranostic markers, and outcome-related genomic features. METHODS: The authors conducted whole-exome sequencing (WES), targeted next-generation

[The value of MRT in primary extramedullary plasmacytoma involvement of the head-neck area].

Six patients with a primary extramedullary plasmocytoma in the head and neck were examined using MR imaging. The morphological appearance of these lesions was described to support the radiologist in the differential diagnosis of this rare lesion. All tumours appeared as oval-shaped and sharply delineated without any signs of infiltration. After contrast media application a distinct central inhomogeneity could be proved in all cases. Furthermore, demoscopical, clinical and therapeutical problems were discussed based on the literature.

GAB1 as a Marker of Recurrence in Anterior Skull Base Meningioma.

BACKGROUND: About one-third of anterior skull base meningiomas show Hedgehog pathway activation. We have recently identified GAB1 as a surrogate marker for Hedgehog pathway-activated meningiomas. OBJECTIVE: To determine the reproducibility and prognostic value of GAB1 marker in anterior skull base meningiomas. METHODS: A retrospective bicentric cohort of anterior skull base meningiomas, operated from 2005 to 2015, was constituted. GAB1 immunohistochemistry was performed in 2 centers, and the GAB1 score was assessed. Clinical and pathological data were reviewed to determine the prognostic val

[Fat suppression in contrast-enhanced MRT of the base of the skull and of the head-neck area: its clinical value].

167 patients with abnormalities at the skull base and at the cervical-skull junction were examined by MRT in order to compare a FATSAT technique with T1- and T2-weighted SE sequences before and after intravenous injection of 0.1 mmol Gd-DTPA/kg KG. The diagnostic information from corresponding FATSAT and T1-SE sequences was correlated with the histopathological findings. In 10.7% of patients fat suppression was inadequate and in a further 11.3% of patients chemical shift artifacts limited the diagnostic value. The number of detectable lesions was not increased by the use of FATSAT sequences bu

Multiple cranial neuropathy as the initial presentation of metastatic prostate adenocarcinoma: case report and review of literature.

The skull base is an atypical metastatic site for prostate carcinoma. It is usually encountered late in the disease process in patients with known advanced disease. However, skull base involvement causing cranial nerve palsies may rarely be the presenting sign of prostate carcinoma. Such patients may present to a number of specialties including neurosurgery and can pose a diagnostic challenge in the absence of lower urinary tract symptoms. Here, we describe an unusual case of prostate adenocarcinoma presenting as a central skull base tumour with multiple cranial neuropathy.

Wegener granulomatosis of the eustachian tube and skull base mimicking a malignant tumor.

Wegener granulomatosis is a well-studied yet sometimes difficult-to-diagnose systemic vasculitis. Early treatment with immunosuppressants is the therapy of choice and usually controls the disease. Two cases of Wegener granulomatosis are presented, both with symptoms and imaging findings suggestive of a malignant tumor of the nasopharynx or skull base. The focus of upper respiratory involvement in each case was the eustachian tube.

Computed tomography in otoneurology.

Modern generation computed axial tomography is of great use in otoneurology with particular advantage coming from its excellent tissue density discrimination, together with its power of simultaneous investigation of both ears and their surrounding intracranial structures and the capability of spatial reformatting. The technique renders outmoded previous techniques including polytomography in most instances of congenital, inflammatory, neoplastic, and traumatic disease. Conventional lateral tomography may identify a small temporal bone fracture, and this examination is recommended in addition t

High-resolution CT of the normal and abnormal fallopian canal.

A two-part anatomic and clinical high-resolution computed tomographic (HRCT) study of the fallopian canal was conducted. From the correlation of HRCT images of eight specimen temporal bones with their corresponding anatomic sectional images, it was evident that the full length of the fallopian canal can be accurately visualized. An axial section demonstrates the labyrinthine segment, geniculate ganglion fossa, and proximal part of the tympanic segment, whereas a Stenver projection is used for the tympanic segment, second knee, and mastoid segment. In clinical studies axial sections simultaneou
