[Abnormal nocturnal behavior mimicking REM sleep behavior disorder episodes in a patient with periodic limb movement disorder].

Periodic limb movement disorder (PLMD) is a condition in which patients experience frequent periodic limb movements of sleep (PLMS). Synchronized arousal responses cause sleep fragmentation, resulting in insomnia, daytime sleepiness, and fatigue. A 59-year-old man was identified as having intense sleep-talking and body movements, suggesting rapid eye movement (REM) sleep behavior disorder (RBD). Attended video-polysomnography (PSG) revealed that sleep-talking and body movements occurred only during non-REM sleep and were associated with PLMS-induced arousals (periodic leg movement arousal inde

Parkinson's Disease: A Narrative Review on Potential Molecular Mechanisms of Sleep Disturbances, REM Behavior Disorder, and Melatonin.

Parkinson's disease (PD) is one of the most common neurodegenerative diseases. There is a wide range of sleep disturbances in patients with PD, such as insomnia and rapid eye movement (REM) sleep behavior disorder (or REM behavior disorder (RBD)). RBD is a sleep disorder in which a patient acts out his/her dreams and includes abnormal behaviors during the REM phase of sleep. On the other hand, melatonin is the principal hormone that is secreted by the pineal gland and significantly modulates the circadian clock and mood state. Furthermore, melatonin has a wide range of regulatory effects and i

Non-REM sleep-predominant reversible paradoxical breathing effort indicates dysregulation of diaphragm movements in multiple system atrophy.

STUDY OBJECTIVES: In multiple system atrophy, sleep-disordered breathing has a broad spectrum of phenotypes, among them inspiratory stridor and obstructive sleep apnea being most frequent. METHODS: We present a case of a 59-year-old woman with cerebellar-type multiple system atrophy, who showed transient paradoxical breathing effort during non-rapid eye movement sleep on diagnostic polysomnography. Because this thoraco-abdominal paradox was atypical for and did not coincide with upper airway obstruction, it most likely indicated central dysregulation of the diaphragm. RESULTS: Continuous pos

Cortical hyperarousal in NREM sleep normalizes from pre- to post- REM periods in individuals with frequent nightmares.

STUDY OBJECTIVES: Frequent nightmares have a high prevalence and constitute a risk factor for psychiatric conditions, but their pathophysiology is poorly understood. Our aim was to examine sleep architecture and electroencephalographic markers-with a specific focus on state transitions-related to sleep regulation and hyperarousal in participants with frequent nightmares (NM participants) versus healthy controls. METHODS: Healthy controls and NM participants spent two consecutive nights in the sleep laboratory. Second night spectral power during NREM to REM sleep (pre-REM) and REM to NREM (pos

GABA-glutamate supramammillary neurons control theta and gamma oscillations in the dentate gyrus during paradoxical (REM) sleep.

Several studies suggest that neurons from the lateral region of the SuM (SuML) innervating the dorsal dentate gyrus (DG) display a dual GABAergic and glutamatergic transmission and are specifically activated during paradoxical (REM) sleep (PS). The objective of the present study is to characterize the anatomical, neurochemical and electrophysiological properties of the SuML-DG projection neurons and to determine how they control DG oscillations and neuronal activation during PS and other vigilance states. For this purpose, we combine structural connectivity techniques using neurotropic viral v

The Sublaterodorsal Tegmental Nucleus Functions to Couple Brain State and Motor Activity during REM Sleep and Wakefulness.

Appropriate levels of muscle tone are needed to support waking behaviors such as sitting or standing. However, it is unclear how the brain functions to couple muscle tone with waking behaviors. Cataplexy is a unique experiment of nature in which muscle paralysis involuntarily intrudes into otherwise normal periods of wakefulness. Cataplexy therefore provides the opportunity to identify the circuit mechanisms that couple muscle tone and waking behaviors. Here, we tested the long-standing hypothesis that muscle paralysis during cataplexy is caused by recruitment of the brainstem circuit that ind

123I-MIBG cardiac scintigraphy provides clues to the underlying neurodegenerative disorder in idiopathic REM sleep behavior disorder.

OBJECTIVE: RBD is considered to be a manifestation of an evolving synucleinopathy, such as Parkinson disease (PD), dementia of Lewy bodies (DLB), and multiple system atrophy (MSA). We tested whether the degree of accumulation of cardiac 123I-MIBG scintigraphy can distinguish the clinical syndromes associated with Lewy body-related disease from the syndrome of PSP (a tauopathy) and MSA. DESIGN: Cross-sectional. SETTING: University-based sleep disorders laboratory. PATIENTS: Subjects comprised 95 patients (31, idiopathic RBD; 26, PD; 10, MSA; 6, DLB; 13, progressive supranuclear palsy [PSP])

Serum metabolic markers for α-synucleinopathies conversion in isolated REM sleep behavior disorder: a prospective cohort study.

BACKGROUND: Previous studies have suggested that metabolic alterations are involved in the pathogenesis of α-synucleinopathies. However, the metabolic changes in isolated REM sleep behavior disorder (iRBD), the prodromal stage of α-synucleinopathies, and their relationship to phenoconversion remain unexplored. OBJECTIVE: To investigate the serum metabolic markers in patients with iRBD and evaluate their association with the risk of conversion to α-synucleinopathies. METHODS: We enrolled 148 patients with video-polysomnography (vPSG)-confirmed iRBD and 142 age- and sex-matched healthy control

Periodic oscillatory slowing and aperiodic broadband shifts in isolated REM sleep behavior disorder are associated with phenoconversion.

STUDY OBJECTIVES: Conventional EEG slowing in isolated REM sleep behavior disorder (iRBD) may be confounded by disease-related shifts in the aperiodic background. We separated periodic and aperiodic EEG components to clarify oscillatory slowing and examine associations with phenoconversion and clinical measures. METHODS: Resting-state EEG was recorded from 134 patients with iRBD and 51 healthy controls (HC). During follow-up, 26 converted to Parkinson's disease or dementia with Lewy bodies. Power spectra (2-50 Hz) were parameterized using FOOOF to extract aperiodic offset, low-frequency slope

Refining the link between REM sleep behavior disorder and neurodegeneration: Genetic correlation, Mendelian randomization, and colocalization evidence.

Observational studies have proposed a link between isolated rapid eye movement sleep behavior disorder (iRBD) and several neurodegenerative diseases. We employed genome-wide linkage disequilibrium score regression (LDSC), standard two-sample Mendelian randomization (MR), and colocalization analysis to assess the causal links between iRBD and these neurodegenerative conditions. iRBD demonstrated a positive causal association with Alzheimer disease (odds ratio [OR] = 1.02, 95% confidence interval [CI]: 1.00-1.03, P = 1.10E-02), Parkinson disease (OR = 1.10, 95% CI: 1.03-1.16, P = 2.96E-03), and

Increased frontal [123I]FP-CIT binding in Parkinson's disease patients with self-reported REM sleep behavior disorder.

Rapid eye movement (REM) sleep behavior disorder (RBD) is a frequent non-motor symptom of Parkinson's disease (PD) and a potential early marker of synucleinopathy-related neurodegeneration. While striatal dopaminergic dysfunction in PD-RBD has been extensively studied, the role of extrastriatal monoaminergic alterations -particularly those involving serotonin - remains poorly understood. In this study, 155 PD patients underwent [123I]FP-CIT SPECT imaging to assess striatal and extrastriatal tracer binding, reflecting dopaminergic and broader monoaminergic function, respectively. Probable RBD w

Plasma neurofilament light chain as a predictor of multiple system atrophy in idiopathic REM sleep behavior disorder.

BACKGROUND: The predictive value of plasma biomarkers for phenoconversion in idiopathic REM sleep behavior disorder (iRBD) remains uncertain. OBJECTIVE: To evaluate the utility of plasma neurofilament light chain (NfL), glial fibrillary acidic protein (GFAP), and DOPA decarboxylase (DDC) levels for predicting phenoconversion in iRBD. METHODS: Fifty patients with polysomnography-confirmed iRBD and 38 matched healthy controls (HCs) were prospectively enrolled. Plasma NfL and GFAP were measured using SIMOA at baseline and after phenoconversion, and DDC levels via ELISA. Clinical assessments inc

Cerebellum and basal ganglia connectivity in isolated REM sleep behaviour disorder and Parkinson's disease: an exploratory study.

REM sleep behaviour disorder (RBD) is a parasomnia characterised by dream-enacting behaviour with loss of muscle atonia during REM sleep and is a prodromal feature of α-synucleinopathies like Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy. Although cortical-to-subcortical connectivity is well-studied in RBD, cerebellar and subcortical nuclei reciprocal connectivity is less established. Nonetheless, it could be relevant since RBD pathology involves brainstem structures with an ascending gradient. In this study, we utilised resting-state functional MRI to investigate

Reorganization of intrinsic functional connectivity in early-stage Parkinson's disease patients with probable REM sleep behavior disorder.

REM sleep behavior disorder (RBD) symptoms in Parkinson's disease (PD) suggest both a clinically and pathologically malignant subtype. However, whether RBD symptoms are associated with alterations in the organization of whole-brain intrinsic functional networks in PD, especially at early disease stages, remains unclear. Here we use resting-state functional MRI, coupled with graph-theoretical approaches and network-based statistics analyses, and validated with large-scale network analyses, to characterize functional brain networks and their relationship with clinical measures in early PD patien

Neurofilament light chain and cardiac MIBG uptake as predictors for phenoconversion in isolated REM sleep behavior disorder.

BACKGROUND: Isolated rapid-eye-movement (REM) sleep behavior disorder (iRBD) is considered as a prodromal stage of either multiple system atrophy (MSA) or Lewy body disease (LBD; Parkinson's disease and dementia with Lewy bodies). However, current knowledge is limited in predicting and differentiating the type of future phenoconversion in iRBD patients. We investigated the role of plasma neurofilament light chain (NfL) and cardiac metaiodobenzylguanidine (MIBG) uptake as predictors for phenoconversion. METHODS: Forty patients with iRBD were enrolled between April 2018 and October 2019 and pro
