Evaluation of Lumbar Spine Bracing as a Postoperative Adjunct to Single-level Posterior Lumbar Spine Surgery.

BACKGROUND: Clinical practice in postoperative bracing after posterior single-level lumbar spine fusion (PLF) is inconsistent between providers. This study seeks to assess the effect of bracing on short-term outcomes related to safety, quality of care, and direct costs. METHODS: Retrospective cohort analyses of consecutive patients undergoing single-level PLF with or without bracing at a three-hospital urban academic medical center (2013-2017) were undertaken (n = 906). Patient demographics and comorbidities were analyzed. Test of independence, Mann-Whitney-Wilcoxon test, and logistic regress…

Anatomical Variations That Can Lead to Spine Surgery at the Wrong Level: Part III Lumbosacral Spine.

Spine surgery at the wrong level is an undesirable event and unique pitfall in spine surgery. It is detrimental to the relationship between the patient and the surgeon and typically results in profound medical and legal consequences. It falls under the wrong-site surgery sentinel events reporting system. This error is most frequently observed in lumbosacral spine. Several risk factors are implicated; however, anatomical variations of the lumbosacral spine are a major risk factor. The aim of this article was to provide a detailed description of these high-risk anatomical variations, including t…

Observer agreement of spine stenosis on magnetic resonance imaging analysis of patients with cervical spine myelopathy.

OBJECTIVES: The purpose of this study was to measure the intraobserver and interobserver reliability of magnetic resonance detection of cervical spondylotic myelopathy with and without operational guidelines. METHODS: Seven radiologists examined images from 10 patients with cord signal abnormalities and clinical signs of myelopathy. Radiologist examined films twice, with and without operational guidelines designed to define stenotic changes, while blinded to the clinical findings of the patients. Analyses included a Fleiss kappa assessment of intraobserver and interobserver reliability. RESU

The current treatment--a survey of osteoporotic fracture treatment. Osteoporotic spine fractures: the spine surgeon's perspective.

For the spine surgeon the problems of the osteoporotic spine can be a painful simple compression fracture, the persisting instability after a fracture, the silent loss of posture due to progressive collapse of multiple vertebrae, and neurological complications related to an osteoporotic fracture. The use of polymethylmethacrylate (PMMA) for the reinforcement of osteoporotic vertebral bodies has turned out to be extremely efficient. Although the natural course of pain due to vertebral fractures decreases within the first weeks in the majority of patients, there remains a number of them with per…

Is the pelvis "organ protective" in pelvic and spine trauma? A database study of spine and pelvic combination injuries.

BACKGROUND: Traumatic spine fractures often occur with multisystem injuries. Pelvic and transverse process (TP) fractures have been linked to visceral trauma, but whether the pelvis behaves as an "organ protective" structure in spine injury is unclear. OBJECTIVE: To determine how concomitant pelvic fractures and TP fracture burden influence visceral injury patterns in patients with spinal fractures. METHODS: This retrospective cohort included adult trauma patients with CT-confirmed spinal fractures at a single Level I trauma center (2015-2023). Patients were grouped into spine-only or spine-…

Physical medicine and rehabilitation and interdisciplinary spine clinic referrals are associated with increased conversion to spine surgery.

BACKGROUND: Currently, in the United States, many patients receive immediate referral to a spine surgeon upon concern for any spine pathology, prior to undergoing conservative management. However, research has demonstrated that referrals to spine surgeons for common problems such as low back pain are often not indicated. This situation leads to inefficient care, delayed conservative treatments, and longer wait times for available surgical consultations. Therefore, this highlights an opportunity to improve efficiency in care for patients and clinicians through proper triaging for spine surgery.…

Congenitally Fused Cervical Spine Is Associated With Adjacent-Level Degeneration in the Absence of Cervical Spine Surgery.

BACKGROUND: Cervical fusion surgery is associated with adjacent-level degeneration, but surgical and technical factors are difficult to dissociate from the mechanical effects of the fusion itself. OBJECTIVE: To determine the effect of fusion on adjacent-level degeneration in unoperated patients using a cohort of patients with congenitally fused cervical vertebrae. METHODS: We identified 96 patients with incidental single-level cervical congenital fusion on computed tomography imaging. We compared these patients to an age-matched control cohort of 80 patients without congenital fusion. We qua

Multiple noncontiguous injuries of the spine in a child: atlantooccipital dislocation and seat-belt injury of the lumbar spine.

Multiple noncontiguous injuries of nonadjoining regions of the spine in children are rare. Such injuries can be overlooked as the neurological deficit caused by one injury can be masked by the other. The combination of atlantooccipital dislocation and a seat-belt injury of the spine in children is rare. We describe an unusual case of atlantooccipital dislocation in combination with a seat-belt injury of the lumbar spine in a surviving child. Early halo-vest immobilization was an effective treatment in this patient. Although survival with atlantooccipital injuries is becoming common, severe and…

Functional Outcome of Biportal Endoscopic Spine Surgery Versus Destandau Endoscopic Spine Surgery: A Structured Narrative Review.

Minimally invasive spine surgery (MISS) has revolutionized the management of degenerative spinal disorders by reducing surgical morbidity and accelerating recovery. Biportal endoscopic spine surgery (BESS) and Destandau endoscopic spine surgery (DESS) are two advanced minimally invasive techniques for managing lumbar degenerative conditions, including lumbar disc herniation (LDH) and spinal stenosis, which have gained prominence. BESS utilizes two separate portals for endoscopic visualization and instrumentation, providing a wider operative field and greater flexibility in addressing complex p…

Efficacy and Safety of Donut-Shaped Circumferential Spine CyberKnife Stereotactic Body Radiotherapy for Metastatic Spine Disease.

BACKGROUND AND OBJECTIVES: Spinal metastases (SM) with epidural spinal cord compression (ESCC) present a significant challenge because of the high risk of radiation-induced injury to critical structures such as the spinal cord and nerve roots. Traditional treatment approaches often avoid circumferential stereotactic body radiotherapy (SBRT) to reduce these risks. The efficacy and safety of donut-shaped circumferential SBRT, designed to target the spinal column while sparing the spinal cord, remains underexplored. The aim of this study was to evaluate the safety and efficacy of donut-shaped cir

Subfascial Lumbar Spine Drain Output Does Not Affect Outcomes After Incidental Durotomies in Elective Spine Surgery.

OBJECTIVE: Postoperative drains have long been regarded as a preventive measure to mitigate the risks of complications such as neurological impairment by reducing fluid accumulation following spine surgery. Our study aims to contribute to the existing body of knowledge by examining the effects of postoperative drain output on the 90-day postoperative outcomes for patients who experienced an incidental durotomy after lumbar decompression procedures, with or without fusion. METHODS: All patients aged ≥18 years with an incidental durotomy from spinal decompression with or without fusion surgery …
