INTRODUCTION: Atraumatic splenic rupture is a rare but life-threatening condition which may be associated with hematological malignancies.
PRESENTATION OF CASE: We present the case of a 63-year-old male patient with a history of chronic myelomonocytic leukemia and sarcoidosis under therapy with prednisone, who suffered an atraumatic splenic rupture with hemodynamic instability. He was managed with proximal splenic artery embolization and secondary open splenectomy. On pathology the diagnosis of peliosis lienalis was established.
DISCUSSION: Peliosis is a rare pathological entity, which prese…

Influenza virus infection may present with fever, chills, headache, myalgia, malaise, and respiratory symptoms, with a few cases developing into pneumonia, respiratory failure, and other organ damage. Very few cases of atraumatic splenic rupture associated with influenza infection have been reported. Atraumatic splenic rupture, while rare, is associated with high mortality. Here, we report the first case of atraumatic splenic rupture associated with influenza infection in the English literature and review the prior reported literature. The patient was diagnosed with influenza A (H1N1) pneumoni…

Splenic abscess is a rare life-threatening clinical entity. There are only a handful of reported cases of spontaneous splenic abscess rupture with pneumoperitoneum. Rupture of splenic abscess associated with gas-producing pathogens may lead to pneumoperitoneum. We hereby report the case of a ruptured splenic abscess with pneumoperitoneum in a young immunocompetent woman masquerading as hollow viscus perforation peritonitis. Ruptured splenic abscess should be kept in mind for treating surgeons as a differential diagnosis of pneumoperitoneum or peritonitis, particularly for immunocompromised pat

Colonoscopy is a common procedure that gastroenterologists perform on a daily basis. It is considered a low-risk outpatient procedure and patients can be discharged on the same day after the procedure. Colonoscopy has become more feasible with the increasing application of standard screening for colon cancer and diagnostic procedures for large intestinal disease. There are reported possible risk factors of splenic rupture during the procedure. However, splenic injury after colonoscopy is considered a rare complication and less than 100 cases have been reported in international literature. Inte…

BACKGROUND: Infectious mononucleosis is typically a self-limited disease commonly affecting young adults. Splenic rupture is a rare but serious complication affecting 0.1% to 0.5% of patients with mononucleosis. Current guidelines (based on published case reports) recommend complete activity restriction for 3 weeks after onset of mononucleosis symptoms to reduce rupture risk. We examined actual timing of mononucleosis-associated splenic injury using a large repository of unpublished patient data.
HYPOTHESIS: The risk of splenic injury after infectious mononucleosis will remain elevated longer…

In this paper we present a rare case of splenic rupture that occurred after an open aortic valve replacement in a male patient with hemangiomatosis of the spleen and the liver. The patient was treated with an emergency splenectomy. He showed no other sings of associated systemic disorder, such as Klippel-trénaunay syndrome or Proteus syndrome.

INTRODUCTION: Angiosarcoma of the spleen is a rare malignancy of vascular origin with a high rate of metastasis and poor prognosis. We report one such rare case of spontaneous splenic rupture, along with a review of current literature.
PRESENTATION OF CASE: A 30 year old man presented to our emergency services with severe abdominal pain, distension, hypotension and splenomegaly. Investigations revealed a marked anaemia, coagulopathy, severe lactic acidosis, and acute kidney injury. Imaging demonstrated splenomegaly with acute haemorrhage and lymphadenopathy. Laparotomy and splenectomy reveale…

Atraumatic splenic ruptures in the course of infectious diseases are rare but have been reported. Various germs of viruses can be at the origin of such rupture. The more often quoted viral disease is infectious mononucleosis. The more frequently involved bacteria are Streptococcus non pneumoniae, Pseudomonas, staphylococci and Clostridium. Rupture mechanism is not clearly elucidated; it can be connected with sepsis diffusion at spleen level via haematogenic way and consequently splenomegaly. Splenic rupture following septicaemia does not always entail major splenomegaly nor abscess formation b…

BACKGROUND: Splenic rupture because of metastasis from a distant organ is extremely rare.
CASE PRESENTATION: An 80-year-old man presented with left flank pain. A computed tomography (CT) demonstrated a poorly enhanced enlarged spleen with bulky thrombus in the splenic vein without extravasations. A CT on the following day showed increased intraperitoneal hemorrhage; therefore, an emergency laparotomy was performed. The spleen was enlarged and ruptured with lacerations on its surface. Macroscopic examination showed congestion with a thrombus in the splenic vein around the hilum. Pathology reve…

Operative management of blunt splenic trauma is rare in the paediatric population. Splenorrhaphy, with or without absorbable mesh, is a surgical procedure rarely used, although it preserves splenic function and has good outcomes. A 12-year-old girl accidentally fell from a height onto an iron bar. She complained of abdominal pain and difficulty breathing. Physical examination showed abdominal guarding and tenderness. Abdominal ultrasound demonstrated haemoperitoneum and splenic lacerations, confirmed by a CT scan, which highlighted active splenic bleeding. As she became haemodynamically unstab

Atraumatic splenic rupture is an uncommon but life-threatening complication of Epstein-Barr virus (EBV) infection. While infectious mononucleosis classically presents with fever, pharyngitis, and lymphadenopathy, we recorded a spontaneous splenic rupture even in the absence of these symptoms. We report a case of a 26-year-old male presenting to the emergency department with syncope. A progressive clinical hemodynamic deterioration was detected, ending hemorrhagic shock. Imaging revealed an atraumatic grade V splenic injury and massive hemoperitoneum. Laboratory tests showed lymphocytosis, prom…

An octogenarian male patient who had experienced 2 bleeding episodes in 1 year was hospitalized for melena. During the examination for the obscure gastrointestinal bleeding, a mixed-type intraductal papillary mucinous neoplasm (IPMN) was found, and an endoscopic ultrasound examination revealed a 4-mm splenic artery aneurysm protruding from the same lesion in the pancreatic tail. Distal pancreatectomy was performed, and the pathological diagnosis revealed ruptured blood vessels and blood components within the IPMN. The patient was diagnosed with hemosuccus pancreaticus due to a ruptured splenic

A 69-year-old male with a history of atrial fibrillation on apixaban with no recent history of trauma presented with acute onset abdominal pain and hypotension with no identified precipitating event. Intraabdominal free fluid was noted on bedside ultrasound, and computed tomography angiography of the abdomen showed splenic rupture. Anticoagulation was reversed with prothrombin complex, and he was taken to the operating room for emergent splenectomy. Spontaneous splenic rupture is uncommon, but as with traumatic splenic injuries, it may be managed operatively or non-operatively. In patients on …

BACKGROUND: Splenic rupture is an exceedingly rare but potentially life-threatening complication of colonoscopy, with an estimated incidence of less than 1 in 100,000 procedures and a reported mortality rate of up to 5%. Due to its non-specific and often subtle clinical presentation-typically involving left upper quadrant or generalized abdominal pain-it may be misdiagnosed or diagnosed late, which can result in a significant worsening of patient outcomes. Early recognition and prompt management are therefore essential not only for improving clinical prognosis but also for preventing potential…

This case demonstrates the importance of considering atraumatic splenic rupture (ASR) in acute abdominal pain, as its vague presentation often leads to failed diagnoses. Patients require prompt stabilisation and surgical intervention. Vaccinations and lifelong prophylactic antibiotics are recommended to prevent postsplenectomy …

INTRODUCTION AND IMPORTANCE: Isolated splenic peliosis is an extremely rare condition. The associations of splenic peliosis with various infections, medications, and conditions have unclear significance. We present three patients from the past twenty years with spontaneous splenic rupture due to peliosis, two of whom had hematologic malignancy, to draw attention to a possible correlation.
CASE PRESENTATION: A 31-year-old male with essential thrombocytopenia and antiphospholipid-antibody syndrome presented with worsening abdominal pain and hypotension. The patient denied any trauma. Computed-t…

Splenic abscess leading to rupture and pneumoperitoneum is a very rare condition that is limited to a few case reports. In this study, we first introduced a case of an adolescent girl with a medical history of diabetes mellitus who presented with left upper quadrant abdominal pain and fever. Ultrasound revealed an abscess in the spleen and a computed tomography (CT) scan of the abdomen revealed evidence of pneumoperitoneum secondary to the rupture of this abscess. The patient underwent splenectomy without postoperative complications. Secondly, we reviewed and discussed the current literature o
