[Diaphragmatic eventration in the adult: repair with Gore-Tex surgical mesh].

The Authors report the case of a fifty-nine-year-old patient, affected by complete eventration of the left diaphragm and cranial stomach dislocation with axial volvulus, associated with an upward movement of part of the left colon and some jejunal loops. The subclinical condition had been occasionally discovered during a routine chest X-ray at the age of eighteen and the patient remained without symptoms up to five years ago, when he began to show significant digestive and cardiorespiratory symptoms. The patient has been successfully treated via thoracotomy with phrenic alloplastic using Gore-

Symptoms and reflux competence in relation to anatomical findings at reoperation after laparoscopic total fundoplication.

OBJECTIVE: To investigate the mechanisms and anatomical failures after total laparoscopic fundoplication using the symptoms and findings at reoperation. DESIGN: Prospective open study. SETTING: University hospital, Sweden. PATIENTS: Twenty-one patients who were reoperated on a median of 33 (0.5-102) months after laparoscopic fundoplication. INTERVENTIONS: The patients were divided into three groups according to the mode of presentation. The first group presented with dysphagia and no gastro-oesophageal reflux (GOR) (n = 6). The second group (n = 11) had recurrent GOR and the third group (n

Breaking Boundaries: A Case Report of Giant Hiatal Hernia Housing Pancreas, Stomach, Small Bowel, and Colon.

Hiatal hernia (HH) is defined as the protrusion of abdominal contents through the esophageal hiatus of the diaphragm into the mediastinum. They are classified into categories I-IV. Type I, known as the sliding hernia, accounts for up to 90% of HH cases and are typically managed medically. Types II-IV combined account for less than 15% of HH cases. Types II-IV HH often require surgical repair, especially when symptomatic, due to the risk of complications such as gastric volvulus, obstruction, or strangulation. Furthermore, pancreatic herniation in a type IV HH has been shown to cause acute panc

Perforated gastric volvulus in a child with undiagnosed congenital diaphragmatic hernia (case report).

INTRODUCTION: Congenital diaphragmatic hernia (CDH) is a rare developmental anomaly where a defect in the diaphragm allows abdominal organs to migrate into the thoracic cavity, impairing lung function. While typically identified in neonates, delayed presentations, though uncommon, can complicate diagnosis and treatment. Early detection is vital to prevent severe complications such as organ strangulation or perforation. PRESENTATION OF CASE: We present the case of a 9-year-old boy with a history of fetal hypotrophy and unexplained poor weight gain, presenting with severe abdominal pain, vomiti

Twists and turns in the body: an imaging spectrum.

Life is full of twists and turns. These surprises can sometimes be wonderfully invigorating. Twists and turns can also occur in the body, however, sometimes with dangerous consequences. Torsion and volvulus are important causes of acute abdominal pain. The clinical symptoms and signs associated with torsion and volvulus are often non-specific and are difficult to diagnose clinically. Clinicians frequently rely on imaging methods to make the diagnosis. Prompt and accurate diagnosis is important to avoid the life-threatening complications of torsion and volvulus. Therefore, it is helpful to be f

Acute Gastric Volvulus Associated with Wandering Spleen and Diaphragmatic Eventration in a 5-month-old Girl.

The triad of gastric volvulus (GV), wandering spleen (WS), and diaphragmatic eventration (DE) is extremely uncommon and only two cases have been reported in the current pediatric literature. In this paper, we aim to describe the diagnostic and surgical management of a 5-month-old baby with this pathological triad and to present the other cases reported in pediatric literature. A 5-month-old baby was referred to our hospital because of vomiting and inconsolable crying from 12 h. The X-ray study demonstrated a severely distended stomach and a left diaphragmatic elevation. Swallow fluoroscopy rev

Congenital hemidiaphragmatic agenesis presenting as reversible mesenteroaxial gastric volvulus and diaphragmatic hernia: a case report.

A 70-yr-old woman complained of left sided chest pain and non-bilious vomiting for four days after taking a gastric bloating agent for an upper gastrointestinal study. The chest radiography revealed gastric air-fluid levels and bowel loops in the left thoracic cavity. An emergency thoracotomy was performed. The abdominal organs (stomach, spleen, splenic flexure of the colon) were in the left thorax and the entire left hemidiaphragm was absent. There were no diaphragmatic remnants visible for reconstruction of the left diaphragm. We provided warm saline irrigation and performed a left lower lob

Repair of a full-thickness gastric rupture in a horse.

A 14-year-old Thoroughbred broodmare was evaluated for signs of mild to moderate abdominal pain of 5 hours' duration. Exploratory celiotomy revealed a 20 x 8-cm subserosal hematoma associated with a focal serosal perforation overlying a muscular tear along the parietal surface of the stomach. The odor of intestinal tract gas was detected, but gross contamination of the abdomen was not evident. Concurrent partial volvulus of the small intestine was evident. Removal of the large colon from the abdomen appeared to coincide with propagation of the serosal perforation to a full-thickness gastric re

Ehlers-Danlos syndrome complicated by eventration of the diaphragm, colonic perforation and jejunal perforation--a case report.

A case of a 61 year old woman with Ehlers-Danlos syndrome who developed recurrent gastrointestinal complications is reported herein. She has been followed by Kyoundo Hospital since 1965, when she originally underwent treatment for eventration of the diaphragm and a volvulus of the stomach at the age of 41. During the past 22 years she has experienced perforation of the colon three times and jejunal perforation once. This case was considered to be type II Ehlers-Danlos syndrome. We have found 12 other cases of Ehlers-Danlos syndrome with gastrointestinal complications in the Japanese literature

Management of acute upside-down stomach.

BACKGROUND: Upside-down stomach (UDS) is characterized by herniation of the entire stomach or most gastric portions into the posterior mediastinum. Symptoms may vary heavily as they are related to reflux and mechanically impaired gastric emptying. UDS is associated with a risk of incarceration and volvulus development which both might be complicated by acute gastric outlet obstruction, advanced ischemia, gastric bleeding and perforation. CASE PRESENTATION: A 32-year-old male presented with acute intolerant epigastralgia and anterior chest pain associated with acute onset of nausea and vomitin

Pathogenesis, diagnosis, and management of gastric ischemia.

BACKGROUND & AIMS: Gastric ischemia is infrequently reported in the medical literature and under-recognized clinically and histopathologically. Various medical terms are used to describe gastric ischemia. We define and review the pathogenesis, diagnosis, and management of gastric ischemia. METHODS: We describe 6 cases of gastric ischemia. We discuss features of the gastric vascular supply and review literature on this disorder. RESULTS: Gastric ischemia results from diffuse or localized vascular insufficiency caused by etiologies such as systemic hypotension, vasculitis, or disseminated thro

Laparoscopic repair of parahiatal hernias with mesh: a retrospective study.

BACKGROUND: Parahiatal hernias are very rare and distinct entities, the diagnosis of which is never made clinically. Laparoscopic repair has been reported in the literature. We present our experiences with the laparoscopic repair of this uncommon type of hernia. PATIENTS AND METHODS: In our institute, we retrospectively identified a total of eight patients with parahiatal hernias from 1999 to 2007, of which four had primary and four had secondary defects. Laparoscopic crural repair was performed for all of the patients, fundoplication wherever indicated and meshplasty in the cases with large

Clinical evaluation of a modified circumcostal gastropexy in dogs.

A modified circumcostal gastropexy was performed in 5 Greyhounds. A 3-cm-long tunnel was created between the seromuscular and submuscosal layers of the pyloric antrum. A 6- to 7-cm-long segment of the cartilaginous portion of the 10th or 11th rib was dissected free of surrounding tissues and transected; the proximal portion of the rib was inserted through the seromuscular tunnel. The transected ends of the rib were overlapped approximately 1.5 cm and sutured in apposition in a side-by-side manner. Mean time for completion of the gastropexy was 11.5 minutes. Persistence of the surgically create

Gastric disease in the dog and cat.

The physiology of the normal gastric defence mechanisms in the dog and cat is reviewed to emphasize the routes by which drugs can be used to protect the gastric mucosa. The action of the main anti-ulcer and pro-kinetic drugs are discussed in relation to the diseases that they may be used to treat. Gastric disease in the form of gastric dilatation/volvulus, chronic vomiting without obstruction and gastric outflow disease are described from the point of view of diagnosis and treatment.

Canine gastric dilatation/volvulus syndrome in a veterinary critical care unit: 295 cases (1986-1992).

Two hundred ninety-five case records were included in an analysis of dogs treated by a standardized protocol for gastric dilatation/volvulus syndrome between 1986 and 1992. A breed predisposition was demonstrated for Great Danes, German Shepherd Dogs, large mixed-breed dogs, and Standard Poodles. One hundred and ninety-three dogs had gastric dilatation and volvulus (GDV) confirmed at surgery, 66 had simple gastric dilatation (GD), and 36 others had gastric dilatation but volvulus could not be proved or disproved (GD +/- V). Among dogs with GDV, the fatality rate was 15% (29/193). Twenty-six (1

Heterotaxy syndromes and abnormal bowel rotation.

BACKGROUND: Bowel rotation abnormalities in heterotaxy are common. As more children survive cardiac surgery, the management of gastrointestinal abnormalities has become controversial. OBJECTIVE: To evaluate imaging of malrotation in heterotaxy with surgical correlation and provide an algorithm for management. MATERIALS AND METHODS: Imaging reports of heterotaxic children with upper gastrointestinal (UGI) and/or small bowel follow-through (SBFT) were reviewed. Subsequently, fluoroscopic images were re-reviewed in conjunction with CT/MR studies. The original reports and re-reviewed images were

Percutaneous endoscopic gastrostomy in children and adolescents.

BACKGROUND: Long-term nasogastric tube feeding is often associated with irritation of the hypopharynx or dislocation of the tube. These pitfalls may be circumvented by percutaneous endoscopic gastrostomy. Although frequently used in adults, there is limited experience with the procedure in children. METHODS: A series of 139 patients (aged 3 weeks to 36.5 years, mean age, 4.4 years; weight 3.1-60 kg, mean weight, 15 kg) underwent placement of a percutaneous endoscopic gastrostomy because of central dysphagia (n = 103); general dystrophy caused by chronic renal failure, congenital heart disease
