Assessment of techniques used to evaluate the effect of posture and cardiac output on cerebral autoregulation.

In this study, the role of cardiac output in Cerebral Autoregulation was assessed. Mean arterial pressure, cerebral blood flow velocity (middle and anterior cerebral artery), stroke volume, cardiac output, total peripheral resistance and cerebral autoregulatory index were assessed in 3 healthy young males and 4 healthy young females during seated and upright thigh cuff testing. Two different methods (long axis left ventricular echocardiography and Modelflow) of evaluating stroke volume and cardiac output were compared. The results showed that cerebral autoregulation was less effective in the u…

Use of pulsed-wave Doppler echocardiography to determine aortic and pulmonary velocity and flow variables in clinically normal dogs.

Transcutaneous pulsed-wave Doppler echocardiography was used to obtain velocity signals from the aortic and pulmonary roots of clinically normal adult dogs tranquilized with acepromazine. Doppler-derived variables included peak ejection velocity, ejection time, and velocity-time integral. The cross-sectional areas of the left and right ventricular outflow tracts were estimated from diameters of the respective orifices measured from two-dimensional echocardiographic images. These data were used to calculate stroke volume and cardiac output for each ventricle. Linear, single variable regressions…

Mechanisms of decreased left ventricular preload during continuous positive pressure ventilation in ARDS.

Continuous positive pressure ventilation is associated with a reduction in left ventricular preload and cardiac output, but the mechanisms r … gated blood pool scintigraphy. Biventricular end-diastolic volumes were then calculated by dividing stroke volume by ejection fraction. As positive end-expiratory pressure increased from 0 to 20 cm H2O, stroke volume and biventricular end-diastolic volumes fell about 25 percent, and biventricular ejection fraction remained unchanged. At 20 cm H2O positive end-e …

Dimensional analysis of right and left ventricles during positive-pressure ventilation in dogs.

Our purpose was to determine the effects of controlled ventilation with positive end-expired pressure (PEEP) on ventricular dimensions and to relate changes in shape to changes in stroke volume and left ventricular volumes. Left and right ventricular dimensions were measured using biplane cinefluorography of dogs with radiopaque markers implanted in their hearts, and left ventricular volumes were derived from left ventricular dimensions by assuming that the left ventricle conformed to the shape of a nonprolate ellipsoid. As PEEP increased from 0 to 5, 10, and 15 cmH2O, stroke volume fell 36%, …

Stroke in mitral valve prolapse: risk factors and left atrial function in cryptogenic versus non-cryptogenic ischemic subtypes.

BACKGROUND AND PURPOSE: Mitral valve prolapse (MVP) has been associated with an increased risk of ischemic stroke. Older age, thicker mitral leaflets, and significant mitral regurgitation (MR) leading to atrial fibrillation have been traditionally considered risk factors for ischemic stroke in MVP. However, specific risk factors for MVP-stroke subtypes are not well defined. The aim of this study is to evaluate clinical and echocardiographic parameters, including left atrial (LA) function, in MVP with cryptogenic (C) vs. non-cryptogenic (NC) stroke. METHODS: In this case-control matched study,

Predictors of clinical outcome in patients with acute perforating artery infarction.

BACKGROUND AND PURPOSE: We aimed to determine the predictive factors for super-acute perforating artery infarctions (PAI) involving lenticulostriate arteries infarctions (LSAI), anterior pontine arteries infarctions (APAI) and thalamic arteries infarctions (TAI). Whether intravenous thrombolysis (iv-tPA) therapy would influence the clinical outcome was also studied. METHODS: We analyzed 84 consecutive patients within 12h of stroke symptom onset from January 2008 to June 2015. All patients had an imaging-proven acute infarction in the perforating territory of LSA (n=53), TA (n=21) and APA (n=1

[Doppler echocardiography quantification of the regurgitant blood volume in patients with mitral valve insufficiency].

The purpose of this study was to assess the accuracy and clinical utility of pulsed Doppler echocardiography in determining the regurgitant fraction in patients with pure mitral regurgitation. In 30 unselected consecutive patients with mitral regurgitation and in 20 patients without valvular heart disease pulsed Doppler echocardiography was performed to measure blood flow at the mitral and aortic valve. The regurgitant blood volume was calculated as the difference of the stroke volumes measured at the mitral and aortic valve. The regurgitant fraction was computed as regurgitant blood volume/mi…

Potential roles for transient receptor potential vanilloid 1 in heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous syndrome in which diastolic dysfunction develops against a background of interacting mechanisms: inflammation, oxidative stress, impaired calcium homeostasis, myocardial fibrosis, coronary microvascular dysfunction, mitochondrial impairment, and neurohumoral activation. Effective mechanism-targeted therapies remain limited. Transient receptor potential vanilloid 1 (TRPV1) is a nonselective cation channel expressed in cardiac sensory nerves, vascular endothelial cells, immune cells, and possibly other cardiovascular cell

Ambulatory Stage C2D Heart Failure Definitions and Current Therapeutic Approaches: JACC: Heart Failure Position Statement.

Symptomatic heart failure (HF) with reduced ejection fraction is traditionally drawn as 2 stages. Most stage C patients report manageable symptoms on guideline-directed medical therapy (GDMT), whereas stage D therapies are transplant or durable circulatory support, if eligible, and otherwise palliation with inotropic infusions or hospice considered for most. However, advanced HF includes not only end-stage D patients but also a large ambulatory population progressing beyond sustained response to GDMT who still seek and may find meaningful life-years ahead. This position statement was commissio

Treating obesity and heart failure: a how-to-manage guide from the European Association for the Study of Obesity and the European Society of Hypertension working group on diabetes and metabolic risk factors.

Obesity is a key contributor to heart failure, driving increased morbidity, mortality, and healthcare costs. This association is supported by multiple pathophysiological mechanisms and consistent epidemiological evidence, particularly not only in heart failure with preserved ejection fraction (HFpEF), but also in heart failure with reduced ejection fraction (HFrEF). Given the rising prevalence of both diseases, integrated management strategies are essential. This review outlines current management options in patients with heart failure and obesity, from lifestyle modification to pharmacologica

Finerenone in heart failure with ejection fraction ≥40%: state of the art and future perspectives.

Heart failure with ejection fraction ≥40% is associated with high mortality and risk of decompensation, as well as poorer quality of life. SGLT2 inhibitors are currently considered the standard therapy for these patients. However, despite treatment with these drugs, patients continue to have a high risk of adverse events. Overactivation of the mineralocorticoid receptor promotes the release of inflammatory and profibrotic factors, which contribute to the development and progression of this entity. However, not all mineralocorticoid receptor antagonists have demonstrated clinical benefit in the

Therapeutic left-to-right shunting in heart failure.

Heart failure with reduced or preserved ejection fraction is associated with elevated left atrial pressure at rest due to fluid overload or during exercise, leading to pulmonary venous congestion. Even with available treatments, heart failure hospitalizations remain high, and improvements in quality-of-life scores and functional capacity are modest. Thus, there is growing interest in non-pharmacological methods to decompress the left atrium and improve heart failure symptoms and outcomes. Left-to-right shunts have emerged as a potential therapeutic option to reduce left atrial hypertension, im
