Stuttering therapy with British-Asian children. I: A survey of service delivery in the United Kingdom.

The results of a UK survey of speech and language therapy services offered to Asian children and adolescents who stutter are set in the context of current therapy approaches favoured in the UK. Eighty-seven speech and language therapists from the major centres of Asian population in the UK completed a postal questionnaire. This yielded data on: service delivery to stuttering children in general; the size and nature of the Asian populations served; therapists' own cultural backgrounds; steps taken by therapists to increase their skill in this area; interpreter services available; changes made t…

Clarifying Neuroplasticity Claims in Parent-Led Early Stuttering Intervention: A Response to Onslow et al. (2026).

PURPOSE: Onslow et al. propose a simplified, parent-led early intervention for childhood stuttering based on reductions in parental speech rate and increased interturn pausing, framed within a neuroplasticity rationale for early treatment. While the goal of developing scalable and accessible interventions is important, the conceptual link between these conversational modifications and neuroplastic recovery warrants clarification. Neuroimaging studies in childhood stuttering have identified neural differences associated with persistence and recovery, but these findings establish correlates of d…

Stuttering priapism in a patient with sickle cell trait treated with automated red cell exchange transfusion.

Sickle cell trait (SCT) is typically an asymptomatic carrier state, but sickling complications can occur under extreme conditions. Priapism is known to be associated with sickle cell disease (SCD); The link with SCT is less well established. We report the case of a 19-year-old man with SCT presenting with prolonged priapism and a refractory, stuttering course requiring multiple invasive procedures over a 5-day period with no clear alternative triggers. In light of ongoing, stuttering priapism, he underwent red blood cell (RBC) exchange transfusion with decrease of hemoglobin S from 45.8% to 11…

Natural recovery from stuttering for a clinical cohort of pre-school children who received no treatment.

PURPOSE: The natural stuttering recovery rate by adulthood is high. Community cohort studies suggest a much lower rate during the first 18 months after onset, but this may be different for clinical cohorts of pre-school aged children. The present research and case presentations add to data reported by Franken et al. by investigating early natural recovery for a clinical cohort. METHOD: Participants were 16 pre-school children presenting to a clinic with stuttering onset 1-15 months earlier. The children were studied for a mean of 19.4 months (84.3 weeks) using parent report and clinician iden…

Relationship between serum homovanillic acid, DRD2 C957T (rs6277), and hDAT A559V (rs28364997) polymorphisms and developmental stuttering.

The involvement of the brain dopamine system in the pathophysiology of developmental stuttering has been previously suggested. In the present study, we aimed to investigate the relationship between developmental stuttering in children and the levels of serum homovanillic acid (HVA), dopamine D2 receptor (DRD2) C957T (rs6277), and solute carrier family 6 member 3 (SLC6A3) human dopamine transporter (hDAT) A559V (rs28364997) single-nucleotide polymorphisms. In a case-control study, serum level of HVA, DRD2 C957T, and DAT A559V were compared between 85 children who stuttered (CWS) and 85 age- and…

Wearable real-time and adaptive feedback device to face the stuttering: a knowledge-based telehealthcare proposal.

Despite first written references to permanent developmental stuttering occurred more than 2500 years ago, the mechanisms underlying this disorder are still unknown. This paper briefly reviews stuttering causal hypothesis and treatments, and presents the requirements that a new stuttering therapeutic device should verify. As a result of the analysis, an adaptive altered auditory feedback device based on a multimodal intelligent monitor, within the framework of a knowledge-based telehealthcare system, is presented. The subsequent discussion, based partly on the successful outcomes of a similar i…

[Stuttering: what does it tell us (body and speech)? What can be proposed (body and psyche)?].

The language can not be broken away from the oral: the actual psychoanalytic conceptions are called again. That allows to approach the stammering in its clinical theoretic aspects. One can consider the relation to the mother, the accession to the spoken word, the voice's wealth and possibilities, the necessity of the listening and of silence, with in all its aspects the emergence of the differences in stammerer subject or not. The links with the body and/or with the corporeal bring the notions on the psyche or psychic reality and permit to understand better the importance of a therapeutic acti

Executive functions in stuttering across the life span: A scoping review of behavioral and caregiver-reported measures.

Developmental stuttering is a multifactorial speech disorder that emerges during early childhood and can persist into adulthood, affecting communication, social participation, and quality of life. Recent research has increasingly focused on the role of executive functions (EFs)-including working memory, inhibitory control, and cognitive flexibility-in the onset and persistence of stuttering. This scoping review systematically maps the existing literature on EFs in monolingual individuals who stutter across the lifespan, synthesizing findings from 89 studies published between 1993 and 2024. Stu…

Speech Motor Deficits in Developmental Stuttering and Parkinson's Disease: From Cellular and Circuit to Network-Level Disruptions.

Speech dysfluency occurs across multiple motor disorders to varying degrees, yet the underlying mechanisms remain incompletely understood. Like locomotion, speech production is relies on basal ganglia circuits, and disruptions in these pathways can lead to varied symptomatic presentations. In this perspective, we discuss evidence from cellular, circuit and network-level studies to highlight shared and disorder-specific mechanisms of speech dysfluency in Parkinson's disease and developmental stuttering. Basal ganglia function and dopaminergic modulation are central to both disorders, while cort…

Efficacy of Noninvasive Brain Stimulation Techniques in Managing Stuttering Behaviors: A Systematic Review and Meta-Regression Analysis.

PURPOSE: This systematic review and meta-regression analysis investigated the overall effectiveness of noninvasive brain stimulation (NIBS) techniques in managing stuttering behaviors. METHOD: A total of 290 papers were initially identified through a comprehensive database search, and after applying inclusion and exclusion criteria, 15 studies were selected for the final analysis. These studies evaluated NIBS techniques both as standalone interventions and in combination with speech therapy techniques. The random-effects meta-analysis was done to investigate the effect of neuromodulation tech…

Case Report of Acquired Stuttering After Soccer-Related Concussion: Functional Magnetic Resonance Imaging as a Prognostic Tool.

BACKGROUND: Acquired stuttering has been reported resulting from various forms of traumatic brain injury. In stuttering patients, there has been an association with higher activity of right frontal operculum activity on functional magnetic resonance imaging (MRI). This report looked to identify any structural lesions in a patient with postconcussive stuttering after a head injury playing soccer. CASE DESCRIPTION: A 16-year-old girl presented with acquired stuttering after striking the back of her head during a soccer match. She did not lose consciousness. She complained of a mild headache, co…

A case of multiple system atrophy-parkinsonian type with stuttering- and palilalia-like dysfluencies and putaminal atrophy.

Both developmental and acquired stuttering are related to the function of the basal ganglia-thalamocortical loop, which includes the putamen. Here, we present a case of stuttering- and palilalia-like dysfluencies that manifested as an early symptom of multiple system atrophy-parkinsonian type (MSA-P) and bilateral atrophy of the putamen. The patient was a 72-year-old man with no history of developmental stuttering who presented with a stutter for consultation with our otorhinolaryngology department. The patient was diagnosed with MSA-P based on parkinsonism, autonomic dysfunction, and bilatera…

A phase II clinical trial of self-imposed time-out treatment for stuttering in adults and adolescents.

PURPOSE: This study reports the development and clinical trial of a new treatment program for persistent developmental stuttering in adolescents and adults. The treatment is based on the operant procedure of self-imposed time-out. This involves the person stopping speaking for a short period after each stutter. METHOD: Twenty-two participants completed Stage 1 (Instatement and Generalisation) of the program and 18 completed Stage 2 (Maintenance) of the program. Stuttering outcome was measured from independent audio and video recordings made outside and inside the clinic, before and after trea…

Attention Bias in School-Age Children Who Stutter: Evidence From a Dot-Probe Task.

PURPOSE: Cognitive models of anxiety attribute anxiety and ruminative thought patterns to selective processing of threat-related stimuli that automatically capture attention. We explored whether stuttering was associated with similar attentional biases by examining: (a) whether school-age children who stutter (CWS) differed from controls in selective processing of threat-related and neutral stimuli and (b) whether attentional biases in CWS were specific to threat stimuli that reflected stuttering-related experience. METHOD: Participants included 39 children (19 CWS), ages 8 to 15 years. Child…

Influences of Attentional Focus on Across- and Within-Sentence Variability in Adults Who Do and Do Not Stutter.

PURPOSE: Research has found an advantage to maintaining an external attentional focus while speaking as an increase in accuracy and a decrease in across-sentence variability has been found when producing oral-motor and speech tasks. What is not clear is how attention affects articulatory variability both across and within sentences, or how attention affects articulatory control in speakers who stutter. The purpose of this study was to investigate the effects of an internal versus external attention focus on articulatory variability at the sentence level. METHOD: This study used linear (spatia
