It was firmly established in the mid-twentieth century that the arachnoid villi represented an open pathway between the subarachnoid space and the dural venous sinuses. Intracellular and extracellular pathways within the villous structure provided the conduit for cerebrospinal fluid (CSF) and particulate matter. The importance of the lymphatic system was established by the demonstration of CSF tracers entering the nasal lymphatic system via the perineural subarachnoid space enveloping the olfactory nerve rootlets. It appears that because of the late development of the arachnoid villus (AV) sys…

Sixteen consecutive patients (8 men; 8 women; age range 6-63 years, median 40 years) with leptomeningeal metastasis were found by radionuclide ventriculography to have cerebrospinal fluid (CSF) flow interruption at the following sites: cisterna magna/basal cisterns (6); cervical (5) and thoracic (3) spinal subarachnoid space. Two additional patients had no evidence of interruption of CSF flow. All patients underwent intralumbar injection of 111Indium-DTPA and imaging of spinal ascent of radionuclide, corroborating sites of CSF flow disruption seen by radionuclide ventriculography. Median (rang…

The pathways of spread of malignant lymphoid cells into the central nervous system (CNS) were studied using a T lymphoblastic leukaemia/lymphoma model of inbred PVG rats. The effects of intraperitoneal, intracarotid, intravenous, intrathecal and intracerebral routes of transplantation were analysed, and the significance of the blood-brain barrier (BBB) in preventing neoplastic cell invasion was studied by disrupting the BBB with focal cold injury. Extraneurally transplanted cells appeared first in the dura and subarachnoid space. From the latter they spread further into the perivascular space …

OBJECTIVE: The goal in this study was to describe the overall organization of the spinal arachnoid mater and spinal subarachnoid space (SSAS) as well as its relationship with surrounding structures, in order to highlight spinal cord arachnoid cisterns.
METHODS: Fifteen spinal cords were extracted from embalmed adult cadavers. The organization of the spinal cord arachnoid and SSAS was described via macroscopic observations, optical microscopic views, and scanning electron microscope (SEM) studies. Gelatin injections were also performed to study separated dorsal subarachnoid compartments.

INTRODUCTION: Optic disc edema has been well documented in astronauts both during and after long-duration spaceflight and is hypothesized to largely result from increased pressure within the orbital subarachnoid space brought about by a generalized rise in intracranial pressure or from sequestration of cerebrospinal fluid within the orbital subarachnoid space with locally elevated optic nerve sheath pressure. In addition, a recent prospective study documented substantial spaceflight-associated peripapillary choroidal thickening, which may be a contributing factor in spaceflight-associated neur…

Neurofluids, a recently developed term that refers to interstitial fluids in the parenchyma and cerebrospinal fluid (CSF) in the ventricle and subarachnoid space, play a role in draining waste products from the brain. Neurofluids have been implicated in pathological conditions such as Alzheimer's disease and normal pressure hydrocephalus. Given that CSF moves faster in the CSF cavity than in the brain parenchyma, CSF motion can be detected by magnetic resonance imaging. CSF motion is synchronized to the heartbeat and respiratory cycle, but respiratory cycle-induced CSF motion has yet to be inv…

Cockayne syndrome (CS) and xeroderma pigmentosum (XP) are caused by deficient nucleotide excision repair. CS is characterized by cachectic d … autopsy cases of CS, XP group A (XP-A) and controls. Small arteries without arteriosclerosis in the subarachnoid space had increased in CS cases but not in either XP-A cases or controls. In addition, string vessels (twisted capillaries) in the cerebral white matter and increased density of CD34-immunoreactive vessels were observed in CS cases. Immunohistochemistry f …

Arachnoid cysts are a rare central nervous system malformation, representing only 1% of all intracranial masses in newborns. Primary (congenital) arachnoid cysts are benign accumulation of clear fluid between the dura and the brain substance throughout the cerebrospinal axis in relation to the arachnoid membrane and do not communicate with the subarachnoid space. Secondary (acquired) arachnoid cysts result from hemorrhage, trauma, and infection and usually communicate with the subarachnoid space. The common locations of arachnoid cysts are the surface of the brain at the level of main brain fi…

Human neurocysticercosis can cause mild or severe neurological symptoms or be completely asymptomatic. This heterogeneity may depend on host factors such as gender, age and immune-inflammatory response. The present study describes the specific peripheral immune response related to the different forms of neurocysticercosis in the adult population of both sexes. Asymptomatic cases (n = 26) mainly presented single calcified cysticerci in brain parenchyma or in the subarachnoid space of the sulci with a predominantly TH2 response (IL-4, IL-5, IL-13), high levels of IL-12 in supernatants of specifi…

Meningitis is accompanied by a differential immigration of leukocytes into the subarachnoid space. Since the mechanisms regulating leukocyte invasion are still incompletely understood, we studied the release of the neutrophil-attracting alpha-chemokines IL-8 and GRO-alpha and the mononuclear cell-attracting beta-chemokines MCP-1, MIP-1alpha, and RANTES during meningitis. In 48 paired CSF and serum samples from patients hospitalized for meningitic symptoms, high levels of IL-8, GRO-alpha, and MCP-1 were detected in the CSF during bacterial and abacterial meningitis. Elevated chemokine levels we…

Headache after unintentional dural puncture with 17- or 18-gauge needles represents a significant source of anesthetic-induced maternal morbidity. We performed this study to determine whether inserting a catheter into the subarachnoid space after dural puncture can significantly alter the incidence of headache. Thirty-five women, requesting labor analgesia, suffered a dural puncture during attempts to identify the epidural space using an 18-gauge Hustead needle (bevel oriented parallel to the longitudinal axis of the back). Subsequently, the anesthesiologist inserted a 20-gauge polyamide cathe…

1 In cats anaesthetized with chloralose, hyper- and hypotonic solutions were injected into the cisterna magna (in 0.5 ml) or into a lateral cerebral ventricle (in 0.2 to 0.3 ml), with aqueduct cannulated to prevent the injected solution from entering the subarachnoid space, and the effects on blood pressure and heart rate were examined. 2 Cisternal injections of hyper- and hypotonic solutions of NaCl (0.51 M and 0.05 M), glucose (1.03 M and 0.10 M), or sucrose (1.02 M and 0.10 M), as well as distilled water produced a rise in arterial blood pressure with tachycardias. Histological sections of the brain from Microtus montanus infected with Trypanosoma rhodesiense and treated after the initial infection period showed that trypanosomes promptly invade the brain tissue and induce a severe encephalitis within a very short time. These findings suggest that the brain invasion and encephalitis that occur after treatment in sleeping sickness must be considered an effect of the treatment. The results illustrate that the brain invasion is probably not hematogenic but rather due to a process of parasite migration from the subarachnoidal space over the Virchow-Robin spa

By applying the technique of transverse-section radionuclide scanning to cisternography, the structure and relationships of the basal cisterns and other subarachnoid spaces of the brain can be visualized more clearly and with more detail than is possible with routine imaging techniques. The ability of this method to separate overlapping areas of radioactivity ensures improved definition of space-occupying processes within the basal cisterns. In the evaluation by cisternography of patients with hydrocephalus and dementia, the transverse-section images clearly separated various normal and abnorm…

Leptomeningeal dissemination of low-grade spinal cord gliomas is an uncommon event. The authors report a unique case of leptomeningeal dissemination of a spinal cord pilocytic astrocytoma (PCA) to the intracranial cerebral subarachnoid spaces in a child. A 2-year-old boy presented with a loss of balance and the inability to walk or stand. An intradural intramedullary spinal cord tumor was identified, and the lesion was subtotally resected and diagnosed by the pathology department to be a PCA. Subsequently, the patient had recurrences of the intradural intramedullary tumor at 6 months and 2 yea…

A new mechanism of myelopathic changes in patients with mucopolysaccharidosis is described. Two similar cases had myelographic evidence of marked concentric impingement of the cervical subarachnoid space and cord compression due to thickening of the dura. Myelography is indicated in the evaluation of these patients because surgical decompression may reverse the progressive neurological deficits.

PURPOSE: To characterize MR findings in acute and chronic coccidioidomycosis meningitis and relate the imaging features to the clinical course.
METHODS: We reviewed MR scans and clinical findings of 12 patients with coccidioidal meningitis.
RESULTS: Patients with active or untreated disease were found to have hydrocephalus and intense enhancement of the cervical subarachnoid space, basilar, sylvian, and interhemispheric cisterns on postcontrast MR scans. Focal parenchymal signal abnormalities suggesting ischemia or infarction were common. Abnormal MR enhancement decreases during therapy, alt…

BACKGROUND: Several possible mechanisms exist for the spread of a primary tumor to the leptomeninges in leptomeningeal carcinomatosis. This report describes a case caused by direct bleeding in the subarachnoid space from a neoplastic cerebral aneurysm rupture.
CASE DESCRIPTION: A 48-year-old Japanese woman, who was diagnosed with breast carcinoma (pT3 pN2 M0) at the age of 45 years and underwent mastectomy and chemotherapy, was admitted in a coma following a sudden-onset severe headache. Computed tomography revealed diffuse hemorrhage in the subarachnoid space, and angiography revealed an ane…

The brain and spinal cord from 14 patients with primary central nervous system lymphomas (PCNSL) were systematically examined to determine the distribution and frequency of spinal cord lesions. Spinal cord involvement was observed in four cases (cases 1-4). In cases 1 and 2, tumor cells were spread widely in the brain and extended downward to C4 and Th8 in the spinal parenchyma and to C2 and Th2 in the subarachnoid space (SAS), respectively. In case 3, tumors were found in the cerebrum but not in the cerebellum or brainstem. In the spinal cord there was massive subarachnoid involvement from C2…

A 6-month-old male presented with seizures. Non-contrast CT revealed subependymal gray matter heterotopia, corpus callosum dysgenesis, colpocephaly, and suspicious closed-lip bilateral parieto-occipital schizencephaly. Benign posterior fossa and subarachnoid space enlargement were noted. This case highlights the diagnostic value of imaging in rare neuronal migration disorders and associated structural abnormalities.
