Local anesthetic sympathectomy restores fMRI cortical maps in CRPS I after upper extremity stellate blockade: a prospective case study.

BACKGROUND: Patients with complex regional pain syndrome type I (CRPS I) show a cortical reorganization with contralateral shrinkage of cortical maps in S1. The relevance of pain and disuse for the development and the maintenance of this shrinkage is unclear. OBJECTIVE: Aim of the study was to assess whether short-term pain relief induces changes in the cortical representation of the affected hand in patients with CRPS type I. STUDY DESIGN: Case series analysis of prospectively collected data. METHODS: We enrolled a case series of 5 consecutive patients with CRPS type I (disease duration 3

Sympathectomy reduces mechanical allodynia, tumor necrosis factor-alpha expression, and dorsal root ganglion apoptosis following nerve root crush injury.

STUDY DESIGN: An analysis of pain behavior and neuronal apoptosis in the dorsal root ganglion (DRG) following crush injury to the L5 nerve root, with or without surgical sympathectomy. OBJECTIVES: To determine whether sympathectomy prevents pain behavior and to compare pain behavior with expression of tumor necrosis factor (TNF)-alpha and apoptosis in the DRG. SUMMARY OF BACKGROUND DATA: Sympathetic block is used to relieve symptoms in radiculopathy patients. One effect of the block is improvement of blood flow to the nerve. However, this beneficial effect continues longer than the expected …

Post-ganglionic chemical sympathectomy of the cat nictitating membrane following systemic epinephrine: degeneration of nerve terminals followed by regeneration.

The present study was designed to provide further evidence that high doses of systemically administered epinephrine can result in a post-ganglionic chemical sympathectomy and to look for evidence of regeneration of nerve terminals. Three days following the systemic administration of a high dose of epinephrine a prolapse of the cat nictitating membrane was observed. Examination of the inferior smooth muscle associated with the membrane revealed ultrastructural evidence of nerve terminal destruction which was associated with a marked depletion of endogenous norepinephrine as measured with a cate…

Factors affecting long-term satisfaction after thoracic sympathectomy for palmar and plantar hyperhidrosis. Is the sudomotor reflex the only villain?

The main objective of this study was to determine if there are variations in the level of improvement of the palmar and plantar hyperhidrotic symptoms, as well as the incidence and intensity of the sudomotor reflex, throughout the seasons of the year, after thoracic sympathectomy for hyperhidrosis. The study also looks for the real impact of these variables in the long-term satisfaction. A cohort of 75 patients was followed through distinct seasons. A multivariate analysis was performed to identify possible variables responsible for dissatisfaction. Both the palmar (P=0.002) and plantar (P<0.0…

Effects of chemical and surgical sympathectomy on expression of beta-adrenergic receptors and guanine nucleotide-binding proteins in rat submandibular glands.

Expression of beta 1-adrenergic receptors and guanine nucleotide-binding proteins in rat submandibular glands was determined after reserpine administration and sympathetic denervation. Pretreatment of rats with reserpine resulted in up-regulation of the density of beta 1-adrenergic receptors and the immunoreactivity of the 64-kDa species of beta 1-adrenergic receptor in submandibular membranes, by 2.6 +/- 0.3-fold (eight experiments), within 7 days. Steady state levels of beta 1-adrenergic receptor mRNA quantified by DNA-excess solution hybridization were 0.15 +/- 0.03 amol of beta 1-adrenergi

Long-term Outcomes of Endoscopic Thoracoscopic Sympathectomy for Primary Focal Palmar Hyperhidrosis: High Patient Satisfaction Rates Despite Significant Compensatory Hyperhidrosis.

BACKGROUND: Endoscopic thoracoscopic sympathectomy (ETS) is the gold standard therapy for primary focal palmar hyperhidrosis (PFPH), resulting in high patient satisfaction rates. The most common side effect of ETS is compensatory hyperhidrosis (CH). Previous studies followed patients' satisfaction degree of surgery and the incidence of CH during a limited follow-up period of 1 to 3 years. The purpose of this study was to investigate the long-term outcomes and patient satisfaction after ETS. MATERIALS AND METHODS: After approval of our institutional review board, we conducted a retrospective r…

Effect of Vagotomy and Sympathectomy on the Feeding Responses Evoked by Intra-Aortic Cholecystokinin-8 in Adult Male Sprague Dawley Rats.

The vagus nerve and the celiaco-mesenteric ganglia (CMG) are required for reduction of meal size (MS) and prolongation of the intermeal interval (IMI) by intraperitoneal (ip) sulfated cholecystokinin-8 (CCK-8). However, recently we have shown that the gut regulates these responses. Therefore, reevaluating the role of the vagus and the CMG in the feeding responses evoked by CCK is necessary because the gut contains the highest concentration of enteric, vagal and splanchnic afferents and CCK-A receptors, which are required for reduction of food intake by this peptide, compared to other abdominal
