Microvascular blood flow fluctuations during tilt table induced syncope.

Aim of this study is to investigate the changes, in the frequency domain, of microvasculature blood flow (BF) occurring in neurally mediated syncope. We studied 20 patients with history of recurrent syncope and 10 control subjects. Peripheral BF was measured (laser Doppler technique) in the palmar region (PBF) and the manubrium sterni (SBF), together with ECG, respiration activity (RA) and blood pressure (BP), at rest (5 minute) and during 70 degrees head-up tilt test. SBF and PBF appear to be characterized by oscillations around 0.16 Hz (0.15-0.19 Hz), that do not have correspondence in the o…

[Recurrent syncope in a 34-year-old woman triathlete].

HISTORY: Some weeks previously a 34-year old athlete, specializing in the triathlon, had 6 syncopes in one day. They had caused abrasions and contusions resulting from the falls. At another hospital paroxysmal atrial fibrillation had been diagnosed and treatment with disopyramide (2 x 200 mg) initiated, but she had about 15 further syncopes within 2 weeks. She was admitted for establishing their cause. INVESTIGATIONS: Initial ECGs and neurological examination failed to provide a diagnosis and she was discharged with an "event recorder". DIAGNOSIS, TREATMENT AND COURSE: Three weeks after disc…

Case report: cough syncope in a U.S. Army aviator.

Syncope is a very common presentation to the primary care provider and the emergency room physician. The consequences of syncope while driving can be tragic. Likewise the consequences in the flight environment can be devastating. This case presents tussive syncope while driving in a 41-yr-old previously healthy male aviator. The diagnosis, management, and aeromedical disposition are presented.

First steps toward a pacing algorithm for vasovagal syncope.

Vasovagal syncope is commonly associated with an intermediate fall in heart rate. Consequently, it seemed possible that a pacemaker algorithm designed to detect intermediate heart rate falls might prove useful as a diagnostic tool to initiate pacing therapy. Subsequent experience suggests that with such an approach it is still difficult to avoid "false-positive" detection of physiologic heart rate slowing (e.g., sleep). On the other hand, findings from several reports suggest that high rate pacing using this algorithm has proved clinically effective.

Neurocardiogenic syncope and Prinzmetal's angina associated with bronchogenic carcinoma.

A clinical case is presented illustrating a previously unreported association of (1) neurocardiogenic syncope of new onset in a 57-year-old man, (2) Prinzmetal's angina, and (3) bronchogenic carcinoma of the lung. Initiation of aggressive chemotherapy resulted in immediate suppression of both cardiac manifestations. This newly described paraneoplastic syndrome is discussed.

Syncope as etiology of road crashes involving elderly drivers.

A significant number of elderly drivers present without discernible external cause for their road crash. We hypothesize that syncope may contribute to their crash etiology and prospectively evaluated drivers older than 60 years with unexplained crash, presenting to our trauma center during a 1-year period. Prehospital data and prior medical history were obtained on all, followed by a standard syncope work-up. Of 79 drivers ages 60-98, 58 (73%) were at fault in the crash. In 33 (57%), the crash etiology could not be determined. Thirteen had prior syncope history. Workup was negative in two pati…

Psychogenic syncope diagnosed by prolonged head-up tilt testing.

Four patients with 'syncope' during prolonged head-up tilt testing despite normal blood pressure and heart-rate behaviour are described. They represent 5.5% of all positive tilt-test outcomes from a prospective study of 181 consecutive patients with unexplained syncope. All four patients were female, and were both younger and more symptomatic than patients with vasovagal syncope or orthostatic hypotension. In three of the patients, clinical episodes of syncope were observed which closely resembled those provoked during tilt. It is suggested that the tilt response is psychogenic and reveals a p…

Possible role of coronary artery spasm in unexplained syncope.

Coronary spasm provocation by intracoronary methylergonovine was performed in 14 patients (8 men and 6 women, mean age 56 +/- 6 years) with syncope that remained unexplained despite neurologic and noninvasive cardiac evaluations. Electrophysiologic testing was also performed in 6 of 14 patients. No patient had structural heart disease or significant fixed stenosis of greater than or equal to 75% in the coronary arteries. Six patients had no history of chest pain even when they developed syncope. Serious arrhythmia was documented in 2 patients, cardiac standstill in 1 and complete atrioventricu…

Syncope and conduction disturbances following sublingual nifedipine for hypertension.

We present a case of a 79-year-old woman with periods of syncope, complete heart block (CHB), and ventricular standstill during periods of increased vagal tone following sublingual nifedipine for hypertension. The syncopal episodes were associated with periods of elevated vagal tone (micturition and vomiting) with one monitored episode showing a clear time course of emesis; CHB then ensued, progressing to ventricular standstill with loss of consciousness that resolved over several minutes. Although nifedipine is not thought to affect conduction at current clinical dosages, it seems likely that…

Trigeminal neuralgia associated with seizure and syncope. Case report.

A patient with trigeminal neuralgia experienced a generalized seizure and a prolonged syncopal episode. He was found to be asystolic during the syncopal episode. There was no recurrence of loss of consciousness after implantation of a pacemaker. Mechanical stimulation of the trigeminal nerve during craniotomy for microvascular decompression of the trigeminal nerve resulted in bradycardia. Since vascular decompression of the trigeminal nerve, there has been no recurrent facial pain, and no further syncope, seizures, or bradycardia. Syncope and seizures have not been previously reported in assoc…

Cough syncope: the possible relation to hydrocephalus ex vacuo.

Pneumoencephalographic data in five male patients with cough syncope were compared to those from age-matched patients with syncope from other causes. The 3rd ventricle width was significantly larger in patients with cough syncope than in the control group (p less than 0.01). Theoretically, the most plausible role of hydrocephalus ex vacuo in cough syncope may be: either it causes increased sensitivity to changes normally encountered during coughing, or--which seems less likely--it may abolish mechanisms normally restraining cough.

International study on syncope of uncertain aetiology 3 (ISSUE 3): pacemaker therapy for patients with asystolic neurally-mediated syncope: rationale and study design.

The deleterious effects of a disrupted copper metabolism are illustrated by hereditary diseases caused by mutations in the genes coding for the copper transporters ATP7A and ATP7B. Menkes disease, involving ATP7A, is a fatal neurodegenerative disorder of copper deficiency. Mutations in ATP7B lead to Wilson disease, which is characterized by a predominantly hepatic copper accumulation. The low incidence and the phenotypic variability of human copper toxicosis hamper identification of causal genes or modifier genes involved in the disease pathogenesis. The Labrador retriever was recently charact

Stretch-induced syncope: An examination of a rarely recognized condition.

Stretch-Induced Syncope is an uncommon form of situational syncope triggered by stretching upper-back, neck, and shoulder muscles. A review of 133 publications in PubMed, Embase, Scopus, Web of Science, Scielo, and Google Scholar, using search terms such as "stretch syncope," "syncope with stretch," and "stretch-induced syncope," identified 12 studies describing 30 patient cases (ages 5-26). Stretch-induced syncope episodes were triggered by neck hyperextension and upper-back stretching, producing brief loss of consciousness. Early reports attributed these events to vertebral artery compressio…

An Elusive Cause of Syncope: The Case of a Right Ventricular Myxoma.

Cardiac myxomas, the most common primary cardiac tumors, are benign tumors that arise from cardiac connective tissue. The most common site of origin is the left atrium, and occurrence at other sites is relatively rare. We present a case of a 19-year-old male, with a history of multiple episodes of syncope. The electrocardiogram showed right axis deviation, right ventricular hypertrophy with a strain pattern, and right bundle branch block. Echocardiography revealed multiple masses in the right atrium (RA) and right ventricle (RV) extending into the RV outflow tract (RVOT) and main pulmonary art…

Characterization of reflex syncope in Brugada syndrome: a literature review.

In Brugada syndrome (BrS), syncope is considered a sign of increased risk for sudden cardiac death (SCD) due to ventricular tachycardia/ventricular fibrillation (VT/VF) episodes. However, arrhythmic syncope in BrS is extremely rare, while nonarrhythmic syncope may occur as in the general active population, mostly from reflex events. Symptomatic patients with BrS show a higher risk profile, requiring a watchful risk stratification. In this scenario, a clinical misjudgment could determine to overlook the risk of SCD as well as to pursue inappropriate therapeutic approaches. Therefore, understand…

Functional Syncope in Children and Adolescents: A Retrospective Cohort Study.

BACKGROUND: Functional syncope, or psychogenic pseudosyncope, is often under-recognized. We aimed to show that functional syncope may be diagnosed in most pediatric patients by the initial neurological consultation. METHODS: We reviewed the medical records of patients who were evaluated from 2006 to 2022 in clinic for apparent transient loss of consciousness (a-TLOC) and probable functional syncope. Inclusion criteria included the following: (1) one or more episodes of a-TLOC; (2) spontaneous recovery; (3) age <19 years; (4) patients collapse or lie immobile andunresponsive to verbal stimulat…
