[The potential of high-resolution sonography in the follow-up of surgically treated flexor tendons].

Sonography of the hand is frequently used for diagnostics of hand diseases. Advantages are the lack of side effects (e. g., X-ray contamination) and easy accessibility. Soft tissues of the hand are easily examined using a linear-array transducer with a high frequency (7.5 to 15 MHz) and, in some cases, a pre-positioned device (water or gel). Also there is the possibility to record functional motility of ligaments or joints non-invasively. The beneficial use of sonography is demonstrated on patients with injured flexor tendons of the hand or fingers. Ultrasound was used to examine ligaments aft…

High-resolution US of non-traumatic recurrent dislocation of the peroneal tendons: a case report.

We report the case of a young girl with recurrent sharp pain in the outer aspect of the ankle. She had no previous trauma. High-resolution US (HRUS) showed a complete intermittent dislocation of the peroneal tendons. Post-traumatic chronic peroneal instability is quite common, whereas complete dislocation is rare. HRUS is an important adjunct to clinical examination and radiographic evaluation of patients with musculoskeletal disorders of the foot and ankle [1]. This paper reports its value in non-traumatic recurrent dislocation of the peroneal tendons.

[Injuries of the flexor and extensor tendons of the hand and forearm and their treatment].

The general principles of treatment of injuries of the flexor and extensor tendons in the hand and distal forearm are mentioned. For the extensor tendons the different types of treatment performed for injuries in the different areas, the finger, the back of the hand, and the wrist, are briefly discussed. Knowledge recently acquired from experimental and clinical studies, especially that of the anatomy, the blood supply, the nutrient pathways, and the healing of the flexor tendons, is discussed, and its applications in our daily clinical work are shown.

Biochemical variations in the matrix of human tendons in relation to age and pathological conditions.

With increasing age, the human tendon shows an increase of collagen and a diminution of mucopolysaccharides and glycoproteins. Similar findings have recently been reported by other authors in the tendons of mammals. Fragments of tendon affected by tendinosis, obtained dlring suture and tenoplasty operations, were also examined. Marked diminution of collagen and an increase of acid mucopolysaccharides and structural glycoproteins were found as compared with the control tendon.

Ultrasonographic Evaluation of the Distal Medial Hamstring Tendons and their Association with Posteromedial Knee Pain.

BACKGROUND: Periarticular abnormalities are common ultrasonographic (U/S) findings in individuals with knee pain. Incidental U/S observations, including thickening of the distal hamstring tendons, require explanations for their clinical importance. In addition, it is unclear whether or not these tendon modifications are related to knee pain. The objective is to determine U/S findings of distal medial hamstring tendons in patients with posteromedial (PM) knee pain and assess the diagnostic significance of tendon thickness in predicting tendinopathy in those patients. METHODS: We studied the di…

Tendon overuse models have different effects on tissue mechanical properties in ex vivo bovine tendons.

Tendinopathy is an overuse injury that occurs in high load bearing tendons, causing decreased function and increased pain. Current tendinopathy treatments produce inconsistent outcomes, creating a need for novel treatments. It is unclear whether current tendinopathy models used to develop new treatments replicate the mechanical effects of the injury. We aim to compare the mechanical properties of 2 established tendinopathy models - collagenase injection and mechanical overload - in ex vivo bovine tendons. Eighteen bovine tendons were split into 3 groups, including: uninjured, collagenase injec…

Comparison of Z-Lengthening to Fractional Lengthening of Forearm Flexor Tendons: A Biomechanical Cadaveric Analysis.

PURPOSE: Contractures of the extrinsic forearm flexor units can be surgically treated with various procedures that lengthen the musculotendinous unit. Z-lengthening performed at the tendon level and fractional lengthening (FL) performed at the musculotendinous junction are two common interventions to address contractures. The purpose of this study was to compare the biomechanical properties of flexor tendons undergoing Z-lengthening versus FL. METHODS: Seventy-two flexor tendons were harvested from 12 paired upper extremities, including flexor carpi radialis, flexor carpi ulnaris, flexor poll…

Diagnostic Musculoskeletal Ultrasound in the Evaluation of the Metacarpal Phalangeal and Proximal Phalangeal Extensor Tendons.

Injuries to the hand and fingers are among the most common injuries in orthopedics and general practice, as they are the part of the human body that interacts most frequently with our environment. Diagnostic musculoskeletal (MSK) ultrasound offers a portable, real-time, and cost-effective alternative that is very useful for obtaining information about small structures like the finger extensor tendons. MSK ultrasound is adept at detecting changes in both tendon tissue composition and integrity. This manuscript will review the utility of MSK ultrasound in evaluating finger extensor tendons and i…

Usage of Dermal Regeneration Templates (Pelnac) for Coverage of Exposed Hand Tendons in Acute Setting.

BACKGROUND: The loss of soft tissue coverage of tendons is a challenging reconstructive problem after acute hand trauma. Subsequent tendon adhesions and the loss of range of movement in addition to the poor aesthetic outcome and donor site scarring should be avoided when deciding the plan of management. Pelnac is one of the commonly used skin substitutes in reconstructive surgery that can be used for coverage of exposed tendons, but the postoperative functional outcome needs to be addressed in detail. METHODS: Twenty-six patients with acute isolated tendon injuries distal to the wrist joint w…

Whole Genome Expression Profiling of Semitendinosus Tendons from Children with Diplegic and Tetraplegic Cerebral Palsy.

Cerebral palsy (CP) is the most common movement disorder in children, with a prevalence ranging from 1.5 to 4 per 1000 live births. CP is caused by a non-progressive lesion of the developing brain, leading to progressive alterations of the musculoskeletal system, including spasticity, often leading to the development of fixed contractures, necessitating tendon lengthening surgery. Total RNA-sequencing analysis was performed on semitendinosus tendons from diplegic and tetraplegic CP patients subjected to tendon lengthening surgery compared to control patients undergoing anterior cruciate ligame…

Comparison of Cellular Responses to TGF-β1 and BMP-2 Between Healthy and Torn Tendons.

BACKGROUND: Tendons heal by fibrotic repair, increasing the likelihood of reinjury. Animal tendon injury and overuse models have identified transforming growth factor beta (TGF-β) and bone morphogenetic proteins (BMPs) as growth factors actively involved in the development of fibrosis, by mediating extracellular matrix synthesis and cell differentiation. PURPOSE: To understand how TGF-β and BMPs contribute to fibrotic processes using tendon-derived cells isolated from healthy and diseased human tendons. STUDY DESIGN: Controlled laboratory study. METHODS: Tendon-derived cells were isolated f…

The Passive Mechanical Properties of Muscles and Tendons in Children Affected by Osgood-Schlatter Disease.

BACKGROUND: Osgood-Schlatter disease (OSD) is a sports-related disorder involving apophysitis, which affects the tibial tuberosity. The identification of factors related to OSD is important for its prevention and early recovery from the disease. This study aimed to compare the passive mechanical properties of the muscle-tendon unit in children affected by an OSD and healthy children, by using ultrasound real-time tissue elastography. METHODS: Eighteen legs affected by OSD (OSD group) and 42 healthy legs (control: CON group) were assessed. The elasticity was obtained from the quadriceps muscle

Prevalence of the extensor digitorum, extensor digiti minimi and extensor indicis tendons and their variations.

We aimed to investigate tendon variations of the extensor digitorum (ED), extensor digiti minimi (EDM), and extensor indicis proprius (EIP) muscles. Our study was performed on 43 fetal cadavers (86 extremities), aged between 17 and 40 weeks of gestation. The number of ED tendons varied from three to six, proximal to the extensor retinaculum (ER), and from three to eight, distal to the ER. The ED most often had four tendons, both proximally and distally from the ER. The ED tendons of the fourth finger were observed to be most frequently duplicated. The most common juncturae tendinum (JT) was ty…
