ACR/EULAR Definitions of Remission Are Associated with Lower Residual Inflammatory Activity Compared with DAS28 Remission on Hand MRI in Rheumatoid Arthritis.

OBJECTIVE: To determine the level of residual inflammation [synovitis, bone marrow edema (BME), tenosynovitis, and total inflammation] quantified by hand magnetic resonance imaging (h-MRI) in patients with rheumatoid arthritis (RA) in remission according to 3 different definitions of clinical remission, and to compare these remission definitions. METHODS: A cross-sectional study. To assess the level of residual MRI inflammation in remission, cutoff levels associated to remission and median scores of MRI residual inflammatory lesions were calculated. Data from an MRI register of patients with …

De Quervain's disease.

This is a tenosynovitis of the abductor pollicis longus and extensor pollicis brevis tendons contained within the osseofibrous tunnel of the distal radius. Multiple tendons for the abductor pollicis longus and a separate, severely narrowed sheath for the extensor pollicis brevis make nonsurgical treatment difficult at times. While avoiding damage to nerve branches, surgical treatment is aimed at release of both tendon sheaths.

Reconstruction of the Extensor Carpi Ulnaris Subsheath Using a Fascia Lata Allograft.

Extensor carpi ulnaris subsheath injuries range from instability, painful snapping, to chronic tenosynovitis. While acute cases often respond to immobilization, surgical intervention is required for refractory cases. This study introduces a novel reconstruction technique using a fascia lata allograft to create an anatomical, radial-based sling under the extensor reticulum. This approach avoids autograft harvesting, preserves the original tendon gliding pathway, is applicable to all types of injury, and enables simultaneous treatment of intrinsic extensor carpi ulnaris pathologies and ulnocarpa…

Tendon Sheath, Bursae, and Clinically Relevant Peritendinous Structures of the Hand and Wrist.

Imaging is essential for evaluating tenosynovitis, bursitis, and pulley lesions. Both ultrasound (US) and MR imaging detect fluid accumulation and synovial thickening, key for diagnosing tenosynovitis and bursitis. They also reveal A1 pulley thickening in trigger fingers and A2-A4 pulley strain or tears in closed flexor pulley injuries (climber's fingers). US enables dynamic assessments in real-time and detects hypervascularity, while MR imaging provides detailed insights into the extent of inflammation in the soft tissues and bone through fluid-sensitive sequences and contrast enhancement. Th…

Wrist extensor retinaculum plasty in tendinous attritional syndrome: A surgical technique.

BACKGROUND: Isolated proliferative extensor tenosynovitis of the fourth dorsal compartment in non-rheumatoid patients is rare and may lead to tendon degeneration, intratendinous tearing, and functional impairment. Surgical management remains controversial, particularly regarding preservation of extensor retinaculum biomechanics. This study evaluates the clinical outcomes of the Kapandji extensor retinaculum plasty for treatment of advanced fourth-compartment extensor tenosynovitis. METHODS: A retrospective case series was conducted including five patients (three men, two women; mean age, 71.2…

An alternative splint design for trigger finger.

Conventional resting splints used to treat digital stenosing tenosynovitis (trigger finger) are often discontinued or not worn consistently by patients. Informal reports by patients indicate that the splints are too bulky, interfere with activities of daily living, and are visibly too noticeable. Since resolution of the condition may take as long as 9 weeks, this is a significant issue. A new splint design has been developed to avoid these shortcomings in hopes that physicians and therapists will have successful compliance when utilizing splinting in the treatment of this condition. It is both…

Recurrent hypertrophic peroneal tubercle associated with peroneus brevis tendon tear.

Stenosing peroneal tenosynovitis resulting from hypertrophy of the peroneal tubercle has been well described. Successful surgical treatment addresses the hypertrophied peroneal tubercle as well as any intrinsic tendon pathology. We report a case of recurrent foot pain caused by stenosing peroneal tenosynovitis in a 16-year-old woman. Four months after excision of a hypertrophic peroneal tubercle, the patient developed a recurrence of symptoms. Imaging studies, repeat operative exploration, and pathologic specimen demonstrated a recurrence of the peroneal tubercle hypertrophy associated with a …
