End-Stage Ankle Osteoarthritis: Exploring the Precedent Causes and Time Course of Joint Degeneration.

INTRODUCTION: End-stage ankle osteoarthritis (OA) is primarily post-traumatic, following injuries such as fractures, dislocations, and sprains. However, the timeline between initial injury and end-stage OA requiring surgical intervention remains poorly understood. This retrospective study aimed to identify injury patterns leading to post-traumatic osteoarthritis (PTOA) and determine the time from injury to definitive surgical treatment with ankle fusion (AF) or total ankle replacement (TAR). METHODS: A retrospective cohort study was conducted using medical records of patients who underwent AF

Characterizing conventional ankle MRI findings of nerve and muscle changes in diabetic patients: a case-control study.

BACKGROUND: Foot and ankle amputation is a feared complication of diabetic neuropathy and diabetes mellitus (DM) accounts for 80% of all in-hospital amputations. Magnetic resonance neurography is an effective tool in characterizing neuromuscular sequelae of the disease. However, conventional ankle MRI is more commonly performed and has not been studied to assess neuromuscular changes of DM. OBJECTIVE: The objective is to characterize neuromuscular changes of diabetic patients in a case-control study using conventional ankle MRI. METHODS: Between November 2019 and July 2021, 110 consecutive a

[A case of chronic progressive motor-dominant multiple mononeuritis associated with primary Sjögren's syndrome].

A 45-year-old female with a history of dry eyes presented with chronic progressive disturbance of her right finger extension, bilateral foot drops, and dysesthesia in the left lower leg. On admission, neurological examination revealed decreased tendon reflex in the right upper limb and bilateral lower limbs, and dysesthesia in the distal outer portions of the bilateral lower legs. Her vibration sensation was moderately diminished in both lower legs. Weakness ranging from moderate to severe was predominantly found in the muscles innervated by the radial (r > l), medial (r > l), ulnar (r > l), d

[Lumbosacral radiculoplexus neuropathy with a small intramedullary lesion in posterior spinal cord: a case report].

We present the unique case of a patient with lumbosacral radiculoplexus neuropathy (LRPN) and a small intramedullary lesion, probably an infarction, in the spinal cord. A previously healthy 58-year-old female developed acute severe pain involving the entire right lower limb, followed by weakness of the right lower limb a few hours later. The patient could not walk and was referred to our clinic. The patient reported no sphincter symptoms. Examination of the cranial nerves and upper limbs revealed no abnormalities. The strength in her right lower limb was reduced to Medical Research Council gra

[A patient of Charcot-Marie-tooth disease with rigid spine and respiratory failure].

This report describes a case of a 17-year-old girl with Charcot-Marie-Tooth disease (CMT) representing rigid spine and respiratory failure. At age 11, she tended to walk on her toes and had difficulty in getting up from the floor without support. She became aware of flexion limitation of the neck at the age of 12. At 15 years of age, She began to have dyspnea on effort. When she was 17 years old, neurological examination revealed mild weakness of the upper extremities and severe weakness of the distal lower extremities, generalized wasting and areflexia. Superficial sensation was mildly impair

Activation of α1 adrenoceptors in ventrolateral orbital cortex attenuates allodynia induced by spared nerve injury in rats.

Recent studies have demonstrated that noradrenaline acting in the ventrolateral orbital cortex (VLO) can potentially reduce allodynia induced by spared nerve injury (SNI), and this effect is mediated by α2 adrenoceptor. The present study examined the effect of the α1 adrenoceptors in the VLO on allodynia induced by SNI in the rats. The mechanical paw withdrawal threshold (PWT) was measured using von-Frey filaments. Microinjection of selective α1 adrenoceptor agonist methoxamine (20, 50, 100 μg in 0.5 μl) into the VLO, contralateral to the site of nerve injury, increased PWT in a dose-dependent

Autonomic and peripheral nervous system function in acute tick-borne encephalitis.

OBJECTIVES: Tick-borne encephalitis (TBE) is an emerging flaviviral zoonosis in Central and Eastern Europe. TBE can present as meningitis, meningoencephalitis, or meningoencephalomyelitis. Dysfunction of the autonomic (ANS) and peripheral motoric and sensory nervous system (PNS) might contribute to acute and long-term complications. We aimed to examine, whether the ANS and PNS are affected in acute TBE. METHODS: Fourteen patients with acute TBE, 17 with diabetic polyneuropathy (d-PNP), and 30 healthy controls (HC) were examined in our single-center, prospective study. ANS and PNS function was

The functional and morphological characteristics of sciatic nerve degeneration and regeneration after crush injury in rats.

BACKGROUND: Peripheral nerve damage induces a sequence of degeneration and regeneration events with a specific time course that leads to (partial) functional recovery. Quantitative electrophysiological analysis of degeneration and recovery over time is essential to understand the process. NEW METHOD: The presented ex vivo neurophysiological method evaluates functional recovery of the propagation of the compound action potential after crush injury of the rat sciatic nerve. A 32 channel electrode array was used to monitor compound action potential propagation at time points between 1h and 35 da

Modifications of the pirogoff amputation technique in adults: A retrospective analysis of 123 cases.

BACKGROUND: The Pirogoff amputation (1854) was initially developed to provide full-weight-bearing stumps and therefore allow a short ambulation without prosthesis. Modifications of the original technique including Boyd (1939) and the "Modified Pirogoff" were developed, which further reduced complications and improved the outcome. However, the current evidence regarding the techniques is scarce. The functional outcome, survivorship and complication rates are unknown. It was the purpose of this study to expand the knowledge with a retrospective case series and ultimately summarize and analyze th

Muscle cramps and neuropathies in patients with allogeneic hematopoietic stem cell transplantation and graft-versus-host disease.

OBJECTIVE: Graft-versus-host disease (GVHD) is an immune-mediated multisystemic disorder and the leading cause of morbidity after allogeneic hematopoietic stem cell transplantation. Peripheral nervous system manifestations of GVHD are rare but often disabling. Whereas immune-mediated neuropathies are an established feature of GVHD, muscle cramps are not well characterized. METHODS: In a single-centre retrospective cohort we studied 27 patients (age 23 to 69 years) with GVHD (acute n = 6, chronic n = 21) who complained of symptoms suggestive of peripheral nervous system complications. Clinical

Sensory conduction in peroneal and posterior tibial nerves using averaging techniques.

A method of obtaining pure sensory nerve conduction velocities in the lower extremities is described. This involves the use of electronic summation (signal averaging). Potentials were obtained and velocities calculated from all normal subjects examined. In patients with peripheral neuropathies it was often possible to obtain nerve velocities with signal amplitudes as low as 0·1 μV and these were often slower than those obtained from the normal subjects. The advantages and disadvantages of this method are discussed. It is of significant clinical value in that pure sensory nerve conduction veloc

Pitfalls in the electrodiagnostic studies of sacral plexopathies.

This retrospective review characterizes the electrodiagnostic (EDX) features and etiologies of sacral plexopathies (SPs) and discusses difficulties in their identification. The EDX findings of 171 clinically suspected SPs were reviewed using the following criteria: reduced/absent sensory nerve action potentials (SNAPs) of the sural or superficial peroneal nerve, denervation of plexus-innervated muscles, and the absence of paraspinal denervation. Sixty cases localized unequivocally to the sacral plexus. The majority were cancer-related, followed by traumatic, idiopathic, and iatrogenic causes.

Intravenous immunoglobulin frequency in multifocal motor neuropathy: a retrospective study of nerve conduction outcomes.

BACKGROUND: In resource-limited rural settings, patients with multifocal motor neuropathy (MMN) often present late due to limited access to neurophysiology services, by which time significant axonal damage may have occurred, rendering them less responsive to immunotherapy-yet this clinically subgroup is consistently underrepresented in randomized controlled trials. In this study, we aimed to examined the impact of intravenous immunoglobulin (IVIg) frequency on electrophysiological changes in such undertreated MMN patients. METHODS: A retrospective analysis was conducted across three rural med

Case Report: Neurolymphomatosis of the sciatic nerve as a presentation of relapsed diffuse large B-cell lymphoma.

BACKGROUND: Neurolymphomatosis (NL) is an uncommon manifestation of lymphoma caused by direct infiltration of the peripheral nervous system by lymphoma cells. Early recognition is particularly difficult when deep nerves such as the sciatic nerve are involved. Combining contrast-enhanced MRI and PET/CT to assess anatomic extent and metabolic activity, together with nerve ultrasound (US) to identify characteristic signs and guide biopsy, may improve diagnostic accuracy. CASE PRESENTATION: A 71-year-old man with systemic diffuse large B-cell lymphoma (DLBCL), histologically confirmed by gastric
