Photodynamic therapy (PDT) has been used for head and neck carcinomas with little experience in the oropharynx due to technical challenges in achieving adequate exposure. We present the case of a patient with a second right tonsil carcinoma following previous treatment with transoral robotic surgery (TORS) and postoperative chemoradiation for a left tonsil carcinoma. Repeat TORS for the right tonsil carcinoma reviewed multiple positive surgical margins. The power output from the robotic camera was modified to facilitate safe intraoperative three dimensional visualization of the tumor bed. The

The authors present the case of the patient suffering from recurrent bleeding from the palatine tonsil. A visual and tactile examination did not reveal any alteration to the tonsil. Following three recurrent bleeding episodes, we conducted an examination of the vessels (through MRI including vessel option) and consequently ruled out any possible presence of vessel alterations in the tonsil and in the adjacent tissues. Subsequently tonsillectomy was carried out. The histopathological examination revealed the presence of carcinoma in the tonsil's flesh, whereas the mucosa of the tonsil remained

A case of Burkitt's lymphoma involving the tonsil in a 10-year-old Bedouin boy, is presented. The biological behaviour and the clinical presentation of this unusual neoplasm are discussed and the English language literature is reviewed. The diagnosis was made by histological examination, electron microscopy and confirmed by immunohistochemistry. The patient showed an excellent symptomatic response to surgery and chemotherapy.

BACKGROUND: Tonsillar squamous cell carcinoma (TSCC) due to human papillomavirus (HPV) infection has seen a dramatic increase in recent years. Bilateral tonsillar squamous cell carcinoma (biTSCC) has a much lower incidence than unilateral TSCC and three main hypotheses of biTSCC pathogenesis prevail: field carcinogenesis, single-clone, and multiple HPV infections.
CASE: A 49-year-old Male with a remote history of chewing tobacco presented with symptoms of spitting up tissue and occasional hemoptysis. Physical exam showed a sole left tonsillar mass which was confirmed to be TSCC on biopsy. The

A 70-year-old man with cerebellar syndromes was clinically diagnosed as paraneoplastic cerebellar degeneration and underwent whole-body F-FDG PET/CT imaging for screening primary tumor. Intensely elevated tracer uptake in both cerebellar hemispheres was revealed, whereas no abnormality was found in MRI. Increased tracer uptake and swelling of the left tonsil and a cervical lymph node were found at the same time, suggesting neoplasm in tonsil with lymph node metastasis. Pathological examination demonstrated lymphoepithelial carcinoma of the left tonsil.

BACKGROUND: Porphyria is a condition of cutaneous photosensitivity. It is unclear if radiotherapy (RT) is safe in patients with porphyria.
METHODS: We report a patient case with uncontrolled porphyria cutanea tarda treated with chemoradiation for p16-positive oropharyngeal cancer. Given the scarcity of data on this subject, we also conducted a literature review on the topic of radiation in patients with porphyria.
RESULTS: The patient in our study did not experience any unusual acute or late toxicity, despite receiving a high dose of cutaneous radiation. We also found the majority of the lit

We report a case of squamous cell carcinoma of the tonsil incidentally detected by F-choline PET/CT. A 55-year-old male patient with a history of prostate cancer underwent F-choline PET/CT for restaging. PET/CT revealed a focal area of increased F-choline uptake corresponding to the left tonsil. No other areas of abnormal F-choline uptake were detected in the rest of the body. Based on these PET/CT findings, the patient underwent clinical examination and biopsy of the left tonsil. Histology demonstrated the presence of a squamous cell carcinoma of the tonsil.

Over a period of four years, beginning in spring 1988, a previously healthy man developed a primary squamous cell carcinoma of the tonsil, treated with radiotherapy, followed by 10 distinct, primary bronchial squamous cell carcinomas. Four of the cancers were surgically resected, all of which were positive by hybridization for human papilloma virus (type 16). Following the institution of alpha interferon, three smaller lesions disappeared and a larger one shrank in size, facilitating surgical resection. Over the following seven years no new ones have appeared. The finding of papilloma virus in

OBJECTIVE: To investigate the prognostic value of the size of primary tumor (T staging) and international prognostic index (IPI) for patients with non-Hodgkin's lymphoma (NHL) of the tonsil, and to recommend the treatment strategy for early stage patients.
METHODS: 306 patients with untreated NHL of the tonsil were reviewed. According to Ann Arbor staging classification, 35 patients had stage I, 178 stage II, 49 stage III and 44 stage IV disease. According to 1997' AJCC staging system, 29 patients had T1, 142 T2, 117 T3 and 18 T4 disease. Twelve stage I patients were given radiotherapy alone

PURPOSE: To identify prognostic parameters and evaluate the therapeutic outcomes for patients with carcinoma of the tonsillar fossa treated with three treatment modalities.
METHODS AND MATERIALS: The results of therapy are reported in 384 patients with histologically proven epidermoid carcinoma of the tonsillar fossa; 154 were treated with irradiation alone (55-70 Gy), 144 with preoperative radiation therapy (20-40 Gy), and 86 with postoperative irradiation (50-60 Gy). The operation in all but four patients in the last two groups consisted of an en bloc radical tonsillectomy with ipsilateral

INTRODUCTION: Emerging data suggest that reduction of dose to the larynx and pharyngeal constrictor may lower the risk of swallowing complications such as long-term gastrostomy dependence and aspiration. Organ avoidance becomes difficult when the primary tumor or involved nodes are present at the level of the larynx.
MATERIALS AND METHODS: Fifteen patients with Stage III-IV squamous cell carcinoma of the head and neck with high-dose target volume at the level of the larynx (but not involving the glottic larynx) were planned with whole-field IMRT (WF-IMRT), as well as a low anterior neck field

Sixty-six patients with carcinomas of the anterior tonsillar pillar and/or soft palate-uvula underwent radiation therapy. Three methods were used: (a) conventional fractionation, (b) a combination of conventional fractionation and radon seed implant, and (c) split course therapy in which two one-week courses were separated by two weeks rest. On time, dose, and fractionation (TDF) factor analysis, no significant difference in the success of tumor control was found between the methods. The determinate disease-free survival at two years was 82.4% for T1, 62.5% for T2, and 56.3% for T3--4 lesions;

Cutaneous malignant melanoma sometimes metastasizes to the upper respiratory and digestive tracts. It may cause significant local symptoms such as airway obstruction and dysphagia, and, in some cases, may represent the initial manifestation of disseminated disease. Of the 8,823 patients with cutaneous malignant melanoma seen at The University of Texas-M.D. Anderson Hospital and Tumor Institute at Houston between 1944 and 1983, metastases to this region developed in 54 patients. The most common sites involved were the tonsil, tongue, nasopharynx, larynx, and lip. Five of ten cases in which an a

Osteonecrosis of the jaw is a significant complication secondary to radiation therapy or drug therapy, most commonly bisphosphonates. Safety data regarding the administration of bisphosphonates in bone metastatic head and neck cancer patients with history of jaw irradiation are almost non-existent. In this paper, we report the case of a Head and Neck (HNC) patient, with history of radiation therapy to the mandible region, treated with intravenous bisphosphonates for bone metastases that resulted in gross, life threatening mouth hemorrhage secondary to advanced, locally invasive ONJ.

To verify whether the submuscular plane of the constrictor muscle, which is the resection margin in a transoral approach, is sufficient for securing a cancer-free deep margin in tonsil cancer patients. Retrospective analysis of medical records and pathology specimens was done in sixty-two patients who were diagnosed with tonsil cancer and underwent surgery via a combined transoral-transcervical (transoral and transcervical lateral pharyngotomy) approach. The status of constrictor muscular invasion in tonsil cancer patients according to the TN stage and its predictors was investigated. Invasion
