Combining Clear Aligner Treatment With Bone Grafting for Comprehensive Care of Missing Teeth.

In cases of missing or damaged teeth, patients who elect a fixed restorative option often choose a bridge, which typically offers convenience, affordability, and fewer appointments than dental implants. Often, such patients may have mild to moderate malocclusion that requires some movement of abutment teeth to ensure an optimal esthetic result. Additionally, socket preservation is crucial to the success of restorative efforts. By offering patients clear aligner therapy combined with bone grafting using a unique collagen plug filled with non-ceramic bone graft crystals, the author demonstrates

Treatment of replacement resorption with Emdogain--preliminary results after 10 months.

Ankylosis of traumatized teeth in children and adolescents may inhibit further development and growth of the corresponding jawbone. Therefore, ankylosed teeth should be removed. As an alternative treatment option to autotransplantation of a premolar, intentional replantation using Emdogain may be considered, provided the ankylosis is detected at an early stage or has only affected a small area of the root. Eleven ankylosed teeth presenting with replacement resorption were treated as follows: after tooth extraction, the root canal was treated extraorally and obturated by retrograde insertion of

Using 45S5 bioglass cones as endosseous ridge maintenance implants to prevent alveolar ridge resorption: a 5-year evaluation.

Bioglass cones acting as space fillers after removal of tooth roots delay the resorption of alveolar ridges. In 1987, 242 implants in 29 patients with a mean postimplantation interval of 19.9 months were reported by the authors. Bioglass cones had been fitted snugly at least 2 mm below the alveolar crest, and dentures were placed no sooner than 6 weeks following tooth removal; 2.9% had been lost and 3.7% developed dehiscences. The present report on 168 implants in 20 recalled patients (mean postimplantation interval of 63.2 months) revealed a loss of 14.3% of the implants and 7.7% of the impla

Effect of alveolar ridge preservation interventions following tooth extraction: A systematic review and meta-analysis.

AIM: The aim of this systematic review was to critically analyse the available evidence on the effect of different modalities of alveolar ridge preservation (ARP) as compared to tooth extraction alone in function of relevant clinical, radiographic and patient-centred outcomes. MATERIAL AND METHODS: A comprehensive search aimed at identifying pertinent literature for the purpose of this review was conducted by two independent examiners. Only randomized clinical trials (RCTs) that met the eligibility criteria were selected. Relevant data from these RCTs were collated into evidence tables. Endpo

Alveolar osteitis associated with methicillin-resistant Staphylococcus epidermidis.

A critical point in dentistry is the empiric prescription of broad-spectrum antibiotics that could increase the levels of antimicrobial resistance. Alveolar osteitis is one of the most common post-op- erative complications in which antibiotic use is controversial. A 35-year-old female, with pain in the right mandibular region and treated with cefixime, was diagnosed with cracked tooth syndrome and pulpitis. The tooth was extracted and a massive purulent bleeding drainage was observed. Irrigation of the socket and a new therapy with azithromycin were done. Bacteriological analysis, a specific m

Root canal filling materials spread pattern mimicking root fractures in dental CBCT images.

OBJECTIVE: To clarify the cause of artificial tooth-root fracture lines in cone beam computed tomography (CBCT) images caused by root canal filling materials and to enhance the diagnostic performance of CBCT. STUDY DESIGN: We analyzed the CBCT images of 18 extracted human teeth (9 upper central incisors and 9 lower premolars) that were filled with a size 50 gutta-percha cone and inserted into tooth sockets in a human skull. RESULTS: The CBCT images of the incisors displayed artificial lines in the oblique directions, notably the mesiobuccal, distolingual, distobuccal and mesiolingual directi…

MT1-MMP expression in the odontogenic region of rat incisors undergoing interrupted eruption.

MT1-MMP (membrane type matrix metalloproteinase-1) has been considered an important membrane-type matrix metalloproteinase involved in the remodeling process in tissue and organ development, including the processes of the tooth and root growth and dental eruption. Therefore, the aims of this study were to evaluate MT1-MMP expression in the odontogenic region, as well as the eruption rate and morphology of the lower-left rat incisor, where the eruption process was interrupted for 14 days by a steel wire attached from the center of the incisor labial face and braced to the first molar. In the in

Primary tuberculosis of the oral cavity.

Two cases of primary tuberculosis of the mouth are reported. Painless ulceration of long duration and enlargement of the regional lymph nodes that did not respond to conservative and antibiotic therapy were the main manifestations of the disease in both cases. In the first case the tubercle ulcer was located in the lower buccogingival sulcus of a 17-year-old girl. In the second case the lesion occurred as a chronic periodontal inflammation around the gingiva of the left lower second molar tooth. When the tooth was extracted, a painless ulceration appeared around the socket, which was filled by

Protective effect of ellagic acid on healing alveolar bone after tooth extraction in rat--a histological and immunohistochemical study.

OBJECTIVE: This study has attempted to evaluate the effects of ellagic acid (EA) on alveolar bone healing after tooth extraction in rats. DESIGN: Twenty-four Sprague Dawley (SD) male rats (200-250g) were selected and were anaesthetised for the extraction of upper left incisor. Then, the rats were divided into two groups, comprising 12 rats each; the first group has been considered as a control group and was given only normal saline, whereas, the second group (treated group) was intragastrically administrated with EA daily once, for 28 days. Then three rats from each group had been selected on

Letter to the editor in response to "Microbial colonization in the partially exposed nonabsorbable membrane during alveolar ridge preservation" (Braz et al., clinical oral Investigations, https://doi.org/10.1007/s00784-024-05763-7.).

OBJECTIVE: This letter addresses the recent study by Braz et al. published in Clinical Oral Investigations in 2024 ( https://doi.org/10.1007/s00784-024-05763-7 ), which reported that intentionally exposed d-PTFE membranes modulate microbial colonization in alveolar ridge preservation (ARP) sites, creating a more homogeneous and anaerobic community on both the inner and outer surfaces of the membrane. MATERIALS AND METHODS: We argue that the study's methodology may have skewed the results, particularly the inclusion of both intact and damaged extraction sockets (with loss of the buccal wall) a

Restoring the electrical microenvironment using ferroelectric nanocomposite membranes to enhance alveolar ridge regeneration in a mini-pig preclinical model.

The maintenance and incremental growth of the alveolar bone at the tooth extraction site, to achieve the required height and width for implant restoration, remains a major clinical challenge. Here, the concept of restoring the electrical microenvironment to improve the effects of alveolar ridge preservation (ARP) was investigated in a mini-pig preclinical model. The endogeneous electrical microenvironment of the dental alveolar socket was recapitulated by fabricating a biomimetic ferroelectric BaTiO3/poly(vinylidene fluoridetrifluoroethylene) (BTO/P(VDF-TrFE)) non-resorbable nanocomposite memb

The Use of Nano-Hydroxyapatite (NH) for Socket Preservation: Communication of an Upcoming Multicenter Study with the Presentation of a Pilot Case Report.

Background and Objectives: The use of biomaterials in dentistry is extremely common. From a commercial perspective, different types of osteoconductive and osteoinductive biomaterials are available to clinicians. In the field of osteoconductive materials, clinicians have biomaterials made of heterologous bones at their disposal, including biomaterials of bovine, porcine, and equine origins, and biomaterials of natural origin, such as corals and hydroxyapatites. In recent years, it has become possible to synthesize nano-Ha and produce scaffolds using digital information. Although a large variety

Management of Severe Midface Retrusion With Distraction Osteogenesis in Patients With Cleft Lip and Alveolus.

A cleft lip, palate, and alveolus is one of the most common birth defects. Depending on the type of cleft, multiple surgeries may be required throughout the growth stage. Traditionally, an orthodontic-surgical approach has been adopted to treat maxillary retrusion. Osteodistraction it is the surgical choice in patients with severe midface retrusion that require maxillary advancement. Our objective is to present our experience using this orthodontic and surgical approach.

Fresh extraction socket: spontaneous healing vs. immediate implant placement.

AIM: To evaluate the impact that immediate implant placement may have on bone remodelling in comparison with adjacent sockets left to heal spontaneously. MATERIALS AND METHODS: In a beagle dog model (N = 16 dogs), mandibular premolars were extracted, and implants were placed in each distal socket (test) with the corresponding mesial site left to heal undisturbed (control). Healing was assessed measuring both the vertical distance between buccal and lingual crest (B'L') and the width of buccal and lingual walls at different levels. Five healing periods were evaluated. Differences between means

Evaluation of Dimensional Changes and Ridge Contour Around Ovate Pontics Inserted Immediately After Extraction with Alveolar Ridge Preservation in the Esthetic Zone.

Alveolar ridge preservation (ARP) is indicated to attenuate anatomic and physiologic changes following tooth extraction. A properly contoured ovate pontic placed immediately into an extraction socket may be adequate to maintain alveolar ridge architecture for improved esthetic results. This prospective clinical study evaluated the ability of immediately placed ovate pontics in conjunction with ARP to attenuate postextraction tissue dimensional changes in the esthetic zone and maintain alveolar ridge contour. Ten patients (11 sites) completed the study. All subjects received a combination of so
