Bacterial tracheitis in children.

Bacterial tracheitis is a relatively rare cause of respiratory distress in children and is associated with a high morbidity and mortality rate. Three children who have survived this affliction are presented: one developed toxic shock syndrome in the acute phase and two developed late complications. The authors review the pediatric literature and postulate possible etiological factors. Prompt diagnosis and aggressive airway management are essential to survival. Early tracheotomy is recommended as the primary form of airway management.

Sleep apnea due to nasal valve obstruction.

Patients with the sleep apnea syndrome are being increasingly referred to the otolaryngologist as it becomes clear that obstructive lesions in the nose, nasopharynx, and oropharynx can play a part in the pathogenesis of the syndrome. Correction of the obstructive lesion obviates the need to do a tracheotomy, which has been the conventional treatment for severe cases. In this paper we discuss two cases of obstructive sleep apnea, in whom surgical correction of nasal valve area obstruction was successful in abolishing the symptoms.

The last 2 decades hav … pre-operatively, died of multiple organ failure 7 weeks postoperatively. A second child required a tracheotomy for bronchomalacia at 6 months and died at 9 months with tracheotomy tube occlusion. Both children had adequate tracheal repairs. Nonintubated Tracheal Surgery.

The article describes an anesthetic management strategy for resection of the cervical trachea due to benign stenosis without using an endotracheal tube. The strategy includes: (1) insertion of an airway stent in the stenotic area, (2) insertion of a supraglottic airway device (SGAD), and (3) advancing a jet ventilation catheter through the SGAD. The stent is removed during surgery together with the resected part of the trachea. The technique of nonintubated tracheal resection allows the surgeon to work most comfortably and helps the anesthesiologist properly maintain the patient's vital functi

[Hemangioma simplex of the larynx in an infant (author's transl)].

Symptoms of inspiratory stridor, dyspnoa and cyanosis appeared in a four month old boy. After the direct laryngoscopy and biopsy a hemangioma simples of the larynx was diagnosed. The tumor itself was successfully treated by cryosurgery. The advantages of this therapy were pointed out and the cryosurgery can be recommended in cases of larynxangioma.

Tracheal Transection After Prolonged Intubation and Tracheostomy: A Case Report.

Tracheal transection is a rare, life-threatening complication after tracheal injury. Most commonly, tracheal transection presents after blunt trauma, but iatrogenic tracheal transection after tracheotomy has not been well described. Here, we present a case without a history of trauma that presented with signs of symptoms of tracheal stenosis. She was taken to the operating room for tracheal resection and anastomosis and was incidentally found to have a complete tracheal transection intraoperatively.

A novel approach in managing challenging tracheoesophageal fistulae.

OBJECTIVE: To analyze the outcomes of an open anterior cervical approach and tospecifically describe a novel extended tracheotomy incision ("Key-hole technique") torepair H-type and other challenging tracheoesophageal fistulae (TOF) at a singletertiary pediatric center. METHOD: A retrospective chart analysis of pediatric patients (0-18 years old) who had undergone repair of TOF's between January 2006 and March 2020 were reviewed. A case series of patients who had undergone open cervical utilizing three different techniques were included. Patient demographics, surgical management and post-oper…

Success and failure in treatment of sleep apnea patients.

A variety of surgical procedures exist to treat obstructive sleep apnea in adults. Some, such as tracheotomy and maxillomandibular advancement, have very high cure rates, over 90%. These procedures have significant disadvantages, however, and there is a need to define the best combination of low morbidity procedures to provide similar success. To do this, better means to diagnose the sites of obstruction must be used and new procedures must be developed. This article reviews where we are in reaching these goals.

Acute epiglottitis.

Although acute epiglottitis is not a rare disease and may be very severe or fatal, it is one not familiar, as it should be, to all physicians dealing with children. Diagnosis may be confirmed clinically by direct or indirect examination of the epiglottis. Vaporized cool water is preferable to steam for reducing the swelling of mucosal tissues that impairs breathing. Chloramphenicol is the drug of choice, as the majority of cases of acute epiglottitis are due to H. Influenzae. Tracheotomy must be carried out if necessary to maintain an airway.

Laryngeal stenosis in epidermolysis bullosa dystrophica.

Epidermolysis bullosa dystrophica is a rare hereditary skin disease of infancy in which minor trauma causes blister formation. We report a rare case of epidermolysis bullosa dystrophica (recessive) with a stenosis of the larynx due to epiglottic deformity. We performed a tracheotomy, and we detained a trachea aperture in the long term this time. In a characteristic of a disease though stimulation to the trachea causes the erosion on a trachea mucous membrane, postoperative course was uneventful and we are going to observe it in future.

[Treatment of laryngeal paralysis in the new born (author's transl)].

In a retrospective study of 108 cases of laryngeal paralysis in new born infants (49 bilaterals and 59 unilaterals) seen during a 6 years period, the authors have analysed the evolution according to the etiology in order to propose an adequate management. The spontaneous recovery or in case of definitive paralysis the good tolerance have led the authors to propose a temporary naso tracheal intubation and later in case of bad tolerance, a laryngeal plasty rather than a classical prolonged tracheotomy.

Surgically correctable causes of sleep apnea syndrome.

This report presents six cases of obstructive sleep apnea associated with discrete anatomic abnormalities of the upper airway tract. All patients were relieved of their sleep apnea by surgical correction of the airway obstruction, and therefore permanent tracheotomy was avoided. The obstructive causative factors were nasal septal deformity, adenoidal hypertrophy, nasopharyngeal stenosis, lingual cyst, lingual tonsillar hypertrophy, and laryngeal cyst. The last-named four conditions have not previously been reported as correctable causes of sleep apnea.
