Association of Early, High Plasma-to-Red Blood Cell Transfusion Ratio With Mortality in Adults With Severe Bleeding After Trauma.

IMPORTANCE: Optimal transfusion management is crucial when treating patients with trauma. However, the association of an early, high transfusion ratio of fresh frozen plasma (FFP) to packed red blood cells (PRBC) with survival remains uncertain. OBJECTIVE: To study the association of an early, high FFP-to-PRBC ratio with all-cause 30-day mortality in patients with severe bleeding after trauma. DESIGN, SETTING, AND PARTICIPANTS: This cohort study analyzes the data included in a multicenter national French trauma registry, Traumabase, from January 2012 to July 2017. Traumabase is a prospective

[POSSIBILITY OF CORRECTION OF METABOLIC DISORDERS WITH REAMBERIN IN ACUTE PERIOD OF TRAUMATIC INJURY].

56 patients at the age of 18-60 years with severe trauma were examined. Influence of the polyelectrolytic (Reamberin)solution on an acid-base state, osmolarity and electrolytic composition of plasma in the acute posttraumatic period was evaluated. It was found that patients, who was treated by isotonic sodium chloride solution and Ringer's solution, had metabolic acidosis and hyperchloremia. In contrast, in the reamberin group 82% of patients had lower concentrations of chloride and had nothing acid-base disturbances on the second day after trauma. Reamberin didn't influence on plasma osmolari

[Recreational or professional participants in Nordic skiing. Differences in injury patterns and severity of injuries].

A total of 441 Nordic skiing injuries were observed and analysed for a period of two winter seasons. The data showed that athletes in squads (0.02/1000 days) or professional clubs (0.09/1000 days) have lower injury risks compared to those doing Nordic skiing in their leisure time (0.51/1000 days). Amongst recreational athletes, women (0.65/1000 days) sustain trauma more frequently than men (0.40/1000 days). In professional athletes, the upper limb is commonly injured, whereas there are fewer head and trunk injuries. Furthermore the injured in this group are younger and have less severe injurie

Rapid physical assessment of the injured child.

Traumatic injuries that involve the dentition and maxillofacial region may occur as isolated events in pediatric patients but are often associated with multisystem injuries and traumatic brain injuries. The primary trauma survey serves to identify and treat life-threatening injuries. The secondary trauma survey is a complete examination of the patient after the stabilization of physiological parameters. Frequent neurologic and physical assessments of the injured child are crucial to preventing deterioration. The role of the dental professional is to identify and refer for medical evaluation pa

Traumatic hemipelvectomy in children: report on 2 survivors with urological involvement.

Traumatic hemipelvectomy through the sacroiliac joint is a devastating injury, mainly because of motor vehicle accidents. Recent improvements in prehospital trauma care have increased the chances of survival for victims. Besides amputation of the lower limb, associated complications usually involve digestive and urological systems. We report on 2 pediatric patients from 2 different European countries. PATIENT 1: A 9-year-old boy suffered uprooting of his left lower limb, laceration of the rectum and anal sphincter, as well as an injury to distal urethra with partial loss of cavernous bodies. I

Shell shock and mild traumatic brain injury: a historical review.

Mild traumatic brain injury is now claimed to be the signature injury of the Iraq and Afghanistan conflicts. During World War I, shell shock came to occupy a similar position of prominence, and postconcussional syndrome assumed some importance in World War II. In this article, the nature of shell shock, its clinical presentation, the military context, hypotheses of causation, and issues of management are explored to discover whether there are contemporary relevancies to the current issue of mild traumatic brain injury. When shell shock was first postulated, it was assumed to be the product of

Profile, outcome and predictors of mortality of abdomino-pelvic trauma patients in a tertiary intensive care unit in Saudi Arabia.

INTRODUCTION: The Kingdom of Saudi Arabia (KSA) is one of countries with the world's highest number of deaths per 100,000 populations from road traffic accidents (RTAs). Numerous trauma victims sustain abdomino-pelvic injuries, which are associated with considerable morbidity and mortality. The purpose of this study was to describe profile, outcomes and predictors of mortality of patients with abdomino-pelvic trauma admitted to the intensive care unit (ICU) in a tertiary care trauma centre in Riyadh, KSA. METHODS: This was a retrospective analysis of prospectively collected ICU database. All

A Multifactorial Analysis of Trauma Outcomes: Comorbidities, Race, and Socioeconomic Status.

INTRODUCTION: Trauma patients with comorbid conditions are known to have poorer outcomes. We hypothesize that these outcomes are further influenced by race or ethnicity and socioeconomic status. METHODS: We queried patient records in the Trauma Quality Improvement Program database from 2017 to 2019 and assessed those with selected comorbidities: chronic kidney disease (CKD), diabetes, cardiac comorbidities (angina pectoris, congestive heart failure, myocardial infarct, and hypertension), and chronic obstructive pulmonary disease (COPD). We used multivariate logistic and linear regression mode

Relative incidence of needle EMG abnormalities in the anterior tibial and gastrocnemius muscles.

Foot drop resulting from weakness of the anterior tibial muscle is commonly encountered in clinical practice. We analyzed the incidence of needle electromyographic (EMG) abnormalities in the anterior tibial and gastrocnemius muscles from traumatic injuries to the sciatic nerve and also from different non-traumatic lower motor neuron conditions. In both traumatic and non-traumatic cases, the anterior tibial muscle showed relatively more EMG abnormalities compared to the gastrocnemius. It appears that the anterior tibial muscle is more susceptible to injury from a variety of pathologic condition

Comparison of medical complications following nontraumatic and traumatic spinal cord injury.

BACKGROUND: Nontraumatic spinal cord injury (NT/SCI) has been shown to represent a significant proportion of individuals admitted for SCI rehabilitation. Although medical complications of patients with traumatic SCI (T/SCI) have been well studied, there is a paucity of literature regarding those with nontraumatic SCI. Our objective was to compare the incidence of secondary SCI medical complications in patients with nontraumatic and traumatic SCI. DESIGN: A 2-year prospective data comparison of 117 patients with SCI admitted to a regional SCI rehabilitation unit and tertiary university medical

Emergency central venous catheterization during resuscitation of trauma patients.

Central venous catheterization during resuscitation of trauma patients remains controversial. Such catheterizations performed at the UMDNJ-Robert Wood Johnson Medical School at Camden Cooper Hospital/University Medical Center (Camden, NJ) trauma center for the period January 1, 1988 to December 31, 1989 were retrospectively reviewed. Patients with underlying hemothorax, pneumothorax, or resuscitative thoracotomy were excluded. There were 269 catheters inserted using the Seldinger technique. Catheters were placed via the internal jugular or subclavian veins by attending traumatologists or emerg

Died of wounds: a mortality review.

OBJECTIVES: Combat casualty care is a complex system involving multiple clinicians, medical interventions and casualty transfers. Improving the performance of this system requires examination of potential weaknesses. This study reviewed the cause and timing of death of casualties deemed to have died from their injuries after arriving at a medical treatment facility during the recent conflicts in Iraq and Afghanistan, in order to identify potential areas for improving outcomes. METHODS: This was a retrospective review of all casualties who reached medical treatment facilities alive, but subseq

Alcohol consumption, helmet use and head trauma in cycling collisions in Germany.

Cycling, being easy, inexpensive and healthy, is becoming one of the most popular means of transport. Cyclists, however, are among the most vulnerable road users in traffic collisions. The aims of this study were to establish which cyclist and cycling accident characteristics are associated with alcohol consumption and helmet use in Germany and to identify risk factors related to head trauma sustained in cycling accidents. The source used for the present analysis was the database of the German in-depth accident study (GIDAS). All cyclists who had been involved in a road accident between 2000 a

Epidemiology and outcomes of non-compressible torso hemorrhage.

BACKGROUND: Non-compressible torso hemorrhage (NCTH) is the leading cause of potentially preventable death in military trauma, but the civilian epidemiology is unknown. The aim of this study is to apply a military definition of NCTH, which incorporates anatomic and physiological criteria, to a civilian population treated at trauma centers in the US. METHODS: Patients (age >16 y) from 197 Level 1 trauma centers (approximately 95% of all US Level 1 centers) in the National Trauma Data Bank 2007-2009 that sustained a named torso vessel injury, pulmonary injury, grade IV solid organ injury, or pe
