Prevalence of complications of open tibial shaft fractures stratified as per the Gustilo-Anderson classification.

The aim of the present study was to comparatively analyse certain outcome measures of open tibial fractures, stratified per grade of open injury and method of treatment. For this purpose, a systematic review of the English literature from 1990 until 2010 was undertaken, comprising 32 eligible articles reporting on 3060 open tibial fractures. Outcome measures included rates of union progress (early union, delayed union, late union and non-union rates) and certain complication rates (deep infection, compartment syndrome and amputation rates). Statistical heterogeneity across component studies wa

Adverse effects of 21 antidepressants on sleep during acute-phase treatment in major depressive disorder: a systemic review and dose-effect network meta-analysis.

STUDY OBJECTIVES: Sleep-related adverse effects during acute treatment with antidepressants undermine adherence and impede remission. We aimed to address subtypes of sleep-related adverse effects and depict the relationship between dose and sleep-related adverse events. METHODS: We searched PubMed, Embase, Cochrane Central Register of Controlled Trials, and Web of Science for double-blind randomized controlled trials of depression published before April 30th, 2023. Eligible studies reporting sleep-related adverse effects during short-term monotherapy were included. The odds ratios (ORs) for s

Interferon in the Treatment of Multiple Myeloma.

BACKGROUND: The role of interferon (IFN) in the treatment of multiple myeloma has been investigated for nearly two decades. The mechanisms underlying antitumor activity of IFN may be mediated by antiproliferative and immunomodulatory effects. The benefits of treatment remain controversial, and guidelines for the use of IFN in myeloma are needed. This review evaluates available data on the impact of IFN therapy on multiple myeloma. METHODS: A MEDLINE search of published prospective, randomized trials of IFN in multiple myeloma provided the data included in this review, as well as selected abstr

Regional differences in treatment rates for patients with chronic hepatitis C infection: Systematic review and meta-analysis.

BACKGROUND & AIMS: Treatment rates with interferon-based therapies for chronic hepatitis C have been low. Our aim was to perform a systematic review of available data to estimate the rates and barriers for antiviral therapy for chronic hepatitis C. METHODS: We conducted a systematic review and meta-analysis searching MEDLINE, SCOPUS through March 2016 and abstracts from recent major liver meetings for primary literature with available hepatitis C treatment rates. Random-effects models were used to estimate effect sizes and meta-regression to test for potential sources of heterogeneity. RESUL

Ventilation heterogeneity and lung tissue abnormalities in children after hematopoietic stem cell transplantation.

BACKGROUND: Hematopoietic stem cell transplantation (HSCT) is a life-saving treatment for children with various malignant and nonmalignant hematological disorders. We aimed to evaluate the abnormalities after autologous and allogeneic HSCT in children by monitoring ventilation heterogeneity derived from nitrogen multiple breath washout (NMBW) data and lung ultrasonography. In addition, we aimed to find the effect of drugs used for conditioning and/or graft-versus-host disease prophylaxis on high ventilation heterogeneity in children after HSCT. METHODS: We carried out a cross-sectional study

Heterogeneity in precision oncology.

Precision oncology is a rapidly evolving concept that holds great promise in cancer treatment. However, a cancer complexity attributed to genomic and acquired tumour heterogeneity limits treatment effectiveness and increases toxicity. These limitations refer to both systemic therapies and radiotherapy, which are two mainstays of non-invasive cancer treatment. By understanding cancer heterogeneity and utilising advanced tools to personalise treatment strategies, precision oncology has the potential to revolutionise cancer care. In this article, we review the current status of precision oncology

Super pulsed thulium fiber laser outcomes in retrograde intrarenal surgery for ureteral and renal stones: a systematic review and meta-analysis.

BACKGROUND: Laser lithotripsy using a thulium fiber laser (TFL) has become an effective treatment option for small renal stones with low complication rates. TFL has a higher absorption coefficient, smaller fibers, and better pulse rate capability. METHODS: We conducted a systematic review and meta-analysis to evaluate the published evidence regarding TFL's lithotripsy performance in retrograde intrarenal surgery (RIRS), for which we primarily assessed the outcomes of stone-free rate, operation time, and complications. We searched different databases from inception to April 2023. We assessed t

Ceftolozane/tazobactam for the treatment of bacteremia: a systematic literature review (SLR).

BACKGROUND: Bloodstream infections (BSIs), or bacteremia, are responsible for considerable disease burden. Increasing rates of antibiotic resistance and delays in selection of appropriate treatment lead to increased morbidity, mortality, and costs. Due to limitations of current standard treatments, especially for bacteremia caused by resistant pathogens, a systematic literature review (SLR) was conducted to understand the utilization of ceftolozane/tazobactam (C/T) in bacteremia. METHODS: Electronic database searches of EMBASE®, MEDLINE®, CCTR and Northern Lights, as well as hand searches of

Assessing the steroid-sparing effect of biological agents in randomized controlled trials for lupus: a scoping review.

Prompt disease control of flares in patients with systemic lupus erythematosus (SLE) is a priority in treatment strategy planning. However, the long-term dosage-related collateral effects of glucocorticoids (GCs) have pushed researchers towards the identification and utilization of novel biological agents that could both induce and maintain low disease activity and remission, especially in the context of lupus nephritis (LN). This scoping review aims at assessing the current evidence of the potential steroid-sparing effect of biologic therapies by reviewing phase II and phase III randomized, p

Prevalence and associated factors of delay in seeking malaria treatment among under five children in the Horn of Africa: A systematic review and meta-analysis.

INTRODUCTION: Malaria is a global public health problem, particularly in sub-Saharan African countries. It is responsible for 90% of all deaths worldwide. To reduce the impact and complications associated with delayed treatment of malaria among children under five, comprehensive evidence about the magnitude and determinants of delayed treatment for malaria could be the solution. But there are no national-level studies in the Horn of Africa for decision-makers. OBJECTIVE: To assess the prevalence and associated factors of delay in seeking malaria treatment among under-five children in the Horn

Atherosclerotic Cell Fates: A Single-Cell View of ER Stress.

The role of endoplasmic reticulum (ER) stress in atherosclerosis has long been recognized, but whether it acts as a uniform pathological signal across all cell types has remained unclear. With the advancement of single-cell sequencing technology (scRNA-seq), we can analyze this problem at the resolution of a single cell subpopulation. In this review, we have synthesized the recent evidence from single-cell studies and proposed the "cell type-specific differential response" mode. We believe that endoplasmic reticulum stress does not universally have a pro-atherosclerotic effect. On the contrary

Separating Ranking Signal, Incremental Feature Value, and Evaluation-Induced Optimism in Breast MRI Treatment-Effect Analysis: Foundation Model and Radiomic Representations.

Treatment-effect prioritization, incremental imaging value, and evaluation-induced optimism require separate assessment. After a reproducibility audit identified a nonstandard ranking statistic, we reanalyzed pretreatment breast dynamic contrast-enhanced MRI from the randomized I-SPY2 cohort within MAMA-MIA using standard centered, doubly robust rank-weighted average treatment effect (RATE) analysis targeting the area under the targeting operator characteristic (AUTOC). The human epidermal growth factor receptor 2-negative cohort (n = 739) was randomly divided into training (n = 370) and indep
