Primary Staphylococcus aureus urinary tract infection: the role of undetected hematogenous seeding of the urinary tract.

Staphylococcus aureus (SA) bacteriuria may accompany SA bacteremia, but primary SA urinary tract infection (UTI) may also occur. Our clinical observation of SA UTIs following intravenous catheter-related phlebitis lead us to review hematogenous and ascending route-related risk factors in patients with primary SA UTIs. The charts from all patients with SA UTIs over a 1.5-year period were reviewed for concurrent or recent hospitalization, intravenous catheterization, and for known UTI risk factors. Patients with concurrent SA bacteremia were excluded. Patients with Escherichia coli UTIs during t

Suprapubic versus transurethral catheterization for bladder drainage in male rectal cancer surgery (GRECCAR10), a randomized clinical trial.

BACKGROUND: Bladder drainage is systematically used in rectal cancer surgery; however, the optimal type of drainage, transurethral catheterization (TUC) or suprapubic catheterization (SPC), is still controversial. The aim was to compare the rates of urinary tract infection on the fourth postoperative day (POD4) between TUC and SPC, after rectal cancer surgery regardless of the day of removal of the urinary drain. METHODS: This randomized clinical trial in 19 expert colorectal surgery centers in France and Belgium was performed between October 2016 and October 2019 and included 240 men (with n

Urethral lengthening with anterior bladder wall flap for urinary incontinence: a new approach.

Urethral irregularity (impeding catheterization) and failure to achieve continence are common complications in surgery for urinary incontinence. We describe a surgical technique using an anterior bladder wall flap that is sutured to the posterior wall in an onlay fashion creating a flap valve mechanism. Experimental work in dogs demonstrated a significant increase in the leak point pressure in the surgical group when compared to controls (p = 0.019). Voiding cystourethrography and bladder inspection demonstrated an anterior flap valve with no fistula formation in all animals. Histological exam

Urinary bladder perforation in a very low birth weight infant. A case report.

Perforation of the urinary bladder is a rare complication with a considerable degree of morbidity and mortality in modern neonatal intensive care units. We report a case of urinary bladder perforation, which probably was triggered by prolonged urinary retention secondary to morphine infusion, using credé maneuver on an already over-distended bladder, and inserting an umbilical catheter as urethral catheterization in an extremely low birth weight infant.

Nontraumatic elevation of prostate specific antigen following cardiac surgery and extracorporeal cardiopulmonary bypass.

We recently treated a number of patients with markedly elevated prostate specific antigen (PSA) levels associated with acute urinary retention in a post-cardiac surgery setting. A controlled study was conducted to determine if this elevation is secondary to trauma from urethral catheterization or more directly associated with the cardiac surgery and extracorporeal bypass. In 68 patients undergoing cardiac surgery serum PSA levels were determined preoperatively and 12 to 18 hours postoperatively (after urethral catheterization). The control patients were 23 men undergoing evaluation for chest p

Efficacy and safety of transcutaneous electrical nerve stimulation combined with clean intermittent catheterization for neurogenic bladder dysfunction in patients with cauda equina syndrome.

PURPOSE: Cauda equina syndrome (CES) due to lumbar disk herniation often leads to bladder dysfunction, severely affecting patients' quality of life. This study aims to evaluate the effectiveness of percutaneous electrical stimulation (TENS) combined with clean intermittent catheterization (CIC) for improving bladder function in CES patients. METHODS: A total of 40 patients with bladder dysfunction caused by CES who underwent posterior lumbar spinal fusion from January 2020 to December 2022 were included. Based on the method of catheterization, patients were divided into the CIC or indwelling

[Using Nursing Digital Technology to Reduce the Rates of Catheter-Associated and Non-Catheter-Associated Urinary Tract Infection].

BACKGROUND & PROBLEMS: Urinary tract infection (UTI), the most common type of healthcare-associated infection, may result from negligence of care regardless of the presence or absence of catheterization. The UTI rate in our unit was .069%, which was the highest among all of the wards in the hospital. PURPOSE: This project aimed to reduce the UTI rate to 0.37%. METHODS: In order to achieve this goal, we amended the SOPs and audit table of catheterization, enhanced access to catheterization and perineal-wash-related instructional videos using quick response (QR) codes, encouraged and reminded

Management of Neurogenic Bladder in Children Following Spinal Injury: A Single-Center Experience.

Neurogenic bladder () in children commonly result from spinal injuries or congenital spinal malformations that disrupt sacral and spinal cord neural pathways. Such damage impairs detrusor contraction and sphincter relaxation, causing high intravesical pressures, detrusor-sphincter dyssynergia, vesicoureteral reflux, recurrent urinary tract infections, and incontinence. These dysfunctions can compromise renal function, growth, and quality of life. Clean intermittent catheterization (CIC) remains the gold standard for management, enabling low-pressure voiding, preventing renal deterioration, red

Bladder contraction strength parameters poorly predict the necessity of long-term catheterization after a pubovaginal rectus fascial sling procedure.

PURPOSE: We verified if the necessity of long-term catheterization after a pubovaginal rectus fascial sling procedure can be predicted by preoperatively determined bladder contraction strength. MATERIALS AND METHODS: We analyzed the files of 58 consecutive nonneurogenic women who underwent a sling procedure for severe or refractory stress urinary incontinence. Preoperative urodynamic measurements in these patients were re-analyzed using the power factor WF and the bladder contractility index as bladder contraction strength parameters. Catheterization was considered necessary if the patient se

Continent urinary diversion: variations on the Mitrofanoff principle.

In 1980 Mitrofanoff described a continent urinary diversion, consisting of the bladder with a surgically closed bladder neck emptied by intermittent catheterization of the appendix, which had its distal end implanted in a submucosal nonrefluxing fashion in the bladder and its proximal end brought out as an inconspicuous stoma. Alternatively, Mitrofanoff described a high transureteroureterostomy in which the distal end of the ureter was brought out as a catheterizable stoma. The theme of this approach is the creation of an adequate capacity storage reservoir that is emptied intermittently by th

The Effect of Bladder Catheterization on Ambulation and Venous Thromboembolism Following Total Knee Arthroplasty: An Institutional Analysis.

BACKGROUND: Although intermittent catheters are immediately removed, indwelling catheterization may lead to decreased ambulation and participation in physical therapy, critical components to post-total knee arthroplasty (TKA) management. Therefore, this study aimed to compare the effect of catheterization treatments on (1) postoperative ambulation distances, (2) deep vein thromboses (DVTs), and (3) pulmonary emboli (PEs) following TKA. METHODS: A total of 9123 prospectively collected primary TKA patients were assessed based on postoperative catheter status. Patient demographics, Charlson Como

Incontinence and clean intermittent catheterization following spinal cord injury.

Clean intermittent catheterization (CIC) is commonly used by individuals with neuropathic bladder dysfunction caused by spinal injury. Although the incidence of urologic complications such as infection, calculi, and urethral erosion have been documented for patients managed by CIC, little is known about the incidence or characteristics of urinary incontinence among these patients. One hundred and fifty patients who had been discharged on CIC were investigated via a structured interview. Contrary to speculations in the literature, only 54% of those individuals who performed CIC experienced any

Overuse of the indwelling urinary tract catheter in hospitalized medical patients.

BACKGROUND: The indwelling urinary tract catheter (IUTC) is an important aspect of medical care. We studied the prevalence of the unjustified use of the IUTC in hospitalized medical patients and identified situations associated with its unjustified use. METHODS: This prospective study involved 202 patients admitted to either the medical intensive care unit (n = 135) or the medical floors (n = 67) of a tertiary care university hospital who were catheterized during the hospital admission. An independent observer assessed the indication of initial catheterization by chart review and interview wi
