This article describes the findings in 228 women with urinary incontinence consecutively referred to the outpatient clinic at the University Hospital in Trondheim. A urotherapist used a structured questionnaire to record the history and also gathered other relevant information, prior to the examination by the specialist in urogynaecology. Based on clinical observations of a naturally occurring phenomenon, a water-prompting procedure was used to facilitate toilet training with a 9-year-old boy with profound mental retardation. Other frequently used toilet-training procedures were either ineffective or were associated with increased self-injury. The water-prompting treatment package may increase continence by eliciting and reinforcing urination under specific stimulus conditions.

To determine the prevalence of urinary incontinence, a questionnaire was administered to 2,911 women by 60 general practitioners, in April and May 1989. The first 50 women seen by the physician in his practice were included in the study. 1,075 women out of 2,911 (37%) declared the presence of episodes of incontinence. The authors designed a new pad for urinary incontinence. It is composed of a cloth of acrylonitrile copper sulfide and another cloth of iron-phthalocyanine. In vitro experiments showed that acrylonitrile copper sulfide cloth inhibited the growth of most bacteria causing urinary tract infection, and that iron-phthalocyanine cloth effectively eliminated bad-smelling gases. The use of this pad relieved diaper-dermatitis and diminished offensive smells from feces and urine. This pad was well received by nurses and helpers taking care of incontinent elderly people.

One hundred and thirteen patients with genuine stress incontinence were treated by a modified Burch Colposuspension after having failed an adequate trial with medical management. Three-quarters of patients were evaluated with pre-operative video-cystometry (video-C.M.G.) while a third underwent post-operative video-cystometry. Flow rates were generally lower after surgery with 3 of the 113 patients developing post-surgery bladder instability. The reliability of a patient's history has been challenged in the preoperative evaluation of stress urinary incontinence. In this study, 122 consecutive patients referred to our Gynecology/Urology clinic were evaluated and an additional 32 control patients (continent women with no urinary symptoms) were evaluated. All patients answered a detailed 64-item questionnaire, and all patients received a complete urodynamic evaluation. Urinary incontinence is a prevalent problem affecting elders of both sexes in community and institutional settings. Much of this problem is reversible. Detailed history, physical, and a residual urine test can provide sufficient information to begin treatment. Out of 534 inquired and examined women 151 (28.2%) of them had urinary stress incontinence and 383 (71.7%) women were continent. Of 151 incontinent women 75 (49.6%) of them repeatedly developed the inflammation of the urinary bladder and of 383 continent women data on chronic cystitis were registered in 74 (19.3%). These data pointed to the fact that the previous repeated inflammation of the urinary bladder and then the appearance of urinary stress incontinence could be regarded as one of etiologic factors in the occurrence of urinary stress incontinence.

Fifty-two patients with objective evidence of pressure equalization incontinence and detrusor instability were evaluated retrospectively to compare nonsurgical modes of therapy with retropubic surgery. Two hundred and twenty-six female patients undergoing investigation for urinary incontinence completed the Eysenck Personality Inventory questionnaire. The results of the questionnaire were correlated with the symptomatology and urodynamic findings. Incontinence and neuroticism were found to be related. In a randomized double-blind manner, 43 women with grade I and II stress urinary incontinence were treated with either phenylpropanolamine p.o. 50 mg twice daily (Rinexin, 1 tablet b.i.d.) or placebo during two weeks.
