Statin Intensity and the Risk of Noninfectious Uveitis.

PURPOSE: To evaluate whether the 5-year risk of incident noninfectious uveitis (NIU) is associated with the intensity of statin therapy in patients with hyperlipidemia. DESIGN: Retrospective propensity score-matched cohort study. SUBJECTS: Patients diagnosed with hyperlipidemia within a nationwide federated electronic medical record network (TriNetX). Patients initiating high-, medium-, or low-intensity statin therapy were compared with 2 mutually exclusive control cohorts: an active comparator control (proton pump inhibitor users) and an unexposed control cohort. Patients with prior uveitis…

Uveitis in Psoriatic Arthritis: A Comprehensive Review.

Psoriatic arthritis (PsA) belongs to the spectrum of spondyloarthritides, primarily affecting skin and joints. Apart from skin involvement, other extra-articular manifestations can coexist. Uveitis, although not very frequently encountered, is one of the most serious of them, necessitating prompt diagno- sis and proper treatment to prevent irreversible sight-threatening complications. Psoriatic arthritis related uveitis is usually unilateral, characterized by anterior segment inflammation, with the absence of redness or pain making it possible to miss the diagnosis. This review gives a compreh…

Uveitis: A snapshot in the MOG antibody spectrum.

Despite the commonly observed association of anti-myelin oligodendrocyte glycoprotein (MOG) antibodies with bilateral optic neuritis, their connection to uveitis is largely unexplored. The presented case involves a 41-year-old male with uveitis and bilateral optic neuritis, subsequently diagnosed with myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). This case, characterized by bilateral optic neuritis associated to anti-MOG antibodies and the concurrent onset of unilateral anterior uveitis, provides further evidence concerning the features of intraocular inflammation in…

Tubulointerstitial Nephritis and Uveitis in a Pediatric Patient.

Tubulointerstitial nephritis and uveitis syndrome (TINU) is a rare autoimmune disease. It is characterized by uveitis and kidney damage. The presentation of uveitis is typically anterior and bilateral, while the renal lesion is an acute interstitial nephritis. We report a case of an adolescent diagnosed with this disease, who presented with ocular and constitutional symptoms. An ophthalmologic examination confirmed the diagnosis of uveitis, and subsequent systemic evaluation revealed impaired renal function. The findings of the renal biopsy established the diagnosis after ruling out other syst…

Prevalence of uveitis in syphilis patients in Taiwan.

Few population-based studies have looked at the risk of uveitis among syphilis patients. Our study addresses the knowledge gap by reporting on uveitis risk in syphilis patients through a retrospective cohort study. The Taiwan National Health Insurance database was used for this study, covering the period from January 1st, 2009, to December 31st, 2020. We created a 1:4 propensity score matched cohort between the syphilis patients and controls, which accounted for gender, age, and comorbidities. The primary endpoint was the incidence of newly recorded uveitis. The assessment of uveitis risk in s…

An unusual case of eosinophilic uveitis in a cat.

An 8-year-old female spayed domestic short-haired cat was examined for recurrent unilateral anterior uveitis of 5 month's duration. No underlying cause was found on infectious disease screening. The cat also had a 4-year history of allergic or immune-mediated skin disease that was controlled with corticosteroid injections followed by long-term oral cyclosporine therapy. Medical management with frequent topical anti-inflammatory drugs (prednisolone acetate 1% suspension, diclofenac 0.1% solution) controlled the intraocular inflammation; however, the uveitis would relapse when therapy was discon…

[Intermediate Uveitis in Adults and Children - Update].

Intermediate uveitis is a rare disease. Interdiscliplinary investigations to identify or exclude an associated systemic disease (in Central Europe, this would primarily mean sarcoidosis and multiple sclerosis) should be initiated even at the first manifestation of disease. Therapy should be started in those patients with marked inflammatory activity or secondary complications and primarily encompasses local and systemic corticosteroids, although some patients need second line steroid sparing systemic DMARD therapy (DMARD: disease-modifying antirheumatic drug).

Biologic Therapies in Sarcoidosis and Uveitis: A Review.

Sarcoidosis and uveitis are chronic inflammatory conditions with potentially debilitating effects on quality of life. Steroids form the mainstay standard therapy in both conditions. Biologic agents are considered to be appropriate alternatives for treatment in steroid-refractory sarcoidosis and uveitis due to the role of tumor necrosis factor (TNF) in mediating the inflammatory cascade seen in both conditions. We performed a thorough literature search using PubMed to compare the extent of use, efficacy, and safety profile of individual anti-TNF agents in the management of these conditions. Our…

Emerging drugs for the treatment of noninfectious uveitis.

Introduction: Uveitis is a leading cause of visual impairment and a significant burden of blindness. Although corticosteroids and conventional immunosuppressive agents have been successfully used, these are non-specific, and their long-term use may induce significant adverse effects. Areas covered: This article discusses existing local and systemic applied treatments for ocular inflammation including corticosteroids, non-biologic, and biologic disease-modifying anti-rheumatic drugs (DMARD). Potential drugs being studied in clinical trials are introduced for both local and systemic use. Expert …

Evaluation of Traumatic Uveitis in the Emergency Department.

Eye trauma is a common presenting complaint in the emergency department. Correctly identifying the underlying pathology in eye trauma is critical to developing appropriate treatment plans that reduce the risk of long-term sequelae, and reduce or eliminate threats to vision. The clinical evaluation and diagnosis can be complex, and the presentation of serious eye conditions can be similar to that of more benign conditions. This article focuses on traumatic uveitis. It addresses the elements of history and examination that should suggest uveitis as the cause of pain and redness following trauma …
