A prospective randomized trial of 40 duodenal ulcer patients is reviewed. The patients had one of four operations (selective vagotomy, proximal gastric vagotomy, selective vagotomy plus pyloroplasty, or proximal gastric vagotomy plus pyloroplasty). The gastric emptying of a hypertonic fluid meal was assessed before and three to four months after operation. The results of surgical treatment of 118 patients with duodenal ulcer complicated with stenosis that have been operated on from 1981 to 1992 have been analysed. Selective proximal vagotomy (SPV) and duodenoplasty was performed in 58 (49.2%) patients (study group). SPV and gastroduodenal anastomosis by the method of Jabulay was performed in 60 (50.8%)-control group. The rate of postoperative morbidity in the study group-5.1%, in the control group-8.4%. There is a sound basis for performing EHSV with reference to the "areas of vagotomy" as described and for wider application of EHSV. Most patients with complications of duodenal ulcer disease can be treated successfully by EHSV, with a small incidence of recurrent ulcers. This is a report on a long-term follow-up study of the results of an extended variant of proximal selective vagotomy in patients undergoing clinical approbation over periods ranging from 7 to 9 years. The details of technical implementation of the variant are described. At zero lethality and complete recovery of the patients followed up, the late results are estimated as excellent.

The article studies the remote results of vagotomy in treatment of 212 servicemen with ulcer of duodenum. Laboratory, endoscopic, roentgenological, pH-metric, manometric, radioimmunological and morphological researches were used before and after operations. Micromorphometry produced a graphic hardware for computer in a study of gastric mucosal hyperemia in rats after proximal selective vagotomy. Hyperemia was found in intervals of 1 day and 7 days after operation in gastric body, whereas lasted only 1 day in pyloric antrum [after 7 days the picture was similar to the sham-operated animals]. Histology did not help to distinguish between a passive and active hyperemia.

In patients with hyperacidic duodenal ulcer the hyperproduction of hydrochloric acid was shown to occur both during the nervous-reflectory and humoral phases of secretion.
